چکیده:
تشنج همراه تب با وجود پیش آگهی عالی، موجب اضطراب بیشتر مادر در مقایسه با سایر بیماریهای کودک می شود. بدین سبب یکی از وظایف مهم پزشکان در برخورد با تشنج همراه تب کودک، آموزش مادران در جهت کاهش اضطراب آنها است. به منظور تعیین تاثیر آموزش در میزان اضطراب مادران بعد از اولین تشنج همراه تب کودکان، این تحقیق طی سال 1377 بر روی مادرانی که کودکان آنها در مرکز پزشکی، آموزشی و درمانی کودکان مفید بستری شده بودند، انجام گرفت. روش تحقیق کارآزمایی بالینی (Clinical trial) از نوع مقایسه قبل و بعد بود و 40 مادر در دو گروه مساوی تجربی و شاهد مورد بررسی قرار گرفتند. میزان اضطراب مادران با پرسش نامه استاندارد (State trait anxiety inventory (STAI در هر دو گروه تعیین شد (Pretest). سپس گروه تجربی تحت آموزش چهره به چهره به مدت 3 ساعت قرار گرفت. برای گروه شاهد هیچ مداخله ای به عمل نیامد. نه روز بعد میزان اضطراب در هر دو گروه با همان پرسش نامه اولیه اندازه گیری شد (Posttest). داده ها با استفاده از Wilcoxon Signed Rank Sum test و Mann-Whitney و McNemar و X² مورد تجزیه و تحلیل قرار گرفت. تحقیق نشان داد که در گروه تجربی اضطراب مادران به طور معنی داری کاهش یافته است (P<0.001)، در حالی که در گروه شاهد تفاوت معنی دار مشاهده نگردید. مطالعه ما نشان داد که آموزش مادران بعد از اولین تشنج همراه تب کودکان موجب کاهش اضطراب و نگرانی آنان می شود و با توجه به نتایج تحقیقات مشابه انجام گرفته، توصیه می گردد برنامه های آموزشی برای همه مادرانی که کودک تب و تشنجی دارند انجام گیرد.
Febrile convulsion, despite its excellent prognosis, is a cause of high anxiety among mothers compared to other childhood diseases. Therefore, one of the physician’s most important responsibilities in approaching febrile convulsions is to train mothers to reduce their anxiety. This study was performed to determine the effect of education on the rate of maternal anxiety after the child’s first febrile convulsion, among mothers whose children were admitted to Mofid Children’s Hospital during 1377 (1998). It this sequential control clinical trial, we divided and studied 40 mothers in two matched case and control groups. Maternal anxiety level was determined in both groups by the State Trait Anxiety Inventory (STAI) standard questionnaire (pretest). Then, the case group underwent face-to-face education for 3 hours. No intervention was performed on the control group. After 9 days, anxiety rate was determined in the two groups using the same questionnaire (post-test). Data were analyzed using the Mann Whitney-Wilcoxon Signed Rank Sum test, McNemar test and chi-square. Results show that in the case group, maternal anxiety decreased significantly (P<0.0001), whereas no significant difference was seen in the control group. Our study proves that maternal education after the child’s first febrile convulsion reduces maternal anxiety, and considering results of similar studies, educational programs for mothers who have children with febrile convulsions are recommended.
