چکیده:
سابقه و هدف: واژینیت کاندیدایی، یکی از شایع ترین بیماری های دستگاه تناسلی زنان می باشد. اخیرا گزارش های متعددی در مورد اثرات ضد قارچی و ضد میکروبی گیاه «مورد» ارایه شده است. این پژوهش با هدف تعیین تاثیر کپسول خوراکی «مورد» بر درمان واژینیت کاندیدیایی در زنان مراجعه کننده به درمانگاه زنان بیمارستان لولاگر وابسته به دانشگاه علوم پزشکی و خدمات بهداشتی و درمانی شهید بهشتی در سال ۹۲-۱۳۹۱ انجام شد.
مواد و روشها: در این کارآزمایی بالینی دوسوکور تصادفی، 100 زن غیر باردار در دو گروه دریافت کننده ی کپسول خوراکی «مورد» و کرم واژینال کلوتریمازول (گروه مورد) و کپسول پلاسبو و کرم واژینال کلوتریمازول (گروه کنترل) قرار گرفتند. علایم و نشانه های بالینی و نتایج آزمایشگاهی در ابتدای مطالعه و پس از تکمیل دوره ی 7-4 روزه ی درمانی توسط پرسشنامه و فرم مشاهده، ثبت گردید. اطلاعات جمع آوری شده با استفاده از نرم افزار کامپیوتری SPSS (نگارش 18) مورد تجزیه و تحلیل و قضاوت آماری قرار گرفت.
یافته ها: نتایج حاصل از این پژوهش نشان داد که میزان موفقیت گروه «مورد» در بهبود خارش (82 درصد) و ترشح (78 درصد) که شایع ترین علایم واژینیت کاندیدیایی می باشند، نسبت به گروه کنترل (به ترتیب 16 و 28 درصد) به طور معنی داری بیشتر است. پس از اتمام دوره ی درمان، کشت ترشحات در گروه «مورد»، 90 درصد و در گروه کنترل 62 درصد منفی بود. تفاوت اثر در دو گروه به لحاظ آماری معنادار بود (001/0P<).
نتیجه گیری: به نظر می رسد کپسول خوراکی «مورد» بر واژینیت کاندیدیایی اثرات درمانی مطلوب و مناسبی داشته است. بنابراین می توان از خواص متعدد این گیاه در تسکین و بهبود برخی علایم و نشانه های مرتبط با عفونت های ولوو واژینیت قارچی در مدت زمانی مشابه با داروهای ضد قارچ از گروه آزول، از جمله کلوتریمازول، بهره جست.
Background and Aim: Although the use of a dynamic hip screw (DHS) is considered to be the suitable treatment for intertrochanteric fractures، the external fixation device could produce clinical outcomes comparable to the outcomes obtained with conventional treatment. Furthermore، because external fixation is minimally invasive، we expected a lower rate of morbidity. Therefore، we compared the two treatments in a clinical trial of elderly patients with intertrochanteric fracture.
Materials and Methods: Participants were included 60 elderly high-risk patients with an age of 785.6 years who were treated for intertrochanteric fracture، resulting from a low energy trauma. Patients were randomly divided in two conditions regarding the treatment. In group A، the patients were treated with DHS، while in group B patients were treated with external fixator.
Results: The fixator was well accepted and no patient had significant difficulties while sitting or lying. The intraoperative time was 7310 minutes in group A and 158 minutes in group B (P<0.01). 27 patients of group A needed blood transfusion postoperatively and none in group B (P<0.001). The duration of hospitalization in group A and group B was 8.42 and 2.21.5 days، respectively (P<0.05). 9 of patients in group B had pin-track infection grade 2 that all were treated by oral antibiotics. There were no differences in comorbidities، quality of reduction، screw cut out، bed sore and HHS between the two groups.
Conclusion: Treatment with external fixator is an effective treatment for intertrochanteric fractures in elderly high-risk patients. The advantages include quick and simple application، minimal blood loss، less radiation exposure، adequate fixation، pain reduction، and early discharge from hospital، low costs and favorable functional outcomes.
