چکیده:
اهداف: توانایی گذر ایمن از پله عامل مهمی در استقلال و کیفیت زندگی سالمندان است. هدف از این مطالعه تعیین تاثیر تمرینات ترکیبی بر عملکرد گذر از پله در سالمندان مرد از طریق بررسی پارامترهای مرکز فشار، تعادل، سرعت و میزان خودکارآمدی بود. مواد و روش ها: در این مطالعه 30 مرد سالمند حداقل 60 ساله ساکن اصفهان در سال 1400، دارای استقلال عملکردی، سلامت ذهنی، توانایی انجام فعالیتهای روزمره بدون استفاده از وسیله کمکی و بدون سابقه سقوط انتخاب شدند و به طور تصادفی و مساوی در دو گروه مداخله و کنترل تخصیص یافتند. طرح مطالعه در کمیته تخصصی اخلاق در پژوهش تائید گشت و همه شرکت کنندگان فرم رضایت آگاهانه امضا کردند و حقوق آنها در طول مطالعه محفوظ بود. تمرینات ترکیبی بر بهبود تواناییهای ضروری برای گذر از پله شامل دامنه حرکتی، قدرت، توانی و تعادل متمرکز بود. گروه مداخله تمرینات را به مدت 12 هفته انجام داد. پارامترهای مرکز فشار با استفاده از صفحه نیرو، زمان بالا رفتن و پایین آمدن از پله توسط سیستم تحلیل حرکت، عملکرد گذر از پله با آزمون زمان بالا و پایین رفتن از پله و میزان خودکارآمدی با مقیاس خودکارآمدی گذر از پله ارزیابی گردید. اطلاعات با آزمون تحلیل واریانس ترکیبی در نرم افزار SPSS تجزیه و تحلیل شد (p≤0/05). یافته ها: تمرینات ترکیبی در کاهش پارامترهای مرکز فشار هنگام بالا رفتن و پایین آمدن از پله موثر بود (p≤0/048). زمان مرحله استقرار و زمان بالا رفتن و زمان پایین آمدن از پله پس از اجرای تمرینات کاهش یافت (p≤0/034). نتایج آزمونهای زمان بالا و پایین رفتن از پله (p≤0/002) و خودکارآمدی گذر از پله (p≤0/048) بهبود عملکرد در گروه مداخله نسبت به کنترل را نشان داد. نتیجه گیری: نتایج تاثیر گذاری تمرینات ترکیبی در بهبود عملکرد گذر از پله در سالمندان را نشان داد. با توجه به گستردگی موقعیتهای گذر از پله در فعالیتهای روزمره و پیچیدگی و خطر بالقوه آن برای سالمندان، بکارگیری ترکیبی از تمرینات کششی، مقاومتی، توانی و تعادلی برای بهبود عملگرد گذر از پله در سالمندان توصیه میشود.
Objectives: The ability to safely stair negotiation is an essential factor in older adults' independence and quality of life. This study aimed to investigate the effect of combined training on stair negotiation performance in older men by examining the center of pressure (COP) parameters, balance, speed, and self-efficacy.
Methods & Materials: In this study, 30 older men at least 60 years old living in Isfahan in 1400, with functional independence, mental health, the ability to perform daily activities without using an assistive device, and without a history of falling were selected and assigned randomly and equally in two intervention and control groups. The research design was approved by the Ethics Committee in Biomedical Research and all participants signed the informed consent form and their rights were reserved during the study. Combined training focused on improving the essential abilities for stair negotiation, including a range of motion, strength, power, and balance. Participants of the experimental group completed 12 weeks of combined training. The COP parameters using the force plate, the time of stair ascending and descending by the motion analysis system, and the stair negotiation performance with the Timed Up and Down Stairs (TUDS) and the Stair Self-Efficacy (SSE) tests were evaluated. Data were analyzed by mixed ANOVA in SPSS24 (p˂0.05).
Results: Combined training was effective in reducing the parameters of the COP when stairs ascending and descending (p≤ 0.048). Stance time and time of ascent and descent decreased after performing the training (p≤0.034). The results of Timed Up and Down Stairs (TUDS) (p≤0.002) and Stair Self-Efficacy (SSE) tests (p≤0.048) showed improved performance in the experimental group compared to the control.
Conclusion: The results showed the effectiveness of combined training in improving stair negotiation performance in older adults. Due to the width of use of stairways in daily activities and its complexity and potential risk for older adults, combined training with an emphasis on the stair negotiation task demands including the range of motion, strength, power, and balance is recommended for older men.
