چکیده:
مقدمه : اضطراب و افســردگی از مشکالت شایع بیماران سندرم کرونری حاد به هنگام بســتری در بخش مراقبت های ویژه قلبی است. با توجه به اینکه معمولا در این بخش ها محدودیت ملاقات اعمال می شــود این مطالعه با هدف تعییــن تاثیر ملاقات برنامه ریزی شده بر اضطراب و افسردگی این بیماران انجام شده است.
روش : این پژوهش از نوع نیمه تجربی است که برروی 72 بیمار بســتری در بخش مراقبت ویژه قلبی دربیمارستان بقیه الله(عج) انجام گرفت. نمونه ها با استفاده از روش نمونه گیری در دسترس مبتنــی بر هدف به صورت تصادفی در دو گروه آزمون و شــاهد تقسیم شدند. برنامه ملاقات برنامه ریزی شده عبارت بود از اجرای ســه نوبت ملاقات در روز در گروه آزمــون، در مقابل یک نوبت ملاقات روتین برای گروه شاهد. اطلاعات با استفاده از پرسش نامه اضطراب و افســردگی بیمارســتانی (HADS) روز اول و سوم بستری جمع آوری گردید.
نتایج: میانگین نمره اضطراب در گروه آزمون به طور معنا داری
در روز سوم بستری نسبت به روز اول کاهش یافت (P =0/021)
در گروه شاهد میانگین نمره اضطراب کلی در روز سوم نسبت به روز اول تغییر معناداری را نشان نداد(p=0/102).
میانگین نمره افســردگی در گروه آزمون، در روز ســوم بستری
نسبت به روز اول کاهش یافت (P= 0/38 ) ولی در گروه شاهد نمره افسردگی در روز سوم بستری نسبت به روز اول افزایش یافته بود (P=0/402 ). این تغییرات در هردو گروه معنا دار نبود.
بحث: نتایج این تحقیــق بیانگر تاثیر مثبت ملاقات برنامه ریزی
شــده در مقابــل ملاقات محــدود در کاهش ســطح اضطراب و افسردگی بیمارستانی در بیماران قلبی است بنابراین اجرای چنین برنامه های ملاقاتی در این بخشها به منظور دســتیابی به نتایج مراقبتی و بهبودی بهتر و سریعتر این بیماران توصیه میشود.
Introduction: Anxiety and depression are common
disorders among acute coronary syndrome patients while
hospitalizing in the cardiac care unit. In this respect,
that, in such a ward, visiting is usually restricted. This
study has been implemented to investigate the effect of
planned visiting on hospital anxiety and depression in
acute coronary syndrome patients, hospitalized in the
cardiac care unit.
Method: This is a quasi-experimental study on 72 patients
with acute coronary syndrome admitted to the cardiac
care unit of Baqiyatallah Hospital in Tehran. The samples
were selected with purposeful sampling method and
randomly divided into case and control groups. Planned
visiting program includes three times visiting a day in the
experimental group versus a routine visiting in the control
group. The results have been collected through Hospital
Anxiety and Depression Questionnaire in the first and
third days of hospitalization in both groups.
Results: The mean of anxiety score in the case group
significantly decreased in the third day in comparison
with the first day(p=0.021) but in the control group, the
mean of anxiety score in the third day in comparison
with the first day was not significant(p=0.102). The
mean score of depression in the case group decreased
in the third day of hospitalization compared with the
first day(p=0.38) but in the control group, the mean
of depression score increased in the third day of
hospitalization compared with the first day (p=0.402)
These changes were not significant in both groups.
Discussion: The results of this research indicated
the positive effect of planned visiting in comparison
with the restricted visiting on decreasing hospital
anxiety and depression in cardiac patients. Therefore,
the implementation of such visiting programs is
recommended to these patients to achieve the better
and faster care and results.
