هدف از این بررسی تعیین شیوع هپاتیت C در بیماران دیالیزی سطح شهر تهران بود. این مطالعه به صورت توصیفی بر روی تعداد 266 بیمار دیالیزی مراجعه کننده به پنج بیمارستان منتخب انجام گرفت. در طول مدت یک ماه تمام بیماران دیالیزی مراجعه کننده به مراکز مربوط وارد مطالعه شدند. پرسش نامه هایی که حاوی مشخصات فردی و عوامل خطر ابتلا به هپاتیت C بود درهر بیمارستان توسط یک پرستار تکمیل گردید. کلیه سرم های جمع آوری شده از بیماران توسط یک ایمونولوژیست تحت آزمون ELISA 3.0) Enzyme Linked Immuno Sorbant Assay 3.0) و تعداد 23 مورد از میان دارندگان آنتی بادی مثبت به طور تصادفی تحت آزمون RIBA II) Recombinant Immuno Blott Assay II) قرار گرفتند. افراد مورد بررسی دارای میانگین سنی 47.7 سال با دامنه 75-15 بودند، 24.7% با تست تشخیصی ELISA 3.0 مثبت گزارش گردیدند و نتایج RIBAII در 82.6% موارد با بیش از یک آنتی ژن مثبت و در بقیه موارد با یک آنتی ژن مثبت نتایج ELISA را تایید کرد. در این مطالعه سایر عوامل خطر از جمله سابقه خالکوبی، حجامت، زردی و تزریق واکسن هپاتیت B در گروه مبتلا و غیر مبتلا اختلاف معنی داری را نشان نداد. با توجه به آمار به دست آمده، در مقایسه با شیوع این عفونت در جامعه و کنار گذاشتن سایر عوامل خطر، نقش همودیالیز در ایجاد عفونت هپاتیت C در این گروه از افراد جامعه بسیار مهم جلوه می کند.
To determine the prevalence of hepatitis C antibody in hemodialysis patients, 266 subjects referred to one of five selected hospitals in Tehran were included in this study. In each hospital, a trained nurse filled out a 15-item questionnaire addressing demographic characteristics and risk factors for each case. Enzyme-linked immunosorbant assay (ELISA) was performed on each frozen serum sample and 23 randomly selected antibody-positive samples were tested using recombinant immunoblot assay (RIBA). The mean age of the patients studied was 47.4 years. ELISA testing was posivite in 24.7%. Of the positive sera tested for RIBA II, 82.6% had at least two positive antigens. Other risk factors, including tattooing, hajamat (traditional phlebotomy), acupuncture, history of surgery of hepatitis, and hepatitis B vaccination showed no statistical difference in the positive and negative cases. Considering the significant difference of these results compared to the very low prevalence of this infection in the normal population, along with the exclusion of other possible risk factors, we conclude that hemodialysis has a significant role in the transmission of hepatitis C.
