خلاصة:
اهداف: یکی از شایعترین حوادث، سوانح ناشی از رانندگی است، بهطوری که حوادث جادهای دومین علت مرگ و عمر ازدسترفته در ایران است. هدف این مطالعه تعیین بقای مصدومان حوادث ترافیکی بستری و ارزیابی کیفیت مراقبتهای بیمارستانی در بیمارستان امامخمینی ارومیه با استفاده از روش TRISS بود.
ابزار و روشها: این مطالعه مقطعی از نوع توصیفی- تحلیلی روی ۷۶۰ مصدوم تصادفی بستری در بیمارستان امامخمینی ارومیه طی سال ۱۳۹۵ انجام شد که بهصورت هدفمند انتخاب شدند. مقادیر تعداد تنفس، فشار خون سیستولیک و مقیاس کمای گلاسکو در بدو ورود به بیمارستان و شدت صدمات بیمار براساس گزارش متخصص مربوطه جمعآوری و با استفاده از روش TRISS احتمال بقا برای بیماران محاسبه شد. برای مقایسه احتمال بقا، آماره Z و W مورد استفاده قرار گرفت.
یافتهها: کل افراد مصدوم، ۵۶۲ نفر (۷۳/۹%) مرد و ۱۹۸ نفر (۲۶/۱%) زن بودند. ۷۴ نفر (۹/۷%) بین صفر تا ۱۴ سال، ۴۳۷ نفر ( ۵۷/۵%) بین ۱۵ تا ۵۴ سال و ۲۴۹ نفر (۳۲/۸%) بالای ۵۴ سال سن داشتند. میانگین RTS برای افراد زنده ۰/۵۵±۷/۶۷ و برای افراد فوتشده ۱/۵۷±۶/۰۰ و میانگین ISS برای افراد زنده ۱۷/۶۳±۲۱/۶۲ و برای افراد فوتشده ۲۵/۰۲±۵۶/۳۲ بود. همچنین ۶۰ مرگ بهدنبال ترومای ناشی از حادثه رانندگی مورد انتظار بود، در حالی که ۶۹ مرگ رخ داده بود.
نتیجهگیری: تعداد مرگهای رخداده بیشتر از تعداد مرگهای مورد انتظار است که نشاندهنده پایینبودن کیفیت مراقبتهای بیمارستانی است.
Aims One of the most common events is traffic accidents. Road accidents are the second death
cause and age loss in the country. The aim of this study was to determine the survival rate of
traffic accident victims and assessing the quality of hospital care in Imam Khomeini Hospital,
Urmia by using TRISS method.
Instruments & Methods This cross sectional study was performed on 760 accident victims
hospitalized to Imam Khomeini Hospital of Urmia in 2017; they were selected purposefully.
Respiratory rate, systolic blood pressure, Glasgow coma scale (GCS), and the severity of injuries
of the patient were collected based on the relevant expert report, and using the TRISS software,
the probability of survival for patients were calculated. Z and W static were used to compare
survival probability.
Findings The number of male injured individuals were 562 (73.9%) and female were 198
(26.1%). 74 (9.7%) injured individuals were between 0 and 14 years old, 437 (57.5%)
individuals were between 15 and 54 years old, and 249 (32.7%) individuals were over 54
years old. The mean of RTS was 7.67±0.55 and 6.00±1.57 for the survived and non-survived
individuals, respectively. The mean of the ISS was 21.62±17.63 and 56.32±25.02 for the
survived and non-survived individuals, respectively. Also, the expected death was 60, and the
observed death was 69.
Conclusion The number of observed deaths is more than the expected ones, indicating the low
quality of hospital care.
