چکیده:
هدف: فلج مغزی نوع آتاکسیک 5 تا 10 درصد از جمعیت افراد با فلج مغزی را تشکیل میدهد و با نواقص تعادلی و هماهنگی مشخص میشود. ترمور غیرارادی، دست و پا چلفتی به نظر رسیدن و نواقص موجود در واکنشهای تعادلی از دیگر ویژگیهای این کودکان است. بنابراین آموزش مهارتهای تعادلی از بخشهای مهم برنامه توانبخشی این افراد محسوب میشود. صفحه تعادلی وی (WBB) یکی از ابزاری است که در جهت آموزش تعادل در کودکان با مشکلات فیزیکی متفاوت از جمله فلج مغزی مورد استفاده قرار گرفته است و کارآمدی آن در مطالعات مختلف به اثبات رسیده است. هدف از مطالعه مقدماتی حاضر، بررسی تاثیر استفاده از WBB بر تعادل عملکردی کودکان با فلج مغزی نوع آتاکسیک و همچنین بررسی میزان ثبات نتایج طی دو ماه پس از اتمام مداخلات آموزش تعادل بود.
روش بررسی: مطالعه مقدماتی حاضر از نوع پژوهش موردی بوده و در سه فاز پایه، مداخله و پیگیری اجرا شده است. 3 کودک (2 دختر و 1 پسر) با فلج مغزی نوع آتاکسیک، سطح یک یا دو سیستم طبقهبندی عملکرد حرکتی درشت (GMFCS) و میانگین سنی 09/1±56/10 سال در این مطالعه شرکت کردند. کاردرمانی رایج در تمامی فازهای مطالعه، سه مرتبه در هفته اجرا شد. مدتزمان جلسات کاردرمانی رایج در فاز پایه و پیگیری 45 دقیقه بود، در صورتی که طی فاز مداخله، 25 دقیقه از جلسه درمانی به کاردرمانی رایج و 20 دقیقه از آن به آموزش تعادل به وسیله WBB اختصاص مییافت (12 ساعت). نتایج حاصل به وسیله مقیاس تعادلی کودکان (PBS) و آزمون برخاستن و رفتن زماندار (TUG)، سه مرتبه در فاز پایه (قبل از شروع مداخله)، سه مرتبه در فاز مداخله (انتهای جلسه 12، انتهای جلسه 24 و انتهای جلسه 36) و دو مرتبه در فاز پیگیری (یک ماه پس از اتمام مداخله و دو ماه پس از اتمام مداخله) توسط یک کارشناس کاردرمانی ناآگاه از روند مطالعه مورد سنجش قرار گرفت و به کمک تحلیل دیداری و شاخصهای آماری PND، Hedeges’g و 2SD مورد تحلیل آماری قرار گرفت.
یافتهها: مقایسه یافتههای حاصل از مقیاس PBS و آزمون TUG در سه فاز مطالعه حاضر نشان داد تعادل عملکردی هر سه شرکتکننده طی روند مداخله بهبود یافته و نتایج نیز در دوره پیگیری پایدار مانده است. PND در هر دو تست برای هر سه شرکتکننده 100 درصد به دست آمد که نشان میدهد آموزش تعادل به وسیله WBB سبب بهبود تعادل عملکردی در موقعیت مداخله و پیگیری نسبت به خط پایه شده است. مقدار Hedeges’g برای هر سه شرکتکننده بیشتر از 8/0 به دست آمد که نشانگر تفاوت قابل ملاحظهای بین فاز پایه و پیگیری است. همچنین 2SD نشاندهنده بهبود قابل توجه نمره PBS و کاهش قابل توجه مدتزمان انجام آزمون TUG در هر سه شرکتکننده بود. تحلیل دیداری یافتهها نیز نشان داد نهتنها تعادل عملکردی هر سه شرکتکننده طی فاز مداخله بهبود یافته است، بلکه نتایج حاصل از مداخلات پس از دو ماه از اتمام مداخلات نیز پایدار مانده است.
نتیجهگیری: نتایج مطالعه مقدماتی حاضر نشان میدهد آموزش تعادل به وسیله WBB، میتواند روشی موثر و ماندگار در بهبود تعادل عملکردی کودکان با فلج مغزی نوع آتاکسیک باشد. هرچند برای بیان نتایج قطعی، انجام مطالعات بیشتر با حجم نمونه مناسب در زمینه بررسی تاثیرات آموزش تعادل به وسیله WBB بر کودکان با فلج مغزی نوع آتاکسیک نیاز است.
Abstract
Objective: Ataxic cerebral palsy has an incidence of 5- 10 percent among the general population of children with cerebral palsy. It is characterized by poor balance and coordination, in addition to clumsiness, involuntary tremor and poorly developed equilibrium responses. As a result, balance training is one of the most important parts of the rehabilitation program in children with Ataxic cerebral palsy. Wii Balance Board (WBB) is an efficient system for balance training in children with different neurological conditions especially Cerebral palsy. The efficacy of this system has been demonstrated in many studies. This study aimed to investigate the influence of WBB on the functional balance and the persistence of the results two months after the end of the balance training intervention in children with Ataxic CP.
Material and Methods: This research was organized in a single subject study, A-B-A design for 3 children (2 females and 1 male) with ataxic CP, mean±SD age:10.56±1.09 years and level I and II of gross motor function classification system(GMFCS). All the participants received a conventional occupational therapy program, 3 sessions per week, 45min per session in baseline and follow-up phases, and 25min followed by 20min of WBB therapy in the intervention phase. Thus, in the current study, WBB therapy had been done for 3 months (12 hours). The changes made in this study were evaluated by the Pediatric Balance Scale (PBS) and Timed Up and Go (TUG) test. A pediatric occupational therapist who was masked to the study design carried out the testing. Outcomes were measured twice during the baseline phase before the initiation of the intervention. Then three times during the intervention phase (at the end of the 12th, 24th, and 36th sessions), and as the two follow-up sessions (one month and two months after the intervention ended). Finally, the results were evaluated by PND, 2SD, Hedgeschr('39')s g, and the visual analysis methods.
Results: The results of PBS and TUG indicated improvement of functional balance in all participants from the baseline phase to intervention and follow-up phases. PND between baseline and follow phases for all participants show a significant difference between phases. Therefore, according to PND, all participants showed a significant change in PBS and TUG results. Hedeges’g for all 3 participants was more than 0.8 that indicates significant difference between baseline and follow-up phases. According to the 2SD method, all of the PBS measurements in intervention and follow-up phases were more than the mean+2SD of the baseline phase that shows improvement in PBS. Also, all of the TUG measurements in intervention and follow phases were less than the mean-2SD of the baseline phase that shows improvement in the TUG test. In addition, the visual analysis revealed a significant improvement in functional balance in the intervention phase and stability of these changes in the follow-up phase.
Discussion: The study results suggested that balance training by WBB can be an effective method for functional balance improvement in children with Ataxic CP. For conclusive result, it is necessary to perform studies with a larger sample size to confirm its effectiveness.
