Abstract:
با توجه به شیوع برداشتن لوزه و روماتیسم قلبی در کودکان و به منظور تعیین رابطه لوزه برداری با بروز علایم بالینی و پاراکلینیکی در مبتلایان به تب روماتیسمی این تحقیق در مراجعین به بیماستارنهای شهدا، مفید و امام حسین (ع) طی سالهای 74-1372 انجام گرفت. تحقیق به روش Cohort بروی 100 نفر از مبتلایان به تب روماتیسمی انجام گرفت که 11 نفر آنها دارای سابقه لوزه برداری (گروه مورد) و 89 نفر بدون سابقه لوزه برداری (گروه شاهد) بودند و از لحاظ سن، جنس، محل و زمان مراجعه به صورت مشابه انتخاب شده بودند. علایم بالینی و پاراکلینیکی با شاخصهای خاص خود مورد بررسی قرار گرفتند و ثبت گردیدند. یافته ها با آزمون دقیق فیشر مورد قضاوت آماری قرار گرفت. گروههای شاهد و مورد با هم مشابه بوده، علایم بالینی در دو گروه اختلاف ناچیزی داشتند ولی تنگی نفس در گروه مورد حدود 9 برابر گروه شاهد بود (P<0.002). شاخص های پاراکلینیکی دو گروه با هم اختلاف بارزی داشتند به گونه ای که میزان آنتی استرپتولیزین طبیعی در گروه مورد به طور تقریبی نصف گروه شاهد بود و سرعت سدیمانتاسیون اریتروسیت های بالای 90 در 57.3% از بیماران گروه شاهد و 72.7% گروه مورد دیده شد. میزان پروتئین فعال C در 34.8% از بیماران گروه شاهد و 54.5% از بیماران گروه مورد بالای +3 بود، کشت حلق مثبت از نظر استرپتوکوک ? همولیتیک گروه A در بیماران گروه مورد تقریبا دو برابر گروه شاهد بود (NS). با توجه به عدم گزارش و یا حداقل عدم دسترسی به گزارش مشابه و با توجه به اختلاف در بروز شاخص های پاراکلینیکی توصیه می گردد که تحقیق با تعداد نمونه بیشتر بخصوص در گروه مورد انجام گیرد.
In view of the prevalence of tonsillectomy and carditis due to rheumatic fever (RF) in children, this study was conducted in patients referred to Shohada, Mofid and Imam Hossein hospitals in 1993-95 to determine the possible relationship between tonsillectomy and the clinical and paraclinical manifestations of patients with RHn this cohort study, 11 cases out of 100 RF patients had a history of tonsillectomy. The remaining 89 patients were studied as controls. Patients of the two groups were matched based on age, sex, hospital and time of admission. Clinical signs including polyarthritis, carditis and Sydenham’s chorea, and paraclinical findings such as CRP, ESR, ASO and throat culture for betahemolytic streptococcus group A were recorded. Results were analyzed using Fisher’s exact test. Clinical signs differed slightly in the two groups, except for dyspnea, which was 9 times more frequent in the cases than in the controls (P<0.002). Paraclinical findings, on the other hand, showed remarkable differences in the two groups. In the cases, ASO titer was nearly half this value in controls. ESR was above 90 in 57.3% of the controls, while 72.7% of the cases had an ESR above 90. CRP was over 3+ in 74.8% of the controls and 54.5% of the cases. In the cases, positive cultures for beta-hemolytic streptococcus group A were two times more frequent than in the controls (N.S.). Considering the differences in these paraclinical values and the apparent lack of similar reports, we suggest more extensive studies be performed on a greater number of cases.
