چکیده:
هدف: مراقبت از یک فرد مبتلا به ام اس میتواند بر عملکرد آکیوپیشن مراقبان در حوزه های مختلف آکیوپیشن مثل شغل، استراحت و خواب، مشارکتهای اجتماعی تاثیرگذار باشد. آموزش به مراقبان بخشی از فرآیند توانبخشی را شامل میشود. مراقبان اصلی برای ایفای نقش مراقبتی خود نیاز به دانش و اطلاعات دارند. اگرچه آموزشهای چهره به چهره مفیدند اما دارای محدودیتهایی نیز میتوانند باشند. مثلا مراقب برای دریافت اطلاعات و مداخلات باید به یک مرکز خاص مراجعه نماید که خیلی از اوقات این کار وقتگیر بوده و ممکن است آنها را در انجام نقش های دیگر از جمله نقش شغلی دچار مشکل نماید. از آموزشهای نوشتاری در حوزه سلامت، به عنوان مکمل یا تقویت کننده آموزش کلامی استفاده میشود تا اثرگذاری آن را به حداکثر برسانند. این آموزشها در افزایش دانش، خود مدیریتی، و تعهد به درمان نقش مهمی ایفا میکنند. زمانی که متخصصان سلامت و خدمات سلامت به راحتی در دسترس مراقبان نباشند، ابزارهای نوشتاری برای آموزش در حیطه سلامت میتوانند تا حدی جوابگوی سوالات و نیازهای آنها باشند. این روش همچنین میتواند در کاهش هزینه های مربوط به خدمات رسانی سلامت برای مراقبان کمک کننده باشد. بنابراین هدف از مطالعه حاضر، تهیه و روا سازی ظاهر و محتوای کتابچه آموزشی برای بهبود عملکرد آکیوپیشن مراقبان اصلی افراد مبتلا به مالتیپل اسکلروزیس (ام اس) بود.
روش بررسی: مطالعه حاضر از نوع روش شناسی بود که در آن کتابچه آموزشی در سه فاز طراحی و رواسازی شد: (1) استخراج محتوای کتابچه آموزشی از یک مطالعه بسترزاد مرتبط با مراقبان اصلی افراد مبتلا به ام اس و چالشهایی که در امر مراقبتی با انها مواجهه بودند؛ تهیه پیشنویس اولیه کتابچه آموزشی، (2)بررسی روایی ظاهر و محتوای کتابچه توسط 22 متخصص سلامت در حوزه های مرتبط با بیماری ام اس، و (3) بررسی روایی ظاهر و محتوای کتابچه توسط 22 مراقب اصلی افراد مبتلا به ام اس؛ و تهیه نسخه نهایی و روایی شده کتابچه آموزشی. شاخص روایی محتوا در سطح آیتم مساوی یا بیشتر از 78/0 و شاخص روایی محتوا در سطح مقیاس مساوی یا بیشتر از 9/0 ، ملاک روایی کتابچه در فاز دوم و سوم درنظر گرفته شد. همچنین از آزمون دو جمله ای برای بررسی پایایی آماری شاخص روایی محتوای آیتم ها با در نظر گرفتن سطح معناداری 05/0 و در نظر گرفتن نسبت توافق مورد انتظار (نسبت آزمون) 8/0 استفاده شد.
یافتهها: بعد از ارسال و تحویل پیشنویس پرینت شده نسخه اول کتابچه آموزشی به متخصصان و مراقبان مشارکت کننده به عنوان داور، حداقل مقدار I-CVI و مقدار S-CVI برای آیتمهای مقیاس طراحی شده جهت بررسی روایی ظاهر و محتوای کتابچه، در متخصصان مشارکت کننده به ترتیب برابر با 9/0 و 97/0 بود. همچنین حداقل نسبت توافق مشاهده شدهی آزمون دو جمله ای برای آیتم ها در این متخصصان برابر با 9/0 بود که این مقدار، بیشتر از نسبت توافق مورد انتظار (نسبت آزمون) (8/0) میباشد. مقادیر I-CVI، مقدار S-CVI، و نسبت توافق مشاهده شده بین مراقبان مشارکت کننده نیز همگی برابر با یک بود. متخصصان و مراقبان مشارکت کننده، کتابچه آموزشی را مثبت ارزیابی کردند و نظرات تکمیلی آنها در مورد ظاهر و محتوای کتابچه در نسخه نهایی لحاظ گردید.
نتیجه گیری: کتابچه آموزشی ذکر شده دارای روایی ظاهر و محتوای قابل قبولی بود و متخصصان توانبخشی میتوانند از آن در فرآیند مداخلات توانبخشی مراقبان اصلی افراد مبتلا به ام اس استفاده نمایند. توصیه میشود که مطالعات دیگری برای بررسی اثربخشی این کتابچه آموزشی بر کیفیت زندگی مراقبان اصلی افراد مبتلا به ام اس، طراحی و اجرا گردد.
Abstract
Objective: Caring for an individual with multiple sclerosis can affect caregiver's occupational performance in a variety of occupational areas such as work, rest and sleep, and social participation. Caregiver education is part of the rehabilitation process. Primary caregivers need knowledge and information to play their caregiving role. Although face-to-face educations are useful, they can also have limitations. For example, a caregiver should refer to a specific center for information and interventions, which is often time consuming and may make them difficult to perform other roles including occupational roles. Written health education is used as a supplement or enhancer of verbal education to maximize its effectiveness. These trainings play an important role in enhancing knowledge, self-management, and commitment to treatment. When health professionals and health services are not easily accessible to caregivers, written health education materials can be partially responsive to their questions and needs. It can also reduce the costs of providing healthcare services for caregivers. Therefore, the purpose of the present study was to provide and validate the appearance and content of an educational booklet to improve the occupational performance of primary caregivers of individuals with multiple sclerosis (PCIMS).
Materials & methods: The present study was a methodological one in which the educational booklet was designed and validated in three phases: (1) extracting the educational booklet content from a grounded theory study of PCIMS and the challenges they faced in caregiving; Initial drafting of the booklet, (2) Validation of the appearance and content of the booklet by 22 expert judges who were health specialist in the field of MS, and (3) Validation of the appearance and content of the booklet by 22 PCIMS judges; and the development of the finalized and validated version of the booklet. Item-level content validity index (I-CVI) equal to or greater than 0.78 and scale-level content validity index (S-CVI) equal to or greater than 0.9 were considered as validity criteria of the booklet in the second and third phases. The binomial test was also used to test the statistical reliability of the I-CVIs with a significance level of 0.05 and the expected agreement proportion (test proportion) was 0.8.
Results: After submitting and delivering the first edition of the printed booklet to the participating specialists and caregivers as the judges, the minimum I-CVI and S-CVI values for the scale items designed to assess the validity and appearance of the booklet in the participating specialists were 0.9 and 0.97 respectively. Also, the minimum observed proportion of agreement for items in binomial test was 0.9 which is higher than the expected agreement proportion (test proportion) (0.8). I-CVIs values, S-CVI value, and the observed proportion of agreement between participating caregivers were all equal to one. Participating experts and caregivers evaluated the booklet positively and their comments on the appearance and content of the booklet were included in the final version.
Conclusion: The mentioned educational booklet had acceptable appearance and content validity, and rehabilitation professionals can use it in the process of rehabilitation interventions for PCIMS. Further studies are recommended to investigate the effectiveness of this booklet on improving the quality of life of PCIMS.
