چکیده:
زمینه و هدف: تقاضای القایی یکی از چالشهای نظام سلامت کشورها است که موجب افزایش هزینههای درمانی مردم، سازمانهای بیمهگر و در نهایت هزینههای نظام سلامت میشود. این پدیده یک پدیده پیچیده و چندوجهی بوده که شناسایی ابعاد تاریک آن میتواند منجر به کنترل این فرآیند شده و به سیاستگذاران حوزه سلامت برای تعدیل چالشهای ناشی از آن، راهکار منطقی و اجرایی ارائه دهد.
روش: این مطالعه از طریق مرور ادبیات و متون علمی موجود درحوزه مدیریت، سیاستگذاری و اقتصاد سلامت و با بررسی مقالهها، گزارشها، اسناد بالادستی و منابع موجود در پایگاههای اطلاعاتی داخلی و خارجی و با استفاده از کلید واژههای تقاضای القایی، تقاضای القایی ارایهدهنده، تقاضای القایی پزشک، نظام سلامت ایران و در محدوده زمانی 10 سال گذشته انجام شده است.
یافتهها: ابتدا 20 مقاله براساس معیارهای مطالعه و اهداف آن استخراج و مورد استفاده قرار گرفت. نتایج مطالعات منتخب مرور و سپس خلاصه شد. با بررسی مطالعات کمی مشخص شد که هدف محققان اغلب پژوهشهای کمی یافت شده، اثبات فرضیه وجود تقاضای القایی در نظام سلامت و یا میزان تأثیر تقاضای القایی در شاخصها و متغیرهای سلامت است ولیکن درمطالعات کیفی یافت شده، محققان در پی یافتن جنبههای غیر شفاف موضوع تقاضای القایی از قبیل عناصر تشکیلدهنده تقاضای القایی، عوامل ایجادکننده تقاضای القایی، چالشها و راهکارهای جلوگیری یا کاهش اثرات تقاضای القایی بودند.
نتیجهگیری: طبق نتایج، عمده این مطالعات به بررسی عوامل تقاضای القایی پرداخته و درصد کمی از مطالعات به چالشها و راهکارهای کاهش آن توجه کردهاند. بنابراین انجام مطالعات بیشتر برای بررسی چالشها و راهکارها بهخصوص مطالعات کمی با متدهای اقتصادسنجی ضروری به نظر میرسد. راهبردهای استخراج شده میتواند در سطح کلان برای ارتقای مدیریت کارآمد نظام سلامت، کاهش چالشهای بین ارائهدهندگان و خریداران خدمت و همچنین کنترل افزایش هزینههای نظام سلامت و افزایش کیفیت خدمات مؤثر باشد.
Background: Induced demand is one of the challenges of the health system of countries; it increases the medical costs of people, insurance organizations and ultimately the costs of the healthcare system. This phenomenon is a complex and multifaceted phenomenon that identifies its dark dimensions and can lead to control of this process and provide health policy makers with a logical and practical solution to mitigate the resulting challenges.
Methods: This study reviewed the literature and scientific texts in the field of management, policy and health economics; and by reviewing articles, reports, upstream documents and resources in domestic and foreign databases and using the keywords induction demand, induction demand provider, assessed the Iranian health system during the past 10 years.
Results: The results of selected studies were reviewed and then summarized. Examining quantitative studies, it was found that the goal of researchers is often quantitative research to prove the hypothesis of induced demand on the health system or the effect of induced demand on health indicators and variables in qualitative studies. The researchers sought to find non-transparent aspects of the subject of induced demand, such as the elements of induced demand, the factors that induce induced demand, as well as the challenges and strategies to prevent or reduce the effects of induced demand.
Conclusion: According to the results, most of these studies examined the factors of induced demand and a small percentage of studies have paid attention to the challenges and strategies to reduce it. Therefore, further studies are needed to examine the challenges and solutions, especially quantitative studies with econometric methods. The extracted strategies can be effective at the macro level to promote efficient management of the health system, reduce challenges between service providers and buyers, as well as control the increase in health system costs and increase the quality of services
