خلاصة:
زمینه و اهداف: سیاستهای کلی سلامت ابلاغی مقام معظم رهبری به لحاظ پشتوانه شرعی و مبتنی بر قانون اساسی کشور تکلیفی حقوقی، الزامآور و قطعی است. مفاد ابلاغیه جامع و کامل است و میتواند اهداف سلامت در جمهوری اسلامی ایران مبنی بر عدالت اجتماعی، مشارکت مردم، همکاری بینبخشی و استفاده از تکنولوژی مناسب را پوشش دهد. در بند دوم این سیاست موضوع سلامت همهجانبه و انسان سالم مورد تاکید قرار گرفته است و سیاستگذار تاکید میکند که تمامی اقدامات مجلس و دولت در وضع قوانین و مقررات تنظیمی باید بر پایه تامین و ارتقای هر چهار بعد سلامت باشد. بنابراین در این مطالعه تلاش بر این است که این مفاهیم تبیین و بومیسازی شوند و برای اجرای مفاد آن شاخصها و مداخلات کلی معرفی شوند.
روش کار: در مرحله اول بهمنظور تعریف دقیق مفاهیم کلیدی، منابع مکتوب داخلی و خارجی مطالعه شد. در مرحله دوم با استفاده از محتواهای بهدست آمده در مرحله اول و کسب نظر خبرگان با روش کیفی علاوه بر تعیین ذینفعان فهرستی از شاخصها و مداخلات زیربندهای مربوط احصاء و فهرست کلی طبقهبندی شد.
یافتهها: در این مطالعه مفاهیم کلی به تفکیک هر زیربند تبیین و تعریف شد و شاخصهای مربوط و مداخلات اجرایی برای هر شاخص در قالب 6 زیربند مربوط تعیین شد.
نتیجهگیری: سلامت در همه سیاستها منعکسکننده یک طرح چندبخشی در جهت سیاستگذاری سلامت ملی یا حکومتی است، سلامت در همه سیاستها بهطور روزافزونی بهعنوان یک استراتژی دولتی بهمنظور بهبود سلامت جامعه با اقدامات هماهنگ در سراسر بخشهای بهداشتی و غیربهداشتی برجسته شده است.
تعیینکنندههای سلامت هسته اصلی سلامت در همه سیاستها میباشد. در تدوین سیاستها و راهبردهای کلان کشور، اولویت پیشگیری بر درمان، رصد تغییرات محیطی و اجتماعی و اصلاح راهکارهای حفاظتی و همچنین بازنگری برنامههای جاری سلامت در کشور و بهروز نمودن آنها، تقویت نظامهای پایش و نظارت در برنامهها و ارزشیابی استانداردها راههای موثر برای حفظ سلامت و پیشگیری از بیماریها و خطرات تهدیدکننده در جامعه و موجب تسهیل دستیابی به اهداف ایران 1404 است.
بنابراین لازم است در درجه اول قوانین کشوری و سیاستگذاریهای کلان جامعه در حوزههای مختلف با محوریت سلامت باشد و در مرحله بعد پیادهسازی قوانین و اجرای سیاستها، با حفظ پایبندی به اصول سلامت و ارزشهای جامعه عملیاتی شوند.
چالشهای پیادهسازی سلامت در همه سیاستها، مسئلهای است که به احتمال زیاد تا مدتها باقی خواهد ماند و ما نباید انتظار بیش از حدی در دستیابی خیلی سریع به دستاوردهای خیلی مهم داشته باشیم. (1)
سیاستهای کلی سلامت از جامعترین اسناد بالادستی سلامت است. این سیاستها نظام سلامتی کارآمد، عادلانه و نوآور را در مسیر تحقق جامعهای سالم و توانمند ترسیم میکند و به ارتقای سلامت و پیشگیری و بهرهمندی مردم از مراقبتهای جامع با کیفیت اهتمام دارد، اما علیرغم جامعیت این سند مانند هر سند کلان دیگر مفاهیم مندرج در آن یا بهطور کامل و به وضوح تعریف نشدهاند و یا دارای تعریف قابل اجماعی نیستند (2).
