Abstract:
سابقه و هدف: کریپتوسپوریدیوم تک یاخته ای با انتشار جهانی و به عنوان یکی از عوامل مهم اسهال در کودکان و افراد دارای نقص سیستم ایمنی شناخته شده است. عمده تحقیقات انجام گرفته در خصوص این انگل در ایران به بررسی شیوع آن در جوامع مختلف پرداخته است. هدف از این مطالعه تعیین تنوع ژنتیکی کریپتوسپوریدیوم در کودکان مبتلا به اسهال بر اساس آنالیز ژنهای پلی مورف COWP، SSU-rRNA و TRAP-C2 با استفاده از تکنیکPCR- RFLP می باشد.
مواد و روش ها: در این مطالعه مقطعی، طی دو سال (آذر ماه 1384 تا فروردین ماه 1387)، نمونه های مدفوع از 1263 کودک مبتلا به اسهال که به بیمارستانهای کودکان استانهای تهران و قزوین مراجعه کردند، جمع آوری شد. با مشاهده لام مستقیم، فرمل اتر و رنگ آمیزی اسیدفاست اصلاح شده، بررسی لازم جهت شناسایی عوامل انگلی صورت گرفت. در نهایت بر روی DNA استخراج شده از ایزوله ها، PCR-RFLP انجام شد.
یافته ها: از مجموع 1263 نمونه مدفوع، 31 ایزوله با استفاده از روشهای مستقیم و رنگ آمیزی های تخصصی کریپتوسپوریدیوم تشخیص داده شدند، که از بین آنها پس از بررسی های مولکولی 25 ایزوله کریپتوسپوریدیوم پارووم، 5 ایزوله کریپتوسپوریدیوم هومینیس و یک ایزوله هردو ژنوتیپ را نشان داد که نتایج در هر سه ژن یکسان بوده است.
نتیجه گیری: هر دو ژنوتیپ انسانی و گاوی در کودکان مبتلا به اسهال دیده می شود ولی با توجه به این که گونه غالب، کریپتوسپوریدیوم پارووم می باشد، انتقال زئونوز شایعتر از انتقال انسانی بوده و تماس با دام بخصوص گوساله به عنوان مهمترین منبع آلودگی انسانی به شمار می رود.
Background & Aim: Since Cryptosporidium is a worldwide distributed protozoan parasite and is considered as one of the most common causes of infection and diarrhea in humans with autoimmune deficiency، as well as in young live stock، molecular epidemiologic studies of cryptosporidiasis will be helpful for underlying transmission and molecular pathogenesis of Cryptosporidium in humans. The aim of the present study was to determine the species and genotypes of Cryptosporidium among children with diarrhea in Tehran and Qazvin provinces by PCR-RFLP using the three polymorphic regions of SSU-rRNA، COWP and TRAP-C2 genes.
Materials & Methods: 1263 stool samples were collected from the children less than 12 years with diarrhea who referred to Pediatrics Medical Centers in Gazvin and Tehran Provinces، Iran، during 2005-2007. After determination of the presence of Cryptosporidium oocytes by Ziehl-Neelsen acid، fast staining genomic DNA was extracted. Nested PCR-RFLP was performed by -rRNA، COWP and TRAP-C2 genes. Results: Results of microscopically positive samples showed that the overall prevalence of infection in children was 31 (2.5 %).
Results of nested PCR amplification showed that of 31 isolates of children، all of three targeted gene were successfully amplified.
Conclusion: Our results indicated that the zoonotic transmission is the main mode of infection in Iran and indicates that direct or indirect contact with animals، especially calf، is possibly the main route of human infection
