چکیده:
میانگین طول گفتهی واجی (PMLU)، مقیاسی کمّی جهت ارزیابی مهارت واجشناختی است و در مطالعات رشدی، بهعنوان معیاری تشخیصی و بالینی در رشد واجشناختی مطرحشده است که میتواند میزان اثربخشی مداخله را نیز نشان دهد. PMLU مقیاسی در سطح کلمه است که میتواند بر اساس کلمات آوانگاری شده در نمونه گفتار کودک محاسبه شود. PMLU با نمره دهی تکتک همخوانها و واکههای کلمات تولیدشده توسط کودک و کلمات هدف در تولید معیار بزرگسالان بومی زبان محاسبه میشود. نسبت مجاورت کل کلمه (PWP)، یکی دیگر از مقیاسهای کمّی واجی است که بر اساس نسبت PMLU کلمات تولیدشده به هدف محاسبه میشود و بهصورت غیرمستقیم بیانگر قابلیت وضوح گفتار است. با توجه به وجود تفاوتهای ساختارهای هجایی و واجی در زبانهای مختلف، این مقیاس باید مختص هر زبان، مطالعه شود. مقیاس PMLU بهطور خاص برای ارزیابی گفتار خودانگیخته طراحیشده است. روشهای ارزیابی گفتار خودانگیخته به دلیل توانایی نشان دادن رشد طبیعی پیچیدگی کلمات و توجه به اثر مهارتهای صرفی، نحوی، افزایش طول و پیچیدگی کلمات، روشهای ارزشمندی هستند. با توجه به اینکه در زبان فارسی میانگین طول گفته واجی بررسی نشده است و در مطالعات در خصوص حساس بودن این مقیاس به رشد تناقضهایی مشاهده میشود، این مطالعه باهدف به دست آوردن مقادیر PMLU و PWP در کودکان 48 تا 60 ماهه فارسیزبان با لهجهی اصفهانی، مقایسهی آنها در دو نوع نمونهگیری روایت داستان و محاوره و همچنین بررسی حساس بودن مقیاس PMLU به رشد انجام گرفت. روش بررسی مطالعه حاضر از نوع مشاهدهای است که بهصورت تحلیلی- مقطعی در سال 1395 به مدت یک سال در شهر اصفهان اجرا شد. در بخش روایت داستان 100 کودک طبیعی شامل 51 پسر و 49 دختر و در بخش محاوره 67 کودک شامل 32 پسر و 35 دختر با بازهی سنی ۴۸ تا ۶۰ ماه شرکت کردند. این کودکان از بین 261 مهدکودک تحت نظارت سازمان بهزیستی استان اصفهان به روش نمونهگیری در دسترس انتخاب شدند. پس از تکمیل فرم رضایتنامه و بررسی معیارهای ورود، نمونه گفتاری روایت داستان و محاوره جمعآوری شد. صدای همه کودکان، در یک اتاق ثابت و با نرمافزار یکسان Clear Record Lite-v2. 1 ضبطشده است. ۵۰ کلمه اول هر نمونه گفتاری، توسط ارزیابها آوانگاری شد. درنهایت مقادیر PMLU هدف و PMLU کودک، به ترتیب بر اساس گفتار بزرگسالان فارسیزبان با لهجهی اصفهانی و بر اساس تولید هر کودک، با استفاده از فرمول محاسبه شد. برای بررسی توافق بین ارزیابها، هرکدام از ارزیابها% 20 کل نمونههای گفتاری را بهصورت تصادفی مجددا آوانگاری کردند و میانگین PMLU کودک و PMLU هدف را محاسبه کردند. نمرات شرکتکنندگان به تفکیک داستان و محاوره وارد نرمافزار SPSS16 شد. برای بررسی توزیع دادهها از آزمون کلموگروف اسمیرنوف استفاده شد که متناسب با توزیع دادهها برای مقایسه مقیاسها از آزمونهای t زوجی و ویلکاکسون و برای بررسی همبستگیها از آزمونهای پیرسون و اسپیرمن استفاده گردید. یافتهها مقیاسهای PMLU کودک و PMLU هدف و PWP به تفکیک روش نمونهگیری روایت داستان 8/794، 8/811، 0/998 و محاوره به ترتیب 9/068، 0/093، 998/0 بدست آمد. نتایج آزمونهای همبستگی، میان سن و مقیاس PWP داستان (0/308r=) و محاوره (0/313r=) همبستگی معنادار نشان دادند. مقایسه PMLU کودک در روایت داستان و محاوره، نشاندهندهی رابطهی معنادار مقیاس PMLU کودک در دو روش نمونهگیری بود (0/000=P). همچنین مقایسه PMLU هدف در دو روش روایت داستان و محاوره، معنادار بود (0/000=P) اما PWP در این دو روش، تفاوت معناداری نشان نداد (0/973=P). توافق بین ارزیابها% 70 بدست آمد. نتیجهگیری میانگین نمرات PMLU بالاتر در زبان فارسی نسبت به مطالعه مشابه، میتواند به علت تفاوتهای زبان فارسی و لهجه اصفهانی باشد. همچنین نبود رابطهی معنادار در PWP داستان و محاوره، احتمالا نشاندهندهی عدم تاثیرپذیری مقیاس PWP از روش نمونهگیری باشد درحالیکه تفاوتهای روشهای نمونهگیری میتواند بر نمرات PMLU تاثیرگذار باشد. مطالعه حاضر به وجود همبستگی میان سن و pwp نیز اشاره کرد. این مطالعه میتواند بهعنوان پایهای برای مطالعات کمی درزمینهٔ ارزیابی واجشناختی کودکان با مقیاسهای کل کلمه در زبان فارسی باشد اما وجود مطالعات طولی، در گروههای سنی مختلف و با سطح شواهد بالا در این زمینه میتواند منجر به اطمینان خاطر بیشتر درمانگران جهت استفاده از مقیاسهای کمی کل کلمه در بالین باشد.
