چکیده:
اختلالات خوردن از بیماریهای روانی مزمن و رو به رشد است. یکی از انواع فرعی اختلال خوردن پر اشتهای روانی است که به موجب آن، فرد بین رژیم گرفتن افراطی و پرخوری شدید در نوسان است. هدف این پژوهش مقایسۀ اثربخشی طرحوارۀ درمانی و درمان شناختی و رفتاری بر علائم اختلال خوردن در بیماران پر اشتهای روانی با توجه به پیوند والدینی بودهاست. روش کار و مواد این پژوهش بدین شرحند: روش پژوهش آزمایشی با طرح پیشآزمون -پسآزمون و پیگیری با گروه کنترل است. جامعۀ آماری شامل بیماران دارای اختلال خوردن مراجعه کننده به مراکز مشاوره و کلینیکهای رواندرمانی درمانی شهر تهران است که در ردۀ سنی 16 الی 23 سال قرار دارند. تعداد بیماران دارای پروندۀ پزشکی 72 نفر بودهاست. بعد از غربالگری، تعداد 39 بیمار با توجه به تشخیص روانپزشک و ملاکهای تشخیصی و آماری اختلالات روانی به روش در دسترس انتخاب شدند. بیماران در دو گروه آزمایش و یک گروه کنترل با توجّه به سن و تعداد رفتار جبرانی نامناسب در هفته همتاسازی شدند. ابزار گردآوری دادهها پرسشنامه (1- پیوند والدینی 2- سیاهۀ اختلال خوردن و 3- طرحوارههای یانگ) و مصاحبه تشخیصی بر اساس ملاکهای تشخیص اختلال خوردن و تشخیص روان پزشک بود. یافتههای این پژوهش از این قرارند: طرحوارۀ درمانی نسبت به درمان شناختی رفتاری در کاهش علائم اختلال خوردن در بیماران دارای پیوند بیش حمایتگر نسبت به مراقبت کم در مؤلّفه نارضایتی از بدن دارای اثربخشی و ماندگاری بیشتری بودهاست. (05/0> 001/0:p ; 430/0=f). نتیجۀ این پژوهش این است که بیمارانی با پیوند والدینی بیش حمایتگر به طرحوارۀ درمانی بهتر پاسخ داده-اند، از این رو به درمانگران توصیه میشود که رویکرد طرحواره را با رویکرد های سیستمی و روابط شیء به صورت تلفیقی به کار گیرند.
Eating disorders are among growing chronic mental illnesses around the world. Bulimia nervosa is a subtype of eating disorder, whereby a person fluctuates between extreme diet and overeating. The study made an attempt to compare the effectiveness of schema therapy and cognitive-behavioral therapy on eating disorder symptoms in patients with bulimia nervosa considering parental bonding. The study employed an experimental design using the pretest-posttest and follow-up design (control group). The study population included all patients aged 16 to 23 years with eating disorders referred to psychiatric, psychotherapy, and clinical treatment centers in Tehran. A total of 72 patients had medical records. After screening, 39 patients were selected according to the psychiatristchr('39')s diagnosis and DSM-5 using a systematic method. Patients were matched in two experimental groups and one control group according to age and inappropriate compensatory behavior per week. Data were collected using questionnaires (1- Parental Bonding 2- Eating Disorder Inventory and 3- Young Scheme Questionnaire) and diagnostic interviews based on diagnostic criteria for eating disorders and psychiatric diagnosis. Compared to CBT, schema therapy was more effective in reducing symptoms of eating disorder and more durable in patients with more supportive parental bonding than less care for the body or body dissatisfaction (f=.430, p=.001 <.05). According to the findings of the study, patients with more supportive parental bonding responded better to schema therapy. Therefore, it is advisable for therapists to apply a combination of the schema approach with other system approaches.
