Abstract:
کونژکتیویت نوزادی، عفونت ملتحمه در یک اول ماه زندگی است. شیوع آن از 1.6 درصد در آمریکا تا 23 درصد متغیر می باشد. اخیرا استفاده از محلول بتادین 2.5 درصد به عنوان پروفیلاکسی از کونژکتیویت نوزادی مطرح شده که یک داروی ضد میکروبی گسترده طیف، ارزان، بدون مقاومت باکتریایی و با سمیت پایین است. در این تحقیق از داروی فوق برای پیشگیری از کونژکتیویت نوزادان در بیمارستان مهدیه استفاده گردید. این تحقیق به روش تجربی و یک سوکور بر روی 200 نوزاد متولد شده به طریقه واژینال که مادران آنها سابقه کیسه آب پاره طولانی تر از 18 ساعت نداشتند، انجام گرفت. خصوصیاتی چون سن، تعداد زایمانها، تحصیلات و وضعیت بهداشتی مادر (براساس تعداد دفعات حمام در هفته، وجود خارش و ترشح واژینال، دفعات تعویض لباس زیر در هفته و وجود مراقبت پره ناتال)، جنس نوزاد، ترشح چرکی و هیپرامی در 24 ساعت اول تولد، نتیجه کشت و اسمیر ترشحات چشم در فرم اطلاعاتی ثبت گردید. نوزادان به طور تصادفی به 2 گروه تجربی و شاهد تقسیم شدند و 20 دقیقه پس از زایمان و پس از تمیز کردن چشم، گروه تجربی قطره بتادین 2.5 درصد را دریافت کردند و گروه شاهد هیچ دارویی نگرفتند. در مرحله دوم که 24 ساعت بعد از تولد بود، از همه نوزادان دیدار به عمل آمد و در صورت وجود ترشح چرکی ملتحمه و هیپرامی ملتحمه جهت کشت و اسمیر نمونه گیری گردید. نتایج حاصل با استفاده از Student T-test و آزمون X² مورد تجزیه و تحلیل آماری قرار گرفت. در کل از 200 نوزاد، 49 درصد دختر و 51 درصد پسر بودند. ترشح چرکی ملتحمه در گروه تجربی 5 درصد و در گروه شاهد 30 درصد مشاهده شد. در نوزادان با ترشح چرکی ملتحمه کشت مثبت در گروه تجربی 80 درصد و در گروه شاهد 63 درصد بود. هیپرامی در گروه تجربی 1 درصد و در گروه شاهد 13 درصد مشاهده گردید. در بررسی آماری دیده شد که سن، تعداد زایمان، تحصیلات، وضعیت بهداشتی مادر و جنس نوزاد در گروه شاهد و تجربی تفاوت معنی داری ندارند در حالی که همبستگی بین نتیجه کشت، وجود ترشح چرکی از ملتحمه و هیپرامی ملتحمه با عدم مصرف قطره بتادین معنی دار بود (P<0.05). شایعترین میکروارگانیسم رشد کرده استافیلوکوک کواگولاز منفی می باشد. با توجه به یافته های فوق به نظر می رسد که اختلاف در شیوع کونژکتیویت بین گروه تجربی و شاهد مربوط به استفاده از قطره بتادین 2.5 درصد است و بدین ترتیب استفاده از داروی فوق به عنوان پیشگیری کونژکتیویت توصیه می گردد.
Neonatal conjunctivitis is the inflammation of the conjunctiva in the frist month of life. Its prevalence is 1.6% to 23% in the United States. Recently 2.5% Betadine, an inexpensive wide spectrum antimicrobial without bacterial resistance or significant toxicity, has ben used for prophylaxis against neonatal conjunctivitis. In this experimental single-blind study, 200 newborns whose mothers and normal vaginal delivery and a ruptured membrane for less than 18 hours were included. Characteristics such as mother’s age, parity, education and hygiene (based on number of baths per week, vaginal itching and discharge, frequency of undergarment changing and prenatal care), newborn’s sex, purulent discharge and hyperemia in the first 24 hours after birth, and smear and culture results were recorded in the data sheet. Newborns were randomly assigned to the case or control group. Twenty minutes after birth, 2.5% Betadine drops were given to the cases while no drug was given to the controls. In the second stage, 24 hours after birth, all newborns were examined, and if signs of purulent discharge or hyperemia were observed, samples were obtained for smear and culture. Resutls were analyzed using Student’s t-test and chi-square. Of the 200 newborns studied, 49% were female and 51% male. Purulent discharge was observed in 5% of the cases and 30% of the controls. In those with purulent discharge, cultures were positive in 80% of the cases and 63% of the controls. Hyperemia was seen in 1% of the cases and 13% of the controls. Results showed that mother’s age, parity, education and hygiene, and the newborn’s sex were not significantly different in the cases and controls, while correlation of culture results, purulent discharge and hyperemia with not receiving Betadine drops was significant (P<0.05). The most prevalent microorganism was coagulase-negative Staphylococcus. These findings indicate the difference in conjunctivitis prevalence in the cases and controls is due to use of 2.5% Betadine. We therefore recommend its use for prophylaxis against conjunctivitis.
