چکیده:
مقدمه: پژوهش حاضر با هدف مقایسه اثربخشی درمان شناختی رفتاری یکپارچه با درمان فراتشخیصی بر علایم بیماران مبتلا به اختلال اضطراب فراگیر همبود با افسردگی انجام شد. روش: برای اعتبارآزمایی رقابتی از طرح نیمه تجربی دو گروهی به روش مورد منفرد استفاده شد. جامعه آماری شامل افراد مبتلا به اختلال اضطراب فراگیر همبود با افسردگی مراجعه کننده به مراکز مشاوره شهر اصفهان در سال 1398 بود که از میان آنان 10 نفر به روش نمونهگیری هدفمند انتخاب و به شیوه تصادفی در دو گروه درمان شناختی رفتاری یکپارچه و درمان فراتشخیصی گمارش شده و مورد مداخله قرار گرفتند. آزمودنیهای دو گروه در مراحل خط پایه، جلسات سوم، پنجم، هشتم، دهم و پیگیری یک ماهه با مقیاسهای اضطراب فراگیر (GAD-7)، مقیاس افسردگی بک (BDI-II) و پرسشنامه نگرانی ایالت پنسیلوانیا PSWQ)) مورد ارزیابی قرار گرفتند. [LR1] جهت گزارش نتایج از تحلیل دیداری نمودارها با شاخص تغییر پایا (RCI)، درصد بهبودی و معناداری آماری و بالینی استفاده شد. یافتهها: نتایج نشان داد که هر دو درمان تغییرات معناداری از نظر بالینی و آماری در آماجهای درمانی ایجاد کرده و تاثیرات درمانی در دوره پیگیری نیز ادامه داشته است. اما درصد بهبودی در آزمودنیهای گروه درمان شناختی رفتاری یکپارچه نسبت به آزمودنیهای گروه درمان فراتشخیصی بیشتر بود. بحث و نتیجهگیری: با توجه به نتایج به دست آمده و بر اساس مبانی نظری اثربخشی درمانی، درمان شناختی رفتاری یکپارچه از نظر اندازه تغییر و ثبات بر درمان فراتشخیصی ارجحیت دارد اما از نظر میزان پذیرش هر دو یکسان هستند. Objectives: The purpose of this study was to compare the effectiveness of integrated cognitive behavioral therapy with Transdiagnostic Treatment on symptoms of patients with generalized anxiety disorder comorbid with depression.
Methods: For competitive validation, a two-group semi-experimental design with a single case method was used. The statistical population included people with generalized anxiety disorder comorbid with depression who referred to counseling centers in Isfahan in 2019, from which 10 people were selected by purposive sampling and randomly assigned to two groups of integrated cognitive behavioral therapy and meta-diagnostic treatment and were intervened. Subjects in both groups were assessed in baseline, third, fifth, eighth, tenth, and one-month follow-up sessions with the Generalized Anxiety Scale (GAD-7), the Beck Depression Inventory (BDI-II), and the Penn State Worry Questionnaire (PSWQ). To report the results, visual analysis of graphs with reliable change index (RCI), improvement percentage and statistical and clinical significance was used.
Results: Visual charting, percentage of improvement, and reliable change index (RCI)) showed that both treatments produced clinically and statistically significant changes in therapeutic outcomes and continued therapeutic effects during follow-up. However, the percentage of improvement in the subjects of the integrated cognitive-behavioral therapy group was higher than the subjects in the Transdiagnostic Treatment group.
Conclusion: Based on the obtained results and based on the Theoretical explanation of findings for effectiveness of therapy, integrated Cognitive Behavioral Therapy is preferred over transdiagnostic treatment in terms of the magnitude of change and stability, but both are similar in acceptance.
