خلاصة:
زمینه و هدف: توسعه مراقبتهای اولیه سلامت با بهرهگیری از برنامه پزشکی خانواده و زنجیره ارجاع، استراتژی اثربخشی است که در آن شاخصهای دسترسی و بهرهمندی به صورت جامع و عادلانه، بهبود می یابد. با توجه به وجود شکاف بین قوانین و سیاستهای برنامه پزشک خانواده و نحوه پیادهسازی این برنامه و همچنین ضرورت اجرای اثربخش برنامه پزشکی خانواده در ایران، مطالعه حاضر با هدف شناسایی الزامات و راهبردهای مورد نیاز انجام شد. روش: یک مطالعه کیفی در سال ۱۴۰۰ انجام شد. مشارکتکنندگان در پژوهش را سیاستگذاران و مدیران وزارت بهداشت درمان و آموزش پزشکی و سازمانهای بیمهگر و اعضای هیئت علمی دانشگاههای علوم پزشکی بودند. دادهها براساس راهنمای مصاحبه و طی ۱۰ جلسه بحث گروهی متمرکز جمعآوری و با استفاده از روش تحلیل محتوا تحلیل شد. یافتهها: الزامات مورد نیاز برای پیادهسازی برنامه پزشکی خانواده در 6 عنوان اصلی (سیاسی، فرهنگی - اجتماعی، اقتصادی، آموزشی، مدیریتی و اجرایی) و 27 عنوان فرعی طبقهبندی شد. راهبردهای مورد نیاز نیز در 3 عنوان اصلی (مدیریت شبکههای بهداشتی، درمانی شهرستان و استان، اجرای برنامههای سلامت و آموزش پزشکان خانواده، مدیران و جامعه) و 28 عنوان فرعی استخراج شد. نتیجهگیری: نتایج حاصل از مطالعه نشان میدهد که باید با ایجاد عزم و اراده مناسب استراتژیهای اجرایی در مدیریت برنامه و پایش و ارزشیابی مورد توجه قرار گیرد. همچنین توجه به بهرهگیری از ظرفیت بخش غیردولتی و جانمایی آن در ساختار شبکههای بهداشتی درمانی و تقویت همکاریهای درونبخشی و بینبخشی می تواند راهکارهای اجرایی مناسب باشد.
Background: Indicators such as access and benefit from the comprehensive and basic health services package are improved by primary health care development and using family medicine program and referral chain as an effective strategy. Due to the gap between the rules and policies of the family medicine program and its implementation, this study aims to identify the requirements and strategies for the implementation of the program in Iran.
Methods: A qualitative study was conducted in 1400. Participants in the study were policy makers and managers of the Ministry of Health and insurance organizations and faculty members of medical universities. Data were collected based on the interview guide during 10 sessions of focused group discussion and analyzed by using content analysis method.
Result: The implementing requirements for family medicine program were classified into 6 titles (political, cultural-social, economic, educational, managerial and executive) and 27 sub- titles. The required strategies were extracted in 3 titles (management of health care networks in the city and province, implementation of health programs and training of family physicians, managers and the community) and 28 sub-titles.
Conclusion: The results of the study show that executive strategies in program management, monitoring and evaluation should be considered by creating appropriate determination and will. And appropriate executive solutions could be using the capacity of the non-governmental sector in the structure of health care networks, intra-sectoral and inter-sectoral collaborations.
