Abstract:
سابقه و هدف: از آنجا که گیرنده های ER، PR و HER-2 نقش مهمی در پیش بینی پاسخ به درمان و پیش آگهی بیماران مبتلا به سرطان پستان دارند و در حال حاضر مطالعات درباره این فاکتورها و ارتباط آنها با مشخصات دموگرافیک و کلینیکی بیماران در کشور ناکافی است، این مطالعه با هدف بررسی ارتباط گیرنده های استروژن، پروژسترون و HER-2 با عوامل پروگنوستیک در مبتلایان به سرطان پستان مراجعه کننده به بیمارستان شهدای تجریش تهران در سال 1383 و 1384 انجام شد.
مواد و روشها: این مطالعه توصیفی- مقطعی در سالهای 84-83 بر روی 138 بیمار زن مبتلا به سرطان پستان از نوع Infiltrating ductal carcinoma که به بخش رادیوتراپی و انکولوژی بیمارستان شهدای تجریش مراجعه کرده بودند انجام شد و کلیه اطلاعات از پرونده بیماران اخذ گردید.
یافته ها: ارتباط معنی دار آماری بین گیرنده های ER، PR، HER-2 و سن بیمار، اندازه تومور و وضعیت درگیری غدد لمفاوی مشاهده نشد. همچنین بین گیرنده های هورمونی ER وPR با گرید تومور ارتباطی معنی دار آماری وجود نداشت. اما بیماران HER-2 مثبت با تومورهای با گرید بالاتر مواجه بودند (05/0 p<). نیز بین گیرنده های ER، PR و گیرنده HER-2 ارتباط آماری معنی دار وجود نداشت. ارتباط بین تومورهای Triple negative (ER-/PR-/ HER2 -) و سن، اندازه تومور، وضعیت درگیری غدد لمفاوی و گرید تومور معنی دار نبود.
نتیجه گیری: چنین به نظر می رسد بیمارانی که وضعیت گیرنده HER-2 در آنها مثبت می باشد تومورهایی با گرید بالاتر دارند، به عبارتی با قدرت تکثیر و تهاجم به سایر نقاط بیشتر و احتمالا طول عمر کوتاهتری مواجه می باشند.
Background and Aim: Studies have shown that breast cancer patients who have estrogen receptor (ER) or progesterone receptor (PR) positive tumors have lower risks of mortality and better response to treatment compared to women with ER and/ or PR negative breast tumors. However، few studies have been performed about these factors and their relation with demographic or clinical characteristics in Iran. So we decided to evaluate the correlation between ER، PR and Human epidermal growth factor 2 (HER-2) with age، tumor size، tumor histological grade and lymph node metastasis as prognostic factors in breast cancer.
Materials and Methods: In this cross sectional study، 138 patients with invasive ductal breast carcinoma who were admitted in Shohadaye Tajrish hospital during 2004-2005 were studied and all information was gathered from patients’ medical records.
Results: Statistically significant correlation between ER، PR، and HER-2 receptors with age، tumor size and lymph node metastasis was not observed. Also correlation between ER، PR and HER-2 receptors with tumor grade was not significant. But there was significant correlation between HER-2 receptors and tumor grade (P<0/05). Correlation between ER and PR with HER-2 receptors was not significant. Also there was not a significant correlation between Triple Negative tumors and age، tumor size، tumor grade، and lymph node involvement.
Conclusion: It seems that patients with HER2 positive tumors tend to face higher grade tumors and metastasis; besides، maybe they face an increased risk of mortality.
