خلاصة:
مقدمه: در طول جنگ تحمیلی عراق علیه ایران، بارها از گاز خردل (سولفور موستارد) به عنوان سلاح شیمیایی علیه ایرانیان استفاده گردید. با گذشت سالهای طولانی از مواجهه با گاز خردل افراد مواجهه یافته هنوز از عوارض دیررس مواجهه شامل مشکلات چشمی، پوستی و ریوی رنج می برند. (SDF-1α (Stromal-Derived Factor 1α یا CXCL12، کموکاینی است که در مطالعات متعدد، نقش مهم آن در فرآیند رهاشدن سلول های پیشساز و بنیادی از مغز استخوان به خون محیطی پس از ایجاد آسیب یا التهاب در بدن، مورد تایید قرار گرفته است.
هدف: هدف از این مطالعه ارزیابی میزان SDF-1α در نمونه های سرم جانبازان شیمیایی سالها پس از مواجهه با گاز خردل می باشد.
مواد و روش ها: مطالعه بر روی 174 نفر جانباز شیمیایی گاز خردل و 39 نفر از افراد غیرشیمیایی به عنوان گروه کنترل انجام گردید. ارزیابی بالینی توسط متخصصین انجام گردید، عملکرد ریه با استفاده از اسپیرومتری انجام و جانبازان شرکت کننده براساس طبقه بندی کمیسیون پزشکی بنیاد شهید و امور ایثارگران به سه گروه با آسیب ریوی خفیف، متوسط و شدید تقسیم گردیدند. میزان SDF-1α در سرم به روش ساندویچ الایزا اندازه گیری گردید . یافته ها: سطح سرمی کموکاین SDF-1α در جانبازان شیمیایی نسبت به گروه کنترل کاهش معناداری یافته بود(0.046=P). میزان کموکاینSDF-1α در سرم افراد مواجهه یافته با آسیب شدید ریوی معنادار بود(0.054=P). این کاهش در دو گروه با آسیب ریوی خفیف و متوسط معنادار نبود.
بحث و نتیجه گیری: با توجه به کاهش معنادار سطح سرمی کموکاین SDF-1α در افراد مواجهه یافته با سولفورموستارد نسبت به افراد گروه کنترل غیر مواجهه، همچنین با توجه به معنی دار بودن این کاهش در افراد دارای آسیب های ریوی شدید می توان نتیجه گرفت کاهش میزان این کموکاین می تواند یکی از عوامل موثر در عدم ترمیم ضایعات دراز مدت سولفورموستارد باشد. کلید واژه: کموکاین(SDF-1α (CXCL12، گاز خردل، جانبازان شیمیایی، مشکلات ریوی.
Background & Purpose: Sulfur Mustard (SM) has been widely used as a chemical warfare agent against Iranian in Iraq-Iran conflict. Many years after exposure، people who have been exposed to SM are still suffering from its late complications including ocular، coetaneous، respiratory and psychological disorders. SDF-1α(Stromal-Derived Factor-1α) or CXCL12 is a member of C-X-C chemokine family that its important role in recruitment of progenitor and stem cells from bone marrow to peripheral blood after injury and inflammation has been approved in many studies. The aim of this study was to determine the serum levels of SDF-1α in chemical victims who were exposed to mustard gas many years ago in comparison with an unexposed control group. Materials & Methods: 174 SM chemical victims and 39 unexposed participants as controls were studied. Clinical evaluation was down by specialists. Pulmonary function was carried out by spirometry and according to the classification of Medical Committee of Iranians foundation of martyr and veterans affairs classifications، chemical victims classified to three groups with mild، moderate and severe pulmonary complications. The serum levels of SDF-1α were measured by a sandwich ELISA technique. Results: The serum concentration of SDF-1α in SM exposed group was significantly reduced compared to the controls(p=0.046). The serum SDF-1α levels in the exposed participants with severe pulmonary complications was significantly lower than in controls(p=0.054). However this reduction was not significant in exposed group with mild and moderate lung injuries. Conclusion: According to the significant reduction of the serum concentration of SDF-1α in the exposed participants comparing to the unexposed control group and its significant reduction in exposed participants with severe lung injury، it could be concluded that down-regulation of this chemokine could be one of the important factors in late clinical complications of sulfur mustard.