Objective: Phonological Mean Length of Utterance (PMLU), a quantitative measure for assessing phonological skills, is a diagnostic and clinical criterion in phonological development. Moreover, it is an indicator showing the efficacy of the intervention. The PMLU is a word level measure that can be calculated on the child’s transcribed speech sample (transcription). To calculate PMLU, all consonants and vowels of the child’s produced words, and target words (standard production of words in native adults) are individually scored. The proportion of Whole-Word Proximity (PWP), another phonological quantitative measure, includes the ratio of the produced PMLU to the targeted word PMLU. PWP indirectly reflects the intelligibility of speech.
Since languages are distinctive in syllabic and phonological structures, PMLU should be studied as a language-specific measure. PMLU has specifically been designed to assess phonological skills in spontaneous speech. Spontaneous speech sampling methods are advantageous since they consider the effect of morphological and syntactic skills, length, and complexity of words, and they could show the normal development of word complexity. This study was conducted to determine PMLU and PWP in 48 to 60 months old Persian-speaking children with Isfahani accents and to compare them in story generation and conversation sampling methods. The potential sensitivity of PMLU to growth was also examined.
Materials & Methods: This is an observational and cross-sectional study that was conducted for one year in 2016 in Isfahan City, Iran. A total of 100 children (51 boys and 49 girls) aged 48-60 months participated in story generation sampling, and 67 children (32 boys and 35 girls) participated in conversation sampling. The participants were selected from 261 kindergartens under the supervision of the Welfare Organization of Isfahan Province using the convenience sampling method. After completing the consent form and considering the inclusion criteria, conversation and story-generation samples were collected. Audio samplings were done in the same room using the same software (Clear Record Litev. 2.1). Raters transcribed the first 50 words of the recorded speech samples. Finally, we used the formula to calculate the target PMLU and child PMLU based on the normal production of an adult who speaks Persian with an Isfahani accent and the child’s production, respectively. To evaluate inter-rater reliability, raters randomly transcribed 20% of all samples to recalculate values. Participants’ story generation and conversation scores were entered into SPSS16 separately. The Kolmogorov-Smirnov test was used to examine the data distribution. Based on the data distribution, paired t-test and Wilcoxon test were used to compare measurements, and the Pearson and Spearman tests were used to investigate the associations.
Results: Child PMLU, target PMLU, and PWP measures of the story generation method were 8.794, 8.811, 0.998, and those of the conversation method were 9.068, 9.093, and 0.998, respectively. Correlation test results showed significant relationship between age and PWP in story generation (r=0.308) and conversation (r=0.313). Comparing child PMLU in story generation and conversation showed a significant relationship between child PMLU in both sampling methods (P=0.000). The result of the target PMLU comparison in two methods of story generation and conversation (P=0.000) was significant. PWP did not significantly differ between the two sampling methods (P=0.973). The inter-rater reliability was calculated at 0.70.
Conclusion: This study can be used as a basis for quantitative studies in the field of children’s phonological assessment using Persian whole words. However, longitudinal studies in different age groups with a high level of evidence in this field can convince therapists to use whole-word measures in clinics.
