هدف: کف پای صاف بهعنوان یکی از شایعترین ناهنجاریهای اندام تحتانی، میتواند الگوی راهرفته را تحت تاثیر قرار دهد. کفی طبی رایجترین راهکار درمانی است که برای این ناهنجاری استفاده میشود؛ با این وجود اثرات بیومکانیکی استفاده از کفی طبی به طور واضح مشخص نیست. هدف از انجام این تحقیق، بررسی اثرات فوری کفی طبی پیشساخته بر تغییرپذیری و الگوی کوپلینگ مفصلی پشت پا و ساق در مرحله اتکای راه رفتن مردان فعال مبتلا به کف پای صاف بود. روش ها: هشت مرد فعال مبتلا به کف پای صاف آزمودنیهای این تحقیق نیمه تجربی را تشکیل دادند. دادههای کینماتیک راه رفتن این آزمودنیها بهوسیله هشت دوربین آنالیز حرکت ضبط شد و پس از محاسبه زوایای سهبعدی، از روش وکتورکودینگ برای ارزیابی کوپلینگ استفاده گردید. از آزمونهای آماری شاپیرو-ویلک برای بررسی نرمال بودن توزیع فراوانی دادهها و آزمون تی همبسته برای مقایسه میانگینها استفاده شد. یافته ها: نتایج نشان داد که در سه زیر فاز اول مرحله اتکای راه رفتن تفاوت معناداری در میانگینهای کوپلینگ مفصلی، بین شرایط کفی و بدون کفی وجود ندارد (0/05<P) درحالیکه در زیر فاز چهارم میانگین زاویه کوپلینگ 19 درصد کاهش معنادار نشان داد (0/05>p). همچنین تفاوت میانگین تغییرپذیری کوپلینگ مفصلی در هیچ از زیر فازها در هر دو شرایط با و بدون کفی طبی تفاوت معناداری نداشت (0/05<P). نتیجهگیری: با استفاده از نتایج این تحقیق میتوان گفت که استفاده از کفی طبی بر الگوی کوپلینگ پشت پا ساق در 75 درصد ابتدایی مرحله اتکای راه رفتن اثری ندارد ولی در 25 درصد انتهایی باعث کاهش زاویه کوپلینگ میشود. بنابراین به نظر میرسد که در انتهای مرحلهی اتکای راه رفتن، حرکات ساق نسبت به پاشنه کاهش مییابد و این تغییر میتواند، کاهش درد زانو و دیگر اثرات سودمند استفاده از کفی طبی در افراد مبتلا به کف پای صاف را توجیه کند.
Objective: The purpose of this study was to investigate the immediate effects of pre-made foot orthotics on rear-foot and tibia variability and joint coupling patterns during stance phase of gait in male athletes with pes planus.
Methods: Subjects of this semi-experimental research included 8 active men with pes planus. Kinematic gait data of these subjects were recorded by a motion analysis system with 8 cameras. After calculating three-dimensional angles، vector coding method was used for assessing coupling. Shapiro-Wilk test was used to test the normal distribution of data and dependent samples t-test was used to compare means in each sub-phase.
Results: Results showed that in the first 3 sub phases of stance، there were no significant differences between joint coupling with and without orthotics (P>0.05)، while in the fourth sub-phase the mean coupling angle decreased significantly by 19% (P=0.002-P<0.05). Also، no significant difference was observed among means of joint coupling variabilities in sub phases (P>0.05).
Conclusion: These results suggest that foot orthotics does not have an effect on rear foot and tibia joint coupling patterns in the first 75 percent of stance phase of gait، but leads to a decrease in these patterns in the last 25 percent. Thus، it seems that movements of the shank decrease relative to heel movements in terminal stance phase، and this change can explain reduction in knee pain and other beneficial effects of using foot orthotics in subject with pes planus.
