چکیده:
زمینه و هدف:کیفیت فیت سوکت، که موجب یک رابطه متقابل میان ساختار استخوانی استمپ و ساختار جامد پروتز می شود، در موثر بودن پروتز اندام تحتانی نقشی حیاتی دارد. حجم استمپ به طور مداوم در حال تغییر است، بنابراین فیت میان استمپ و سوکت برهم می خورد. تاثیر شرایط فیت مناسب و نامناسب بر توانایی های عملکردی افراد قطع عضو تاکنون مورد ارزیابی قرار نگرفته است. هدف از بررسی تاثیر شرایط فیت نامناسب و مناسب و راحتی سوکت بر پارامترهای مربوط به عملکرد فرد قطع عضو.
مواد و روش ها: 25 فرد قطع عضو زیرزانو یکطرفه وارد مطالعه شدند. از آنها خواسته شد در شرایط شرایط فیت نامناسب بین استمپ و سوکت به پرسشنامه های SCS، TAPES و LCI5 پاسخ دهند. 15 روز پس از تعویض سوکت ، در صورت دستیابی به شرایط فیت مناسب از داوطلبان خواسته می شد تا دوباره به پرسشنامه ها پاسخ دهند. در این مرحله 11 نفر از داوطلبان به دلیل نارضایتی سوکت جدید از مطالعه خارج شدند.
نتایج: تفاوت بین متغیرهای مربوط به عملکرد داوطلبان در شرایط فیت نامناسب و مناسب معنادار نبود. اما معیار راحتی سوکت افزایش معناداری داشت.
بحث: به نظر می رسد در شرایط فیت نامناسب افراد قطع عضو با در پیش گرفتن مکانیسم های جبرانی مانع از کاهش گسترده ی توانایی های عملکردی شان می شوند. کاهش محدودیت در انجام فعالیت های ورزشی (مربوط به پرسشنامه TAPES)، افزایش توانایی های پیشرفته (مربوط به پرسشنامه LCI5) نمونه هایی از تاثیرات مثبت بهبود شرایط فیت بر توانایی های عملکردی بودند.
Background: Socket is a mechanical coupling between residual limb and the prosthesis. The Lower limb prosthesis function is highly influenced by the quality of this coupling. Furthermore، the residual limb volume fluctuation compromise the prosthetic socket fit and hence the amputee’s function. Purpose:To examine the effect of ’bad’ and ‘good’ socket fit conditions on amputees’ performance-based parameters. Method & Materials: 25 unilateral trans-tibial amputees were recruited. They were asked to answer SCS، TAPES، LCI5 questionnaires for the ’bad’ socket fit condition. Following change to a new socket they were asked to fill in the questionnaires again providing that they had been regularly using the prosthesis for at least two weeks and satisfied with the socket fit. At this stage، 11 volunteers were excluded due to dissatisfaction with the new socket fit. Results: The differences between variable relating to the performance of the candidates in ’bad’ and ’good’ socket fit conditions were not statistically significant. However، there was a statistically significant increase in the SCS in favour of ‘good’ socket fit condition. Conclusion: Lack of differences between two conditions could be explained by amputees taking a compensatory mechanisms in ’bad’ socket fit condition to avoid a massive reduction in their functional capabilities. Reduction in sport activities restrictions (related to the TAPES questionnaire)، and increase in advanced activities (related to the LCI5 questionnaire) are examples of the positive influences of improvement in socket fit.
