چکیده:
زمینه: تحقیقات نشان داده است پریشانی روانشناختی عامل خطر مهمی در بیماریزایی سوء هاضمه کارکردی است که در ارتباط با فرآیندهای روانشناختی حوزه درد و محرکهای احشایی کیفیت زندگی بیماران را شدیداً تحت تأثیر قرار میدهد. با این وجود پژوهش راجع به تدوین مدل ساختاری برای پیشبینی کیفیت زندگی بر اساس پریشانی روانشناختی با میانجیگری خودکارآمد پنداری درد، حساسیت احشایی، تصاویر ذهنی مرتبط با درد و فاجعهآفرینی درد مغفول مانده است.
هدف: هدف از پژوهش حاضر بررسی مدل معادلات ساختاری پریشانی روانشناختی با کیفیت زندگی با نقش میانجی خودکارآمد پنداری درد، حساسیت احشایی، تصاویر ذهنی مرتبط با درد و فاجعهآفرینی درد بود.
روش: روش پژوهش همبستگی از نوع مدل معادلات ساختاری است. جامعه آماری را کلیه افراد مبتلا به سوء هاضمه کارکردی مراجعه کننده به کلینیکهای گوارشی شهر اصفهان تشکیل میدادند. نمونه آماری 363 نفر بودند که به روش نمونهگیری در دسترس انتخاب شدند. برای گردآوری دادهها از پرسشنامههای شاخص سوء هاضمه نپین (تالی و همکاران، 1999)، افسردگی، اضطراب و تنیدگی (صاحبی و همکاران، 1384)، خودکارآمد پنداری درد مزمن (بشارت، 1387)، تصاویر ذهنی درد مزمن (کچویی و همکاران، 1394)، شاخص حساسیت احشایی (پورتوکلی و همکاران، 1397) و فاجعهآفرینی درد (سجادیان و همکاران، 1391) استفاده شد. دادهها با روش مدلسازی معادلات ساختاری و با استفاده از نرمافزار smart-PLS تحلیل شد.
یافته ها: بر اساس نتایج بدست آمده مدل پژوهش حاضر از برازش کلی قوی و قابل قبولی برخوردار است (0/857 =GOF). ساختار کلی روابط مورد آزمون از طریق دادههای بدست آمده تأیید گشت (0/907 =NFI و 0/056 =SRMR). تمامیضرایب غیر مستقیم در مدل معنادار است (0/05 P؛ 1/96 t). همچنین همه ضرایب مستقیم به جزء تأثیر مستقیم حساسیت احشایی و خودکارآمد پنداری درد بر کیفیت زندگی معنادار است (0/05 P؛ 1/96 t).
نتیجهگیری: کیفیت زندگی به طور مستقیم و غیر مستقیم از طریق متغیرهای خودکارآمد پنداری درد، حساسیت احشایی، تصاویر ذهنی مرتبط با درد و فاجعه آفرینی درد با پریشانی روانشناختی مرتبط است. یافتههای حاصل از این مدل به درک کاملتری از نحوۀ تأثیر پریشانی روانشناختی بر کیفیت زندگی مبتلایان به سوء هاضمه کارکردی کمک میکند و در نظر گرفتن این روابط در سببشناسی و درمان افراد مبتلا به این بیماری میتواند مفید واقع شود.
Background: The research has shown that the psychological distress is an important risk factor in pathogenesis of the functional dyspepsia. It severely affects the quality of patients’ life regarding psychological processes in the field of pain and visceral stimuli. However, there is a lack of structural models among the literature for predicting the quality of life based on psychological distress considering the effect of mediating roles of pain self-efficacy, visceral sensitivity, pain-related mental imagery, and pain catastrophizing.
Aims: The purpose of this study is investigating the relationship between the psychological distress and the quality of life in functional dyspepsia considering the effect of the mediating roles of pain self-efficacy, visceral sensitivity, pain-related mental imagery, and pain catastrophizing.
Methods: The method used in this research is the model of “correlation of structural equation modeling”. The statistical population of the model consists of 363 patients with functional dyspepsia who referred to gastrointestinal clinics in Isfahan selected using the “convenience sampling method”. Nepean dyspepsia index (Talley et al., 1999), depression, anxiety and stress scale (Sahebi et al., 2005), chronic pain self-efficacy questionnaire (Besharat, 1387), short form -McGill pain questionnaire (Kachooei et al., 1394), visceral sensitivity index (Pourtavakoli et al., 1397) and pain catastrophizing scale (Sajjadian et al., 1391) are used to acquire data. The data are analyzed using the developed structural equation model and the smart-PLS software.
Results: The results obtained by the developed model shows a strong overall fit (GOF= 0.857) and the general structure of the tested relationships is confirmed by the obtained data (NFI= 0.907 and SRMR= 0.056). All indirect coefficients in the model are considered as significant with t≥ 1.96 and p≤ 0.05. Also, all direct coefficients are significant excepts the direct effect between the quality of life and both the visceral sensitivity and the pain self-efficacy with t≥ 1.96 and p≤ 0.05.
Conclusion: The results of the present research demonstrate that the quality of life has significant relations to the psychological distress through pain self-efficacy, visceral sensitivity, pain-related mental imagery, and pain catastrophizing. The findings of this model help to understand the effect of psychological distress on the quality of life of patients with functional dyspepsia and considering these relationships can be of great importance in the etiology and treatment of patients.
