مقدمه: یکی از عوارض دیررس عمده ناشی از مواجهه با سولفورموستارد، عوارض ریوی است. به نظر می رسد که پاسخ های ایمنی و التهابی در ایجاد عوارض کلینیکی نقش دارند.IL-22 عضوی از سایتوکاین های خانوادهIL-10 است که نقش آن در بسیاری از بیماری های التهابی ریوی به اثبات رسیده است.هدف این مطالعه، بررسی رابطه سطح سرمی IL-22 و عوارض دیررس ریوی جانبازان شیمیایی مواجهه یافته با سولفور موستارد بود.
مواد و روش ها: در این مطالعه 219 نفر از جانبازان شیمیایی دارای مشکلات جدی ریوی به عنوان گروه مورد و 54 نفر از افراد سالم به عنوان گروه کنترل، با استفاده از روش نمونه گیری سیستماتیک انتخاب شدند. بیماران گروه مورد تحت معاینات کامل ریوی و اسپیرومتری قرار گرفتند و بر اساس معیار طبقه بندی کمیسیون پزشکی بنیاد شهید و امور ایثارگران به دو گروه دارای عوارض شدید (142 نفر) و دارای عوارض خفیف تا متوسط ریوی (57 نفر) طبقه بندی گردیدند. همزمان با معاینات، 2 سی سی نمونه خون از بیماران گرفته و سرم آن جداسازی شد و در دمای 70- درجه فریز گردید. سپس سطح سرمی IL-22 ، با استفاده از کیت R&D، اندازه گیری گردید.
نتایج: سطح سرمی IL-22در گروه مورد و گروه کنترل از نظر آماری اختلاف معنی داری نداشت. همچنین تفاوت معنی داری میان سطح سرمی IL-22 در گروه دارای عوارض شدید ریوی با گروه دارای عوارض خفیف و متوسط مشاهده نگردید. در این بررسی ارتباط معناداری میان سطح سرمی IL-22 و سن بیماران وجود داشت که نشان می داد با افزایش سن میزان این سایتوکاین در سرم بیماران کاهش یافته است.
بحث: اگرچه سطح سرمی IL-22 بین دو گروه مورد و شاهد،تفاوت معنی دار آماری را نشان نداد، ولی این یافته دخیل بودن IL-22 درپاتوژنز عوارض دیررس گاز خردل در ریه را رد نمی کند، چرا که شاید بیانIL-22 در ریه به صورت موضعی دچار تغییر شده باشد، لذا بررسی این سایتوکاین بصورت موضعی نیز توصیه می شود.
Introduction: Respiratory tract complications are considered as one of the most serious long-term effects of sulfur mustard (SM). Immune responses and inflammatory mediators seem to play the primary role in clinical complications. IL-22 - a member of IL-10 family of cytokines - has important role in various pulmonary clinical situations. In this study، the association between serum levels of IL-22 and long term pulmonary complications in SM exposed population was assessed. Materials and methods: This study was conducted on 219 seriously sulfur mustard lung injured and 54 unexposed healthy controls. The clinical evaluations and Spirometry were done for all patients. Severity of pulmonary involvement was assessed by the classification of medical committee of veteran’s affair. The exposed subjects were categorized into two major subgroups [sever (n=142) and mild-moderate (n=57)] based on spirometry findings. 2 ml of peripheral blood was drawn into vacutainer tubes; the sera were separated and kept frozen at −70 °C until laboratory tests. Human IL-22 DuoSet® ELISA Development Kits (R&D Systems) were used to measure the IL-22 levels in the sera. Results: There were no significant differences between the serum level of IL-22 in the SM exposed participants and the control group، and no differences was found between the groups with sever and mild-moderate lung complications. However، there were a significant correlation between serum levels of IL-22 and age. Discussion: Although there were no significant differences in the serum levels of IL-22 in the SM exposed participants compared to the control group، but this finding doesn’t roll out the role of IL-22 in pathogenesis of long term respiratory complications of sulfur mustard، because it may have local significant changes in lung. Therefore، locally assessment of IL-22 would be valuable.
