چکیده:
سابقه و هدف: سرطان پروستات یکی از شایعترین سرطان ها در مردان بوده و روش های تشخیصی گوناگونی برای آن پیشنهاد شده است. هدف از این مطالعه، مقایسهی یافته های این روش های تشخیصی مختلف از جمله بیوپسی با گاید سونوگرافی، نتایج پاتولوژی، سطح سرمی PSA کل و آزاد، و یافته های سونوگرافی ترانس رکتال در بیماران مشکوک به سرطان پروستات می باشد.
مواد و روش ها: در این مطالعه ی مقطعی، کلیهی اطلاعات موجود در پرونده ی بیمارانی که به علت بالا بودن سطح سرمی PSA، تحت بررسی با روش اندوسونوی رکتال و بیوپسی قرار گرفته بودند، بررسی شده و سطح PSA، PSA آزاد، تشخیص پاتولوژی و نتایج سونوگرافی در نمونه های آنها جمع آوری و بین دو گروه سرطانی و بدون سرطان، مورد مقایسه قرار گرفتند.
یافته ها: بر اساس یافته های پاتولوژی، از 220 پرونده، 25% مبتلا به سرطان پروستات بودند. سن بیماران مبتلا به سرطان پروستات 6/81/69 سال بود. سطح PSA سـرمی در بیـماران مبتلا بـه سـرطان پـروستات، بـه طور معناداری بیشتر از سایر بیماران بود (ng/dl 5/195/17 در مقابل ng/dl 1/85/10) (003/0P=). در نقطه ی برش ng/dl 4، حساسیت PSA در تشخیص کانسر پروستات 92 درصد و اختصاصیت آن 2 درصد بود. با در نظر گرفتن نقطه ی برش 10 درصد برای نسبت PSA آزاد به PSA کل، حساسیت این نسبت در تشخیص سرطان پروستات، 17 درصد و اختصاصیت آن 82 درصد بود. همچنین حساسیت سونوگرافی با محوریت ضایعهی هیپواکو در تشخیص سرطان پروستات 34 درصد و اختصاصیت آن 93 درصد محاسبه شد.
نتیجه گیری: اندازه گیری PSA کل و آزاد و همچنین محاسبه ی نسبت این دو، در کنار سونوگرافی ترانس رکتال، از روش های تشخیصی مطلوب برای کانسر پروستات می باشند.
Background and Aim: We conducted this study to evaluate and compare the sensitivity and specificity of various para-clinic parameters in detecting prostate cancer.
Materials and Methods: In this cross-sectional study، 220 patients who underwent prostate biopsies for either high serum PSA level or abnormal digital rectal examination. Sensitivity and specificity of serum PSA، free to total PSA ratio and transrectal ultrasonography findings were calculated.
Results: Based on the pathological findings in 220 cases، 25% were diagnosed with prostate cancer. The mean age of patients with prostate cancer was 69.118.6 years. Mean serum PSA level among prostate cancer patients was significantly higher than in other patients (19.517.5 ng/dl vs. 10.58.1 ng/dl) (P=0.003). The sensitivity and specificity of PSA for detecting prostate cancer، considering cut-off value of 4 ng/dl، was 92% and 2%، respectively. Free to total PSA ratio with cut-off value of 10% revealed 82% sensitivity and 17% specificity. Moreover، hypoechoic lesions detected by transrectal ultrasound had a sensitivity and specificity of 34% and 93%، respectively.
Conclusion: combination of different PSA related parameters with tranrectal ultrasound findings might increase sensitivity and specificity in detecting prostate cancer and reduce unnecessary biopsies.
