با توجه به شیوع سرطان پروستات و نظر به گزارشات متفاوت از قدرت برش برداری متداول در تشخیص آن و به منظور تعیین شیوع سرطان پروستات در مبتلایان به بزرگی خوش خیم پروستات و تعیین قدرت روش برش برداری متداول نسبت به روش متوالی، تحقیق حاضر در افراد مبتلا به بزرگی خوش خیم پروستات مراجعه کننده به بیمارستان مدرس تهران انجام گرفت. این تحقیق در مرحله اول توصیفی و در مرحله دوم کارآزمایی بالینی از نوع تشخیصی می باشد که در 194 بیمار مبتلا به بزرگی خوش خیم پروستات که بطور مستمر در سال 1375 مراجعه نمودند، صورت پذیرفت. بیست و سه نفر به دلایل مختلف از مطالعه کنار گذاشته شدند. بافت رزکت شده از طریق مجرا (TUR-P) در 171 نفر باقیمانده به دو روش متداول و متوالی مطالعه آسیب شناسی گردید. در روش متداول تعداد بلوک ها و برش های تهیه شده 0.9±2.20 و در روش متوالی تعداد بلوک ها 3.1±8.5 و تعداد برش ها 3.6±9.7 بود. شیوع سرطان پروستات در روش متداول 2.9% و شامل سه مورد Stage A1 و 2 مورد A2 و در روش متوالی 7.6% شامل 8 مورد A1 و 5 مورد A2 می باشد (P<0.05). با احتمال 95% شیوع Stage A سرطان پروستات با روش متداول از 0.4 تا 5.4 و با روش متوالی از 3.6 تا 11.6 درصد در جامعه برآورد می گردد. بر مبنای روش متوالی، ارزش پیشگویی کننده مثبت روش متداول در تعیین شیوع سرطان 100%، در تعیین A1 برابر 50% و در تعیین A2 برابر 100% بود و ارزش پیشگویی کننده منفی آن در تعیین شیوع سرطان 95، در تعیین A1 برابر 95.8 و در تعیین A2 برابر 98.8 درصد بود. شیوع سرطان پروستات در جامعه مورد بررسی بیش از کشورهای خاور دور و کمتر از کشورهای غربی می باشد. روش برش برداری متوالی تمام بافت پروستات برای تشخیص سرطان و Staging آن توصیه می گردد.
The prevalence of prostate cancer and the conflicting reports regarding the power of routine biopsies in its diagnosis, led us to conduct the following study in patients with benign prostatic hyperplasia (BPH) referred to Tehran’s Modarress Hospital, to determine the prevalence of prostate cancer and the diagnostic power of routine biopsy compard to serial biopsy in patients with BPH. The study was descriptive in the first step, followed by a diagnostic clinical trial. One hundred ninety four patients with BPH who regularly came to the clinic in 1996 were studied. Twenty-three were excluded due to various reasons. In the 171 remaining patients, pathology studies were performed on tissue resected through the urethra (TUP-P) by the routine and serial methods. The number of blocs and sections was 2.2±0.9 in the routine method, and there were 8.5±3.1 blocs and 9.7±3.6 sections in the serial method. Prevalence of prostate cancer was 2.9% in the routine method, with 3 cases of stage A1 and 2 cases of stage A2. In the serial method, the prevalence was 7.6%, with 8 cases of A1 and 5 cases of A2 (P<0.05). Prevalence of this stage of prostate cancer in the general population was estimated, with 95% confidence, to be 0.4 to 5.4 percent by the routine method and 3.6 to 11.6 percent by serial biopsy. Compared to serial biopsy, the positive predictive value of the routine method was 100% for determining the prevalence of cancer, 50% for stage A1, and 100% for stage A2. The negative predictive value was 95.2% for cancer, 95.8% for A1 and 98.8% for A2. Therefore the prevalence of prostate cancer in the study population was higher compared to the Far East, but lower than Western countries. We recommend serial biopsy of whole prostate tissue for cancer diagnosis and staging.
