سابقه و هدف: بیماریهای منتقله از طریق غذا یکی از مشکلات بهداشتی عمده در کشورهای مختلف محسوب می گردند. شاغلین در مراکز تهیه مواد غذایی درصورت عدم رعایت اصول بهداشتی، می توانند نقش مهمی در انتقال این بیماری ها داشته باشند. این مطالعه با هدف تعیین فراوانی سالمونلا و شیگلا و انگل های روده ای در متصدیان تهیه ی مواد غذایی شهر کرمان انجام گرفت.
مواد و روش ها: مطالعه از نوع توصیفی مقطعی است که در بازه ی زمانی سال 1390 لغایت 1391 انجام گرفت. تعداد 450 نفر از افراد شاغل در آشپزخانه های مراکز تهیه ی مواد غذایی انتخاب شدند. با مراجعه به محل کار این افراد، در سه نوبت نمونه ی مدفوع گرفته شد و با دو روش آزمایشگاهی مستقیم و رسوبی فرمالین اتر، آزمایش شدند. جهت تشخیص سالمونلا و شیگلا، از روش کشت مدفوع استفاده شد. همزمان، پرسشنامه ای که شامل اطلاعات فردی و شغلی این افراد بود، تکمیل گردید. داده ها با نرم افزار SPSS و آزمون مربع کای، تجزیه و تحلیل شد. 05/0P<، معنی دار در نظر گرفته شد.
یافته ها: از 450 فرد مورد مطالعه، 6/59% مرد و 4/40% زن بودند. فراوانی کلی انگل های روده، 7/14% به دست آمد. از این تعداد، 8/13% آلوده به یک انگل و 9/0% آلوده به دو انگل بودند. بیشترین فراوانی، در عفونت به ژیاردیا 3/7%، انتاموبا کلی 6/3%، همینولوپیس نانا 6/1% مشاهده شد. بین جنس، سن و آلودگی انگلی، از نظر آماری ارتباط معنی داری مشاهده نشد. از نظر استفاده از وسایل بهداشت فردی (دستکش و لباس فرم) و داشتن کارت بهداشتی با آلودگی انگلی، از نظر آماری ارتباط معنی داری وجود داشت (به ترتیب 016/0P و 001/0P<). آلودگی به باکتری سالمونلا، یک مورد (2/0%) و آلودگی به شیگلا سه مورد (7/0%) مشاهده شد.
نتیجه گیری: نتایج، بیانگر شیوع نسبتا بالای آلودگی به انگل های روده ای به ویژه تک یاخته های بیماری زا در بین متصدیان تهیه ی مواد غذایی می باشد. بنابراین لازم است کنترل های بهداشتی بیشتر شده و طبق برنامه های مدون، بیماریابی از طریق آزمایش مدفوع با روش های تغلیظ، به عمل آید. طبیعی است که در این راستا، تداوم و گسترش آموزش در ارتقای سطح سلامت این افراد نقش تعیین کننده ای دارد.
Background and Aim: Food borne diseases are an important public health problem in different countries. Food handlers with poor personal hygiene working in food establishments can have an important role in the transmission of these diseases. This study was undertaken to determine the prevalence of bacteria (Salmonella، Shigella) and intestinal parasites among food handlers in the city of Kerman.
Materials and Methods: This cross-sectional study in years 2011-12 was conducted. 450-food handlers were employed in the kitchen preparing food centers. We recourse to food centers and stool sample were taken in three different time from each person and samples were studied by direct observation and formalin-ether concentration method for intestinal parasites and culture for detected bacteria (Salmonella and Shigella). At the same time، a questionnaire were with variables such sex، age، education، use gloves and form، restroom and health cart filled. The data analyzed by statistical software SPSS.
Results: Among 450 food handlers، included females 40.4% and male 59.6%. The prevalence of intestinal parasites is 14.7% that 13.8% were infected with 1 parasite and 0.9% showed infection to 2 parasites. The highest prevalence is related to Giardia lamblia 7.3%، Entamoba coli 3.6%، Heminolopis nana 1.6%. There was no significant effect between age and sex with parasitic infection (Ρ=0.087 and Ρ=0.054، repectively). Whereas، using the protective facilities (gloves and uniforms) and having health cart، both significantly reduced the parasitic infection (P=0.016 and P<0.001، respectively). The Shigella isolated from 3 (0.7%) cultures and Salmonella isolated from 1 (0.2%) culture.
Conclusion: The results show that the prevalence of intestinal parasitic infections are relatively high، especially pathogenic protozoa in food handler. Therefore، health controls should be increased and screening with concentration method should be done. It is natural in this context، the continuation and expansion of health education has a crucial role.
