چکیده:
مقدمه: استقلال در انجام فعالیت های معمول و پیچیده روزمره زندگی نقش مهمی در فرد مبتلا به معلولیت در بازگشت به زندگی نرمال دارد. هدف از انجام این مطالعه تعیین سطح فعالیت های پیچیده روزمره زندگی (Instrumental Activity of Daily Living( IADL در جانبازان با قطع هر دواندام تحتانی بود.
مواد و روش ها: 335 نفر از 578 نفر از افرادی که در طول جنگ عراق علیه ایران دچار قطع هر دو اندام تحتانی شده بودند در این مطالعه شرکت کردند. جهت تعیین وابستگی جانبازان در فعالیت های پیچیده روزمره زندگی از پرسش نامه IADL –Lawton and Brody استفاده شد. جهت تعیین ارتباط معنی داری بین متغیر ها و سطح IADL از آزمون کای اسکوئر استفاده شد. یافته ها: میانگین سنی افراد مورد مطالعه 6.33±42.05 بود. اکثر جانبازان (97.6%) 327 مرد و متاهل بودند. در بررسی سطح فعالیت های پیچیده زندگی(IADL) بیشترین نیاز به کمک جانبازان دوبل اندام تحتانی مربوط به انجام کار در منزل 306 نفر (91.3%) و تهیه کردن غذا 253 نفر (75.6%) و کمترین نیاز به کمک مربوط به استفاده از تلفن 14نفر (4.2%) بود. سن، سطح تحصیلات، وضعیت اشتغال، آسیب های همراه، درد فانتوم، شدت درد و مدت درد فانتوم جانبازان ارتباط معنی داری (P≤0.05) با سطح فعالیت های پیچیده روزمره زندگی داشتند.
بحث و نتیجه گیری: جانبازان با قطع هر دو اندام تحتانی در انجام برخی از فعالیت های مربوط به IADL مستقل و در برخی نیازمند کمک بودند. برنامه های توانبخشی می تواند به ارتقاء و استقلال عملکرد جانبازان قطع هر دو اندام تحتانی در انجام فعالیت های پیچیده روزمره زندگی کمک کند.
Objective(s): Independency in life activities has an important role in individuals with disability to bring them back to normal life. The objective of this study was to determine Instrumental Activity of daily Living in survivors of the Iran-Iraq war with bilateral lower limb amputation. Methods: A cross-sectional study was performed to assess the level of Instrumental Activity of daily Living (IADL) - Lawton and Brody IADL scale questionnaire- in Iran-Iraq war related bilateral lower limb amputees (n=335). The Chi-square test was employed as statistical methods. Data were analyzed using SPSS software. Results: The average age of the patients was 42.05 (SD=6.33).The majority of the cases %97.6 (n=327) were male and married. The most dependency in IADL were housekeeping (91.3%)، food preparation. And the least was ability to use telephone (4.2 %). Age، level of education، employment status، co morbidity، phantom pain and its severity and duration was significantly (P≤0.05) associated with Instrumental Activity of daily Living scale. Conclusion: This study showed that war related bilateral lower limb amputees were dependent in some IADL scales، especially housekeeping and food preparation. Rehabilitation programs might help the amputees to be more independent in their normal life activity.