بهمنظور برنامهریزی و اجرای بند دوم سیاستها، الزامات و ملزومات قابل توجهی در کشور فراهم است. مثل تجربه گذشته ایجاد شبکهای کارآمد و عدالت محور برای ارائه کلیه مراقبتهای اولیه بهداشتی و الگوی موفقی برای تربیت نیروی انسانی و ادغام آموزش پزشکی، همچنین مصوبات قانونی حمایتکننده موجود مثل بند اصلاحی ماده 89 برنامه ششم توسعه که وزارت بهداشت را مکلف به اجرای موارد زیر نموده است: نظام خدمات جامع و همگانی سلامت، اولویت پیشگیری بر درمان و مبتنی بر مراقبتهای اولیه سلامت با محوریت نظام ارجاع مبتنی بر پزشک خانواده، سطحبندی خدمات، پرونده الکترونیک سلامت ایرانیان، و یا مصوبه استانداردهای ملی پیوست سلامت که در خرداد ماه سال 92 در شورای عالی سلامت تصویب و توسط سازمان مدیریت و برنامهریزی کشور به دستگاههای اجرایی ابلاغ شده است ولی هنوز به اجرا در نیامده است.
یکی از شاخصهای روشن و کمککننده به اجرای بند دوم در سیاستهای ابلاغی، تعیین تکلیف تولیت و تولیگری این حوزه است که به روشنی در بند 7 به وزارت بهداشت، درمان و آموزش پزشکی سپرده شده است. وزارت بهداشت در این سیاستها مسئول سیاستگذاری، برنامهریزی، ارزشیابی و نظارت است. وزارت بهداشت تاکنون از این توان حقوقی خود بخوبی استفاده نکرده است. به نظر میرسد این باور در سایر نهادهای مجری مثل وزارت رفاه، صنعت و معدن و کشاورزی بخوبی نهادینه نشده است.
بهمنظور تدوین سیاست و برنامهریزی برای دستیابی به اهداف این بند مثل سایر بندها ضرورت دارد ابتدا نگرش اصلی کشور به موضوع سلامت تبیین و دورنمای مراقبتهای پیشگیرانه اولیه، بیمارستانی و درازمدت و کیفیت آنها معلوم شود. (3)
Abstract
Background and Aim: General Health Policies announced by Islamic Republic of Iran`s supreme leader is legal ,mandatory, and definitive duty on the basis of shari'a and according to the constitution of the country.
The provisions of the document are comprehensive and complete and it covers health objectives in the I.R of Iran based on social justice, people's participation, intergovernmental cooperation, and the use of appropriate technology. Part 2 of the policies emphasize the issue of health all -round and healthy people. And the politician insists that all actions of the parliament and the government in the establishment of laws and regulations should be based on the provision and promotion of all four dimensions of health. So this study attempts to explain and localize the concepts and introduce the indicators and general interventions for implementation of the provisions.
Method: First, in order to accurately define the key concepts, internal and external documentation were reviewed. In the next step, using the content obtained in the first step and the opinion of the experts in qualitative method stakeholders were identified and a list of indicators and interventions for relevant subcategories were classified.
Results: In this study, 30 general concepts were identified and explained for each sub-sections. And 35 relevant indices and 51interventions were selected in the form of six sub- sections.
Conclusion: Health in all policies reflects a multidisciplinary plan for national or state health policy. Health in all policies has increasingly been highlighted as a government strategy to improve the health of the community through coordinated action across health and non-health sectors. Health determinants are the core of health in all policies. The effective ways to maintain health and prevent diseases and threats in the community and facilitate the achievement of Iran's goals of 1404 are prioritizing prevention, observation of environmental and social changes, and correction of protective measures as well as reviewing and updating current health plans in the country, strengthening monitoring and evaluation systems in programs and evaluations for standards in developing the country's policies and strategies. There for, in first step country laws and major policies in a variety of areas should be health-oriented. The next step is implementing the rules and regulations, in compliance with principles of health and the values of the community.
In order to plan and implement part 2 of the policy, there are significant requirements in the country. For example, the past experience of establishing an efficient and justice-based network for providing all primary health care and a successful model for training human resources and integrating medical education, as well as support legislations , such as Article 89 of Iran 6th Development Plan, which obliges the Ministry of Health to implement a comprehensive service system and public health, the priority of primary health care, and establishment of primary care referral system in family practice, the level of service, the Iranians electronic health record, or the National Health Standards Act, approved by the High Council on Health in June 2012, and Plan and Budget Organization has been communicated to the executive agencies, but has not implemented yet.
One of the key elements for implementing part 2 is to clarify the stewardship of health system and to establish this concept Ministry of Health and Medical Education is responsible for this function, as clearly stated in part 7. The Ministry of Health is responsible for policy making, planning, monitoring and evaluation but it has not used authority well. It seems that this belief has not been well established in other executive agencies such as the Ministry of Cooperatives Labor and Social Welfare, Ministry of Industry, Mining and Trade and Ministry of Agriculture -Jahad.
In order to create plan to achieve this part goals (like other parts), it is necessary to explain the image of healthy society firstly and then reveal the health system`s function in primary, secondary and tertiary preventive care and their quality of care.
