Abstract:
اولین عامل مرگ و میر در کشورهای صنعتی و غیر صنعتی، بیماریهای قلبی-عروقی می باشد که از طریق ایجاد آترواسکلروز و پی آمد بالینی آن و بیماری شریانهای کرونر، نقش خود را ایفا می کند. در بروز آترواسکلروز، لیپوپروتئین های مختلف سرم دخالت دارند. از سال 1963 که Berg لیپوپروتئین (a) را کشف کرد، به تدریج و طی سالها اهمیت نقش آن مشخص گردید، به گونه ای که در سالهای اخیر آن را به عنوان یک عامل خطر مستقل برای ایجاد بیمارهای عروق کرونر نام می برند. آپو (a) یک ملکول درشت شبیه LDL است که ملکول آپولیپوپروتئین B بصورت کوالان به آن متصل گشته است. در بررسی حاضر میزان لیپوپروتئین (a) علاوه بر سایر لیپوپروتئین های سرم (HDL و LDL و VLDL و TG)، در 60 تن از کسانی که به علت بیماری ایسکمی قلب برای آنژیوگرافی به بیمارستان شهید مدرس مراجعه کرده بودند اندازه گیری شد. بیماران دو دسته شدند: دسته اول، گروه شاهد، متشکل از 6 زن و 19 مرد با میانگین سنی 2.3±52.3 سال بودند که در بررسی های آنژیوگرافی شریان کرونر طبیعی داشتند و دسته دوم یا گروه مورد 35 نفر بودند (8 زن و 27 مرد) با میانگین سنی 2±51.2 سال که تنگی عروق کرونر (بیش از 75 درصد) آنها با آنژیوگرافی تایید شد. میانگین سرمی لیپوپروتئین (a) در افراد گروه شاهد 1.8±11.7 و در گروه مورد 5.3±46.5 mg/dl به دست آمد. آزمون آماری این تفاوت را معنی دار نشان داد (P<0.0001). در 74 درصد افراد گروه مورد، میزان لیپوپروتئین (a) سرم بالاتر از حد طبیعی بود. در همین گروه، 25.6 درصد از افراد سایر لیپوپروتئین های سرمشان در محدوده طبیعی قرار داشت، یعنی در 25% افراد مورد فقط میزان لیپوپروتئین (a) سرم بالا بود که می تواند بیانگر نقش مستقل این لیپوپروتئین در بروز بیماریهای عروق کرونر باشد.
Cardiovascular diseases are the leading cause of mortality in industrialized and non-industrialized countries alike. These diseases exert their lethal effect by leading to atherosclerosis and its clinical consequence, coronary artery disease (CAD). Various lipoproteins play different roles in the etiology of this condition. Years after Berg discovered apolipoprotein (a) in 1963, the important role of this lipoprotein gradually became so evident that in the past few years it has been recognized as an independent risk factor in the development of CAD. Apo(a) is a large molecule similar to LDL with apolipoprotein(b) covalently bound to it. In this study, apo(a), along with other serum lipoproteins (HDL, LDL, VLDL and TG), were measured in 60 ischemic heart patients referred to Shahid Modarress Hospital for angiography. Patients were divided into two groups. Nineteen males and six females (age mean and standard deviation of 52.3±2.3 years) who had a normal coronary artery angiography were chosen ad controls. The second group consisted of 35 cases (27 males and 8 females; age mean and standard deviation 51.2±2) with coronary vessel stricture (over 75%) confirmed by angiography. Serum apo(a) mean and standard deviation were 11.7±1.8 mg/dl in the controls and 46.5±5.3 mg/dl in the cases. Statistical tests showed this difference to be significant (P<0.0001). In 74% of the cases, apo(a) levels were far above normal. In this very same group, 25.6% had normal levels of other serum lipoproteins. In other words, in ¼ of the cases apo(a) was the only lipoprotein above normal. We may therefore conclude that this lipoprotein has an independent role in the development of CAD.
