مقدمه: هر نوع تومور به عنوان یک خطر موذی برای زندگی در نظر گرفته میشود که تشخیص زودهنگام آن میتواند بر روند درمان و پروسه بهبودی تاثیر مفید داشته باشد. عوامل متعددی تشخیص زودهنگام تومورهای مغزی را به چالش میکشند. از آنجایی که علائم روانپزشکی، ویژگیهای پاتوژنومیک تومورهای مغزی را نشان نمیدهند، ممکن است علت ارگانیک زمینهای نادیده گرفته شود. مطالعه حاضر با هدف تعیین فراوانی اختلالات خلقی و خواب در بیماران مبتلا به همانژیوبلاستومای سیستم عصبی مرکزی پس از انجام عمل جراحی انجام شد. روش بررسی: این مطالعه بهصورت توصیفی-تحلیلی و مقطعی و شامل ۳۲ بیمار مبتلا به همانژیوبلاستوما بود که طی سالهای ۱۳۹۶ تا ۱۴۰۱ تحت درمان جراحی قرار گرفتند. اطلاعات بیماران توسط چکلیست و پرسشنامههای استاندارد جمعآوری شد. برای ارزیابی وضعیت عملکرد از دو مقیاس وضعیت عملکردی کارنوفسکی و ECOG، مقیاس خلقی برونل و پرسشنامه کوتاه خواب MSQ استفاده شد. یافتهها: سن بیماران ۱۵.۱۵±۴۴.۲۸ سال و ۱۸ نفر (۵۶.۳ درصد) مرد بودند. زمان پیگیری بیماران ۳.۵۷±۶.۷۱ سال بود. همچنین میانگین اندازه تومور ۱۲.۹۸±۳۵.۴ میلیمتر بود. ۹ نفر (۲۸.۱ درصد) از بیماران علاوه بر درمان جراحی، تحت رایوتراپی و ۵ نفر (۱۵.۶ درصد) تحت شیمیدرمانی قرار گرفتند. وضعیت عملکردی کارنوفسکی و ECOG در حد مطلوب بود. اختلالات خواب نشاندهنده مشکلات خواب و بیداری در حد متوسط و کلیه ابعاد اختلالات خلق (تنش، افسردگی، خشم، خستگی، سرزندگی و سردرگمی) بیشتر از حد متوسط بود. نتیجهگیری: در مطالعه حاضر نیز مانند سایر مطالعات انجام شده، افسردگی و تنش از عوامل خلقی مهم در بیماران با تومور مغزی بودند. علاوه بر آن تأثیر معناداری را بر وضعیت عملکردی بیماران گذاشته بودند. در این راستا پیشنهاد میشود که مطالعات دقیق و جامعتری برروی اختلالات روانشناختی بیماران با هر نوع تومور مغزی انجام شود و درمانهای روانشناختی از اجزای پروسه درمان این بیماران در نظر گرفته شود.
Introduction: Any type of tumor is considered as an insidious risk to life, and its early diagnosis can be effective in the treatment and recovery process. Several factors challenge the early diagnosis of brain tumors. Since psychiatric symptoms do not reflect the pathogenomic characteristics of brain tumors, the underlying organic cause may be overlooked. The present study was conducted with the aim of determining mood and sleep disorders in patients with central nervous system hemangioblastoma after surgery.
Methods: This descriptive-cross-sectional study included 32 patients with hemangioblastoma who underwent surgical treatment between 2016 and 2016. Patient information was collected by checklists and standard questionnaires. Karnofsky's and ECOG functional status scales, Brunel's mood scale and short sleep questionnaire MSQ were used to evaluate the performance status.
Results: The age of the patients was 44.28±15.15 years and 18 (56.3%) were male. The follow-up time of the patients was 6.71±3.57 years. Also, the average tumor size was 35.4±12.98 mm. 9 patients (28.1%) underwent radiotherapy in addition to surgery and 5 patients (15.6%) underwent chemotherapy. Functional status of Karnofsky and ECOG was optimal. Sleep disorders indicate moderate sleep and wakefulness problems, and all dimensions of mood disorders (tension, depression, anger, fatigue, vitality, and confusion) were more than average.
Conclusion: In the present study, as in other studies, depression and tension were important mood factors in brain tumor patients. In addition, they had a significant impact on the patients' functional status. In this regard, it is suggested to conduct more detailed and comprehensive studies on the psychological disorders of patients with any type of brain tumor and to consider psychological treatments as part of the treatment process for these patients.
