اهداف: با بالارفتن امید به زندگی، شاهد افزایش بیماریهای دژنراتیوی مثل استئوآرتریت هستیم. تعادل عامل مؤثری بر تحرک و فعالیتهای روزمره زندگی افراد مبتلا به استئوآرتریت شدید زانو است؛ بیمارانی که به این دلیل کاندیدای جراحی جایگذاری مفصل زانو میشوند. درمانهای سنتی مرسوم بر کاهش درد و افزایش قدرت عضلات استوار هستند و به صورت موضعی در همان ناحیه زانو به کار میروند. بهکارگیری نوروفیدبک روش نوینی برای بهبود تعادل در سطح سیستم عصبی مرکزی است. هدف از این مطالعه مقدماتی، بررسی تأثیر نوروفیدبک بر تعادل پوسچرال و توجه پایدار بیماران مبتلا به استئوآرتریت زانو پس از جراحی جایگذاری کامل دوطرفه مفصل زانو است.
روش بررسی: تحقیق حاضر پژوهشی نیمهتجربی است که به صورت کارآزمایی تکگروهی قبل و بعد انجام شد. جامعه آماری پژوهش با روش نمونهگیری در دسترس، همه افراد دارای عمل جراحی دوطرفه مفصل زانو بودند که در بهار و تابستان ۱۳۹۶ به کلینیک کاردرمانی دانشکده توانبخشی دانشگاه علوم پزشکی شهید بهشتی مراجعه کرده بودند. در کل ۸ بیمار با میانگین سنی ۶۷/۵ با جایگذاری دوطرفه مفصل زانو (در میانگین زمانی سه ماه از جراحی تا شروع مداخله) بر اساس معیارهای ورود و خروج، وارد این مطالعه شدند. معیارهای ورود عبارت بود از: سن بالای ۵۵ سال؛ رضایت آگاهانه؛ عدم اختلال شناختی (کسب حداقل نمره ۲۰ در ارزیابی کوتاه وضعیت روانی (MMSE)؛ عدم سابقه جراحی در سایر مفاصل اندام تحتانی؛ داشتن پروتز سیمانی؛ مبتلا نبودن به بیماریهای حاد عروقی قلب، فشارخون بالای کنترلنشده، نقایص میدان بینایی و اختلاف طول اندام (بیشتر از یک سانتیمتر)؛ معتاد نبودن به مواد مخدر و مشروبات الکلی؛ عدم اختلالات دهلیزی که ممکن است تعادل را مختل کند؛ توانایی راه رفتن مستقل و دریافت نکردن مداخله نوروفیدبک پیش از این مطالعه. معیارهای خروج از مطالعه نیز غیبت بیش از دو جلسه و عدم رضایت از شرکت در پژوهش بود. بیماران هشت جلسه با برنامه تعادلی، نوروفیدبک دریافت کردند. ارزیابی و ثبت دادهها در سه زمان قبل از شروع مداخله، چهارمین جلسه و در پایان مداخله، با آزمونهای توجه گوشبهزنگی از سیستم تست وین و آزمون ثبات دینامیک دستگاه تعادلسنج بایودکس سطح شش انجام شد. برای تحلیل دادهها از نرمافزار SPSS نسخه ۲۲ و آزمونهای آماری تحلیل اندازهگیریهای مکرر، تی نمونههای زوجی و تی دونمونهای وابسته استفاده شد. سطح معناداری کلیه آزمونها ۰/۰۵ تعیین شده است.
یافتهها: نتایج نشاندهنده بهبود آماری معناداری (P نتیجهگیری: نتایج این مطالعه مقدماتی حاکی از وجود شواهد اولیهای بود که نشان میداد احتمالاً در ۴ ماه اولیه پس از جراحی، نوروفیدبک میتواند باعث بهبود سریع عملکرد ثبات پوسچرال در زنان مبتلا به استئوآرتریت زانو با جایگذاری دوطرفه مفصل زانو شود، اما در توجه پایدار تأثیرات محدودی دارد؛ بنابراین میتوان از این برنامه درمانی غیرتهاجمی و بدون عارضه به عنوان درمانی مکمل در فرایند توانبخشی این بیماران استفاده کرد.
Objective: As life expectancy increases, the incidence of degenerative diseases such as osteoarthritis also increases. Dynamic balance is one of the essential factors that affect the mobility and daily living activity of patients suffering from knee osteoarthritis after undergoing Bilateral Total Knee Replacement (BTKR) surgery. Traditional rehabilitation approaches are based on reducing pain and increasing muscle strength, which are applied topically on the knee area. One of the new methods used to improve balance is neurofeedback, which acts at the central nervous system level. This pilot study aims to examine the effect of neurofeedback on dynamic stability and sustained attention in women who underwent BTKA.
Materials & Methods: This is a quasi-experiment study with a pre-test/post-test design and no control group. The study population consisted of all patients with BTKA referred to the outpatient clinic of the Department of Occupational Therapy at Shahid Beheshti University of Medical Sciences from April 2017 to September 2017. Of these patients, 8 female patients with a mean age of 67.5 years (3 months had passed since their surgery) were selected using a convenience sampling method and based on the inclusion and exclusion criteria.
Results: The inclusion criteria were being over 55 years old; having a history of BTKR surgery in the last 2 to 4 months; being able to walk independently; using a cement prosthesis; lacking a record of cognitive problems (mini-mental state exam score> 20), cardiovascular diseases, uncontrolled high blood pressure, visual impairment, drug abuse or alcohol use, vestibular disorders that can impair balance, use of neuroleptics and sedatives; not receiving neurofeedback before this study, or undergoing surgery in other joints of lower limbs, such as ankle or hip. The exclusion criteria were the absence of more than two sessions from therapeutic protocol and unwillingness to collaborate in the research process. All patients received 8 sessions of standard neurofeedback training. Assessments were done at three times: before the intervention, and at the 4th and 8th sessions using vigilance subscale of Vienna test system and Biodex balance system (dynamic stability level 6 test). Conclusion: Neurofeedback training can improve dynamic stability in female patients with knee osteoarthritis 4 months after BTKR surgery, but it has limited effects on their sustained attention. Therefore, this non-invasive and uncomplicated program can be used as a complementary treatment method for the rehabilitation of these patients.
