زمینه و هدف: اولویت سیاستگذاران رشته چشمپزشکی، ارتقای کیفیت آموزش و خدمات است. هدف این پژوهش ملی، ارائه راهکارهای خبرگان برای تحقق وضعیت مطلوب در آموزش فلوشیپ چشمپزشکی ایران در افق 10 سال آینده است. روش: این تحقیق کاربردی به شیوه آیندهپژوهی کیفی با مشارکت 11 نفر از مطلعان کلیدی در نشست تحلیلی خبرگان در اسفند 1401، صورت پذیرفت. بحثهای متمرکز پنل به روش تماتیک تحلیل شد. یافتهها: راهکارهای تحقق وضع مطلوب فلوشیپ چشم پزشکی کشور، در چارچوب 14 درونمایه اصلی و 44 درونمایه فرعی طبقهبندی شد. براساس اجماع خبرگان 14راهکار اصلی عبارتند از: ارتقاء کیفیت آموزش، ارتقاء پاسخگویی اجتماعی، ارتقاء اخلاق حرفهای، ارتقاء تحقیقات دردوره فلوشیپ، اصلاح نظام ارزشیابی و اعتباربخشی فلوشیپ، تمرکززدایی در پذیرش فلوشیپ چشم پزشکی، اصلاح ظرفیت پذیرش رشتهها، بازنگری تعداد رشتههای چشم پزشکی، ادغام وحذف بعضی رشتهها، ایجاد نظام برنامهریزی نیروی انسانی، تحلیل هزینه اثربخشی سیاست توسعه، اصلاح تعرفهها به نسبت خدمات فلوشیپ و ریسک ارائه خدمت، بازنگری سیاستهای تشویقی ماندگاری چشم پزشکان، تامین و توزیع عادلانه تجهیزات، ارتقاء زیرساختهای آموزشی نوین چشم پزشکی. نتیجهگیری: راهکارهای اولویتدار در تحقق وضعیت مطلوب آموزش فلوشیپ چشمپزشکی در پاسخ به تقاضای جامعه و نظام سلامت تعیین شد و شایسته است از طریق تجدیدنظر در سیاستها و اصلاح استانداردها و مکانیسمهای عملی توسط برنامهریزان آموزشی برای ارتقاء فلوشیپ در رشته چشم پزشکی اجرایی شود.
Background: Promotion of Ophthalmic Education and practice is one of the main priorities for policy makers in ophthalmology. This study aimed to present expert solutions to accomplish an ideal status for ophthalmic fellowship education in iran over a 10- year horizon.
Methods: In this applied qualitative research, 11 key informants of Iran’s Ophthalmic Fellowship Education were selected using purposive sampling, and invited to participate in an expert panel on February 2023. The data analyzed by thematic approach.
Results: The identified solutions to achieve desired status for fellowship education in ophthalmology were presented in 14 main themes, and 44 subthemes. Based on experts consensu, this study proposed 14 applied solutions including: revising the ophthalmic fellowship curriculum to the highest quality, promoting accountability, promoting professionalism, constant interaction with clinical research sectors, reform in national system of fellowship admission, reform in fellowship program evaluation and accreditation system, decentralization of ophthalmic fellowship admission, Integration of some ophthalmic fellowship disciplines, cutting unnecessary admissions and increasing admissions in required ophthalmic fields, foresight in workforce planning, cost-effectiveness analysis in fellowship programs development, ophthalmic services tariffs reform, ophthalmologists retention policy reform, and investing and building technological infrastructures.
Conclusion: The priority areas and applied solutions to actualize the ideal situation for ophthalmic fellowship education of the highest quality in response to community and health system demands are determined. The expert recommendations should be considered through revision of policies, standards and practical mechanisms by educational planners to promote ophthalmic fellowship education in Iran.
