سابقه و هدف: سپسیس یک وضعیت بالینی بحرانی است که در اغلب موارد در اثر عفونت های باکتریایی، ایجاد شده و با مرگ و میر بالایی همراه است. پیش آگهی این بیماری با درمان به موقع با آنتی بیوتیک ها ارتباط داشته و شرط درمان سریع آن، تشخیص به موقع می باشد. از آنجا که تشخیص قطعی سپسیس با انجام کشت خون صورت می گیرد و این تست معمولا زمان بر است، برخی ازنشانگرهای بیوشیمیایی برای تشخیص سریع سپسیس مورد بررسی قرار گرفته اند که پروکلسیتونین (PCT) یکی از این نشانگرهاست. این تحقیق با هدف تعیین حساسیت و ویژگی PCT در تشخیص سپسیس نوزادی انجام شد.
مواد و روش ها: در یک مطالعه مقطعی در بیمارستان شهدای تجریش، 76 نوزاد مشکوک به سپسیس وارد مطالعه شدند. از آنها نمونه خون برای کشت خون و اندازه گیری PCT اخذ گردید و پس از مقایسه مقادیر PCT با نتایج کشت خون، حساسیت و ویژگی PCT در تشخیص سپسیس نوزادی و همچنین میزان توافق آن با کشت خون تعیین شد. یافته ها: در 8 نوزاد (5/10%) کشت خون مثبت بوده و نتیجه تست PCT در 45 نوزاد (2/59%) مثبت گزارش شد. حساسیت و ویژگی PCT در تمام نوزادان به ترتیب 75% و 6/42%، در نوزادان 48 ساعته و کمتر 100% و 5/22% و در نوزادان با سن بالاتر از 48 ساعت 4/71% و 4/71% بود. نتیجه گیری: مجموع یافته های این مطالعه نشان داد که از PCT می توان برای تشخیص سپسیس در تعداد قابل توجهی از نوزادان مبتلا استفاده کرد. لیکن ویژگی آن پایین بوده و امکان تشخیص افراد سالم با محدودیت همراه است. محققین این مطالعه، از ارزش PCT به عنوان جزئی از بررسی تشخیص سپسیس حمایت کرده و انجام مطالعات بیشتر در این زمینه را برای رسیدن به نتایج قطعی تر لازم می دانند.
Background and Aim: Sepsis is a critical condition witch is mostly by bacterial infection with high rate of mortality. Prognosis of Sepsis depends on early diagnose and treatment by proper antibiotics. Since definite diagnosis of sepsis is a positive blood culture and this test needs a long time to determined، other biochemical markers like procalcitonin (PCT) has been scrutinized. In this study we tried to scrutinize sensitivity and specificity of PCT in diagnosis of neonatal sepsis. Materials and methods: 76 infants with clinical suspicion of neonatal sepsis were included in this cross-sectional and prospective study. After taking detailed history and physical examination، serum PCT concentrations were measured by a semi quantitative method، and the results of PCT test were compared with blood cultures results to determine sensitivity and specificity of PCT assay in diagnosis of neonatal sepsis. Results: In 8 infants (10.5%) blood cultures were positive and 45 infants (59.2%) had positive PCT test (PCT>0.5 ng/mL). Sensitivity and specificity of PCT in all infants were 75% and 42.6%، respectively. In infants aged 48 hours and younger، sensitivity and specificity of the test were 100% and 22.5%، respectively. In infants aged older than 48 hours، both the sensitivity and specificity of the test were 71.4%. Conclusion: PCT is a useful marker as a part of evaluations for diagnosis of neonatal sepsis. It is an easy method for the work-up of sepsis، but it is not sufficient as a sole marker of diagnosis of sepsis. Studies with larger sample sizes are required to establish value of PCT.
