Abstract:
سابقه و هدف: افت وزن از مهمترین مشکلات سلامتی کودکان در جوامع درحال توسعه است. در بعضی مناطق بیماریهای عفونی و در برخی دیگر عوامل تغذیه ای بعنوان عوامل خطرساز افت وزن کودک معرفی شده اند. از آنجا که شناخت عوامل خطرساز سوءتغذیه و پیشگیری از آنها درکودکان بسیار مهم است، این مطالعه با هدف تعیین فراوانی افت وزن و مهمترین عوامل خطرساز آن تا دوسالگی در کودکان مراجعه کننده به مراکز بهداشتی-درمانی شرق تهران انجام شد.
مواد و روش ها: این مطالعه با استفاده از داده های موجود انجام گرفت. با روش نمونه گیری خوشه ای دومرحله ای 1660 کودک دوساله مراجعه کننده به مراکز بهداشتی انتخاب و اطلاعات لازم از پرونده بهداشتی آنها استخراج گردید. کودکان تک قلو که تولد به موقع داشته و از بیماری خاص در بدو تولد یا بیماریهای ژنتیکی و مادرزادی رنج نمی بردند در صورت مراجعه منظم برای پایش رشد وارد این مطالعه شدند. آزمونهای کای-دو، من ویتنی و تحلیل رگرسیونی لجستیک برای تعیین ارتباط عوامل خطرساز با افت وزن مورد استفاده قرار گرفت و بر اساس مقادیر نسبت شانس مهمترین عوامل خطرساز تعین شد.
یافته ها: دختران 5/55% کودکان را تشکیل دادند. وزن تولد کودکان 463±3202 گرم بود. 1119(4/67%) کودک از ابتدای تولد تا دو سالگی حداقل یک بار افت وزن داشتند. بیشترین فراوانی افت وزن در سنین 9 تا 11 ماهگی مشاهده شد. عوامل خطرساز افت وزن تا 6 ماهگی کمترین بسامد را داشتند. نسبت شانس عمده ترین عوامل خطرساز به ترتیب متعلق به قطع تغذیه کودک با شیرمادر(7/64)، شروع تغذیه تکمیلی (9/16)، ابتلا به اسهال (3/15) و عفونتهای تنفسی (8/11) بود. بین عوامل دموگرافیک و افت وزن کودکان رابطه معنی دار آماری یافت نشد.
نتیجه گیری: فراوانی افت وزن در نمونه مورد مطالعه بالا بوده و با توجه به عوارض شناخته شده آن باید بیشتر مورد توجه قرار گیرد. از آنجا که مهمترین عوامل خطرساز افت وزن کودکان تا 2 سالگی قابل کنترل می باشند به نظر می رسد توانمند سازی مادران در زمینه شیوه های مناسب تغذیه کودک در سلامت و بیماری به کاهش موارد بروز افت وزن کمک کند.
Background and Aim: Weight faltering is one of the most important health problems of children in developing communities. Factors such as infectious diseases in some regions and nutritional factors in the others have been suggested as risk factors for weight faltering. Since determining the risk factors of malnutrition and preventing them in children is crucial، a study was conducted to determine the major immediate risk factors of weight faltering up to 2 years of age in children attending health care centers in east region of Tehran.
Materials and Methods: This study was carried out using existing data. Via cluster sampling 1660 two-year old children attending to 5 health care centers in east region of Tehran were selected and data was gathered from their health files. Term birth، being offspring of a uniparous labor، having no certain post-natal، genetic or congenital diseases and on time attending to health center for growth monitoring were the study inclusion criteria. The chi-square، Mann-Whitney U and logistic regression tests were used to determine relationship between risk factors and weight faltering. The major risk factors were determined by odd's ratio.
Results: Girls consisted 55.5% of the children. Mean of the birth weight was 3202±463 grams. Weight faltering was occurred in 1119 (67.4%) children from birth up to 2 years of life. The great number of children had weight faltering in 9 to 11 months of age. Frequency of risk factors of weight loss was the least during the first 6 months of life. Weaning (OR=64.7)، introducing complementary foods (OR=16.9)، diarrhea (OR=15.3) and respiratory infections (OR=11.8) were the major risk factors. Mother's educational level and job status were not related to weight loss.
Conclusion: Prevalence of weight faltering was high in the studied sample، hence، further attention and survey are strongly recommended. Since the major risk factors of weight faltering up to 2 years of age are controllable، it seems that enabling mothers towards proper feeding of children could help to reduce the prevalence of weight faltering.
