خلاصة:
مقدمه: ارائه خدمات توانبخشی مطلوب به معلولان همواره از دغدغههای مهم نظام سلامت در جهان محسوب میگردد. شناسایی نظامهای توانبخشی معلولان برای سیاستگذاران و متولیان سلامت و توانبخشی یک ضرورت اجتناب ناپذیر است. در این میان پرداختن به مطالعات تطبیقی و بهره مندی از تجارب کشورهای موفق بسیار مفید خواهد بود. این مطالعه با هدف مقایسه تحلیلی نظامهای ارائه خدمات توانبخشی به معلولان در ایران با سایر کشورهای مطرح در زمینه توانبخشی طراحی و اجرا گردید.
روش بررسی: این پژوهش به صورت تطبیقی در سال 1399 انجام گردید. جهت تعیین چارچوب مقایسه جامعه پژوهش از چهار بعد عملکرد نظامهای سلامت بر مبنای گزارش سال 2000 سازمان جهانی بهداشت به عنوان الگو استفاده شد و نه کشور در ابعاد تولیت، تولید منابع، تامین مالی و تدارک خدمات مورد مقایسه قرار گرفتند. یافته های به دست آمده از هر کشور به تفکیک ابعاد مذکور در جداول تطبیقی ارائه گردید. الگوی مورد استفاده در این مطالعه، الگوی بردی و هیلکر بود که چهار مرحله توصیف، تفسیر، همجواری و مقایسه را در مطالعات تطبیقی مشخص می کند. در پژوهش حاضر، نمونهگیری هدفمند مورد استفاده قرار گرفت. جامعه آماری شامل نظامهای توانبخشی در جهان بود و نمونه پژوهش نظام توانبخشی کشورهای ایران، چین، ترکیه، هندوستان، آمریکا، مکزیک، آلمان، انگلیس و آفریقای جنوبی بودند. ملاکهای ورود بر حسب دسته بندی جغرافیایی، انتخاب حداقل یک کشور از قاره های آمریکا، اروپا، آسیا و آفریقا و بر حسب مدل کارکردی سازمان جهانی بهداشت، قابل دسترس بودن داده های مربوط به ابعاد چهارگانه عملکرد نظامهای سلامت بود.
یافته ها: نتایج مطالعه نشان داد، تولیت توانبخشی معلولان در نظام ارائه خدمات توانبخشی در ایران با سازمان بهزیستی بوده، ولی در اغلب کشورها وزارت سلامت متولی توانبخشی معلولان بود. از نظر تولید منابع، تمامی کشورهای مورد مطالعه در زمینه تربیت نیروی انسانی متخصص از طریق نظام دانشگاهی اقدام می نمودند. همچنین در ایران، هندوستان و مکزیک دسترسی به داده های ملی جامع به طور کامل وجود نداشت، ولی در آمریکا، آلمان، چین، آفریقای جنوبی و ترکیه این بستر جهت برنامه ریزی و سیاستگذاری فراهم بود. تامین مالی در کشورهای آلمان، آمریکا، چین و ترکیه بیشتر از طریق نظام بیمه ای، در انگلیس بیشتر از محل مالیات ولی در ایران از طریق پرداخت یارانه از محل درآمدهای عمومی و مالیات صورت می گرفت و در نهایت تدارک خدمات در ایران علاوه بر فعالیت بخش خصوصی، در بخش دولتی به صورت غیرمنسجم توسط نهادهای مختلف از جمله وزارت بهداشت، درمان و آموزش پزشکی، سازمان آموزش و پرورش استثنایی، هلال احمر، سازمان بهزیستی و بنیاد شهید و امور ایثارگران و در سایر کشورها اغلب از طریق بیمارستان ها و بخش خصوصی ارائه می گردید. در اغلب کشورهای مورد مطالعه توانبخشی معلولان با بخش بهداشت و درمان دارای پیوستگی قابل ملاحظه ای بود.
