چکیده:
زمینه: سندرم روده تحریک پذیر، یک اختلال عملکردی دستگاه گوارش و یکی از شایع ترین علتهای دردهای مزمن شکمی است که بر کیفیت خواب این بیماران تاثیر می گذارد. اما آیا مداخله های پذیرش و تعهد و شناختی رفتاری عدم تحمل بلاتکلیفی می تواند بر بهبود تیپ شخصیتی D و کیفیت خواب بیماران با سندرم روده تحریک پذیر دارای اختلال اضطراب فراگیر کمک کند؟ هدف: هدف پژوهش بررسی اثربخشی مداخلههای پذیرش و تعهد و شناختی رفتاری عدم تحمل بلاتکلیفی بر تیپ شخصیت D و کیفیت خواب بیماران دارای اختلال اضطراب فراگیر با سندرم روده تحریک پذیر بود. روش: این مطالعه شبهآزمایشی با طرح پیش آزمون، پس آزمون و پیگیری سه ماهه با گروه کنترل بود. جامعه آماری، تمامی بیماران دارای اختلال اضطراب فراگیر مبتلا به سندرم روده تحریکپذیر شهر تهران در سال 1398-99 بود. 60 نفر بیمار به روش نمونهگیری هدفمند انتخاب و به صورت تصادفی در دو گروه آزمایش و یک گروه گواه (20 نفره) قرار گرفتند. گروههای آزمایش 12 جلسه 90 دقیقه ای مداخلات شناختی رفتاری (داگاس و رابیچاوود، 2007) و پذیرش و تعهد (آیفرت و فورثایث، 1396) را دریافت کردند و گروه گواه در این مدت هیچ گونه مداخله ای دریافت نکرد. آزمودنی ها با استفاده از پرسشنامه های کیفیت خواب پیتزبورک (1989) و پرسشنامه تیپ شخصیت D دنولت (2005) در مراحل پیش آزمون، پس آزمون و پیگیری ارزیابی و داده ها با استفاده از آزمون تحلیل واریانس مختلط تحلیل شد. یافته ها: نتایج نشان داد هر دو مداخله اثر معناداری بر بهبود کیفیت خواب بیماران سندرم روده تحریک پذیر و تیپ شخصیت D داشتند (0/01> P) و در طول دوره پیگیری، هر دو مداخله شناختی رفتاری و اکت بر کیفیت خواب و مداخله اکت بر تیپ شخصیتی D دارای پایداری اثر بودند (0/01P). نتیجهگیری: بر اساس نتایج این مطالعه میتوان گفت هر یک از مداخله های شناختی - رفتاری و پذیرش و تعهد به عنوان یک مداخله موثر، میتواند در مراکز درمانی و بهداشتی در جهت بهبود کیفیت خواب و اصلاح ویژگیهای شخصیت D بیماران دارای اختلال اضطراب فراگیر با سندرم روده تحریک پذیر به کار برده شود. همسران شهید و جانباز مطرح باشد و لذا آگاهی و استفاده از این روش جهت بهبود احساس معنای زندگی میتواند کمککننده باشد.
Background: Irritable bowel syndrome (IBS) is a functional gastrointestinal disorder that can be stimulated and intensified by psychological factors such as anxiety and negative imotions and affects the quality of life of these patients. But can help the behavioral cognitive and acceptance and commitment interventions on improve the qulailty of life and emotional schemas in patients with irritable bowel syndrome with generalized anxiety disorder?
Aims: The aim of study was the efficacy of behavioral cognitive and acceptance and commitment interventions on qulailty of life and emotional schemas in patients with irritable bowel syndrome with generalized anxiety disorder.
Methods: This study was a quasi-experimental study with a pretest and posttest design with a control group and follow up for three months. The statistical population included patients with irritable bowel syndrome and generalized anxiety disorder in Tehran. 60 patients were selected through targeted sampling and were assigned randomly into two experimental groups and one control group (n= 20 each). Experimental groups received twelve 90-minute sessions of behavioral cognitive (Dugas & Robichaud, 2007) and acceptance and commitment (Eifert & Forsyth, 1396) interventions, and the control group received no intervention during this period. Participants were assessed by Patrick & Drossman's IBS Specific Quality of Life Questionnaire (1998) and Jiang & et.al emotional schemas questionnaire (2019) in pre-test and post-test. The results were analysed using multivariate analysis of variance.
Results: Both interventions had a significant effect on improve the quality of life and emotional schemas (Pefficacy and both treatments are equally effective.
Conclusion: Each of the behavioral cognitive and acceptance and commitment interventions as an effective intervention can be used in medical centers to improve the quality of life and emotional schemas of patients with irritable bowel syndrome and generalized anxiety disorder.
