چکیده:
هدف: مالتیپل اسکلروزیس اختلالیست که با درگیرشدن سیستم اعصاب مرکزی همراه است. در افراد مبتلا به مالتیپل اسکلروزیس، اختلالات شناختی تاثیر مستقیم و زیادی بر روی عملکردشان در فعالیتهای روزانه زندگی ایجاد میکند و از این رو مشارکت این افراد مختل میشود. پس بررسی حیطههای شناختی میتواند ما را در امر تصمیمگیری بالینی و مداخلات یاری کند. در جهت توانبخشی شناختی این افراد و اینکه بتوانیم ارتباط میان عملکرد شناختی (آزمون کامپیوتری و آزمون عملکردی) و میزان مشارکت را تخمین بزنیم، باید همبستگی این دو مورد را بررسی کنیم. هدف ما از این مطالعه این است که همبستگی مشارکت با دو نوع از ارزیابیهای شناختی که کامپیوتری و عملکردی میباشند را مشخص کنیم. روش بررسی: مطالعه ی حاضر از نوع مقطعی و نمونهگیری بصورت در دسترس انجام گرفت. جامعه هدف، بیماران با مالتیپل اسکلروزیس پذیرش شده در مرکز مالتیپل اسکلروزیس شهرستان اصفهان به شرط دارا بودن معیارهای ورود به مطالعه در زمستان 1399 بودند. 45 بیمار با مالتیپل اسکلروزیس بصورت در دسترس (9 مرد و 36 زن) با نمره مقیاس گسترش یافته ناتوانی (Expanded Disability Status Scale) بین 1 تا 5 انتخاب شدند. جهت ارزیابی عملکرد شناختی آنها، از آزمون Cambridge Neuropsychological Test Automated Battery (CANTAB) و آزمون عملکردی (LOTCA) The Lowenstein Occupational Therapy Cognitive Assessment وهمچنین برای تعیین میزان مشارکت از پرسشنامه تلفیق اجتماعی استفاده شد. سپس دادهها با نرم افزار SPSSمورد پردازش قرار گرفتند. یافتهها: براساس نتایج بررسی شدت و جهت ارتباط سنجهها، یکپارچگی در فعالیتهای هدفمند با تمام سنجههای لوتکا و کانتب وجود دارد. این سنجه با جهتگیری، ادراک بینایی و پراکسی حرکتی از آزمون لوتکا و DMS از آزمون کانتب ارتباط معنیدار داشت. از نکات قابل ذکر نتایج آن میتوان به همبستگی بالای میان یکپارچگی در خانه پرسشنامه تلفیق اجتماعی با آزمون Stocking of Cambridge از کانتب (r: 0/353)، یکپارچگی در اجتماع پرسشنامه تلفیق اجتماعی با آزمون Delayed Matching to Sample از کانتب (r: 0/538)، یکپارچگی در فعالیتهای هدفمند و مولد پرسشنامه تلفیق اجتماعی با پراکسی حرکتی از لوتکا (r: 0/422) اشاره کرد. نتیجهگیری: با توجه به نتایج حاصل شده، نشان میدهد هر دونوع آزمون با پرسشنامه مشارکت همبستگی مناسبی دارند شاید بتوان از آزمون کامپیوتری که همبستگی خوبی با مشارکت دارد، در کنار آزمون عملکردی استفاده کرد.
Objective: Multiple sclerosis is a disorder that endanger the central nervous system. In people with MS, whose cognitive impairments have a direct and significant impact on their performance in the activities of daily living, and hence the participation is disrupted. Then exploring cognitive domains can help us to make clinical decisions and interventions. In order to cognitively rehabilitate these people and to be able to estimate the relationship between cognitive performance (computer test and performance test) and the level of participation, we should the correlation between the two to examine. The aim of this study is to determine the correlation between participation and two types of cognitive assessments, that's mean computerized and functional.
Methods: The present study was a cross-sectional correlational study and available sampling. The target population was patients with MS admitted in the MS center of Isfahan, provided they met the criteria for admission to the study in the winter of 1399. Forty-five patients with available MS (9 males and 36 females) with an Expanded Disability Status Scale between 1 and 5 were selected. To assess their cognitive performance, Cambridge Neuropsychological Test Automated Battery and The Lowenstein Occupational Therapy Cognitive Assessment functional test were used and also a Community integration questionnaire (CIQ) was used to determine the level of participation. The data were then processed with Statistical Package for Social Science software.
Results: Based on the results of the study of the intensity and relationship between the measures, there is integration in targeted activities with all measures of LOTCA and CANTA. This measure had a significant relationship with orientation, visual perception and motor praxis from LOTCA test and DMS from CANTAB test. Notable results include a high correlation between home integration of the Community Integration Questionnaire and the Stocking of Cambridge test from CANTAB (r: 0.353), and social integration of the Community Integration Questionnaire with the Delayed Matching to Sample test from CANTAB (r: 0.538), integration in purposeful and productive activities of the Community integration questionnaire with motor proxy from LOTCA (r: 0.422).
Conclusion: According to the results, which show that both types of tests have a high correlation with the CIQ and can be used as a computer test that has a good correlation with participation, instead of the performance test.