نتیجه گیری: علیرغم نقاط قوت متعدد در نظام ارائه خدمات توانبخشی به معلولان در ایران، ادغام بخش توانبخشی سازمانهای موازی در ایران در وزارت بهداشت و درمان، علاوه بـر افـزایش هماهنگیهای درون و برون بخشی و انسجام مدیریت و سیاستگذاری واحد، می تواند موجـب کـاهش هزینـه هـا، تخصیص صحیح و افزایش منابع مالی این بخش شود. علاوه بر این، میتواند منجر به کاهش همپوشانی وظـایف و مـسئولیت هـا و جلـوگیری از دوبـاره کـاری، تربیت نیروی متخصص متناسب با نیاز و نهایتا ارائه خدمات مطلوب تر گردد. بنابراین ضروری است سیاستگذاران و برنامه ریزان این حوزه با دید وسیع تری نسبت به بازنگری و اصلاح ساختار، قوانین و فرآیندها در راستای حرکت به سوی شکلگیری نظام ارائه خدمات توانبخشی به معلولان در ایران اقدام نمایند.
Objective: Providing desirable rehabilitation services to the people with disabilities has been always one of the most important concerns of the health system in the countries. Today, it is important for policymakers and health care providers recognize the different systems for providing rehabilitation services to people with disabilities. In this regard, it will be very useful to conduct comparative studies and use the experiences of successful countries. The purpose of this comparison-analytical study was to compare the systems for providing rehabilitation services to people with
Materials & Methods: This comparative study was conducted about nine countries in 2020. In order to determine the comparison framework in this research, four dimensions of the functions of health systems were used based on the 2000 report of the World Health Organization and countries were compared in dimensions of stewardship, resource production, financing and service provision. Findings obtained from each country were presented separately in the comparative tables. The model used in this comparative study was based on Beredy model that contain four stages include description, interpretation, proximity, and comparison. In the present study, purposive sampling was used. The statistical population included rehabilitation systems in the world and the research examples were Iran, China, Turkey, India, USA, Mexico, Germany, England and South Africa. Criteria for entering the research according to the functional model of the World Health Organization were the availability of data and By geographical category, select at least one country from the Americas, Europe, Asia and Africa.
Results: The findings showed that in Iran, the Welfare Organization was responsible for providing rehabilitation services for people with disabilities, while in other countries it was the Ministry of Health. Also, to compare resource production, two indicators of staff training and access to comprehensive national data were used. In all countries studied, the training of specialized human resources is done through the university system. Iran, India, and Mexico did not have full access to comprehensive national data, but the United States, Germany, China, South Africa, and Turkey provided the platform for planning and policy-making. Moreover, funding in Germany, the United States, China and Turkey is mostly provided through the insurance system, In the United Kingdom is mostly provided through the tax, but in Iran it is provided through subsidies from the public revenues and taxes. Finally, in addition to the private sector, rehabilitation services in the public sector in Iran was provided in an incoherent manner by various organizations, including the Ministry of Health and Medical Education, the Exceptional Education Organization, the Red Crescent, the State Welfare Organization, and the Martyrs and Veterans Affairs Foundation, while in other countries often provided these services through hospitals and the private sectors.
Conclusion: Despite the numerous strengths in the system of providing rehabilitation services to the disabled in Iran, the integration of the rehabilitation sectors of various organizations within the Ministry of Health not only increases the coordination of inter-sectoral and intra-sectoral coordination, the coherence of management and unified policy, but also causes, leads to reduced costs, proper allocation of resources and increased financial resources in this sector. In addition, it leads to a reduction in overlapping tasks and responsibilities and prevention of duplication of work, training of specialized personnel based on the needs, and finally providing more desirable services. Therefore, it is necessary for policymakers and planners to review and reform the laws and processes in order to move towards the formation of a system for providing rehabilitation services to the disabled in Iran.
