چکیده:
سابقه و هدف: سندرم روده تحریک پذیر یکی از شایعترین اختلالات گوارشی است. با توجه به اهمیت اتیولوژیک و اهمیت وضعیت روانشناختی که بر بروز بیماری نقش می گذارد و بعضی از حیطه های روانشناختی مثل خشم که در بیماران ایرانی مبتلا بررسی نشده است، این بررسی به منظور تعیین رابطه مشکلات روانشناختی در بیماران مبتلا به سندرم روده تحریک پذیر و مقایسه آنها با گروه شاهد انجام شد.
مواد و روشها: این تحقیق به روش Case-control انجام گرفت. گروه مورد شامل 45 بیمار مبتلا به سندرم روده تحریک پذیر و گروه شاهد 50 فرد سالم بود. تشخیص سندرم روده تحریک پذیر توسط فوق تخصص گوارش تایید شده و بیماران تحت درمان بودند. مشکلات اضطراب، افسردگی، استرس و خشم به وسیله آزمون DASS و STAXI 2 اندازه گیری شد و با آزمونهای آماری مورد قضاوت قرار گرفت.
یافته ها: افراد دو گروه به لحاظ سن و جنس مشابه بودند. اختلال اضطراب منتشر در گروه شاهد (9/7%) و در گروه مورد (40%)، اختلال وسواسی جبری در گروه شاهد (5/2%) و در گروه مورد (18%)، اختلال پانیک در گروه شاهد (2%) و در گروه مورد (13%)، و افسردگی در گروه شاهد (12%) و در گروه مورد (24%) به دست آمد (05/0 P <).
نتیجه گیری: به نظر می رسد که مشکلات روانشناختی در بروز سندرم روده تحریک پذیر نقش دارد. بررسی تاثیر درمان اختلالات روانشناختی بر وضعیت بیماران سندرم روده تحریک پذیر را توصیه می نماییم.
Background and Aim: Irritable bowel syndrome (IBS) is one of the most common gastrointestinal disorders. Psychological factors are major factors involved in this disease. Some psychological areas such as anger have not been investigated in Iranian patients. Therefore this study was conducted to determine the correlation between psychological problems and IBS، compared to controls.
Materials and Methods: Participants in this case-control study included 45 patients with IBS and 50 healthy controls. Clinical diagnosis and treatment of IBS was carried out by a gastroenterologist. Anxiety، depression، stress and anger were evaluated by DASS 21 and Staxi2 scales and the results were analyzed with statistical tests.
Results: These two groups were similar in terms of age and gender. Generalized anxiety disorder was seen in 40% of IBS patients compared with 7.9% of the healthy group، obsessive compulsive disorder was in 18% of IBS patients and in 2.5% of the healthy group، panic disorder was seen in 13% of IBS patients and in 2% of the healthy group، depression was seen in 24% of IBS patients and 12% of the healthy group (P < 0.05).
Conclusion: It seems that psychological problems are involved in the development of IBS. Study on the effect of psychological treatment in IBS is suggested.
