خلاصة:
سابقه و هدف: با توجه به مراجعه مبتلایان به فتق دیسک کمری و نگرانی از پیش آگهی درمان در این بیماران و برای پاسخ به این سوال که آیا نمره بندی JOA قادر به پیش بینی نتایج درمان خواهد بود یا خیر، این تحقیق روی بیماران مراجعه کننده به چند بیمارستان طی سالهای 89-1387 انجام گرفت.
مواد و روشها: این مطالعه به روش Cross sectional و بر روی کلیه بیماران با تشخیص فتق دیسک کمری که اندیکاسیون جراحی داشتند، انجام گرفت. ابتدا وضعیت درد و علائم کلینیکی بیماران بر اساس نمره بندی JOA و Finneson مشخص گردید. سپس بیمارانی که حداقل سه ماه از درمان آنها گذشته بود فراخوان شدند و نتیجه درمان بر اساس اختلاف نمره بندی JOA قبل و بعد از عمل (DJOA) و رضایتمندی بیماران مشخص شد. میزان همبستگی و معادله بین رتبه JOAb (JOA قبل از عمل) با رضایتمندی تعیین شد و قدرت پیش بینی این معادله در یک مطالعه آزمایشی روی 10 نفر بررسی گردید.
یافته ها: طی مدت دو سال، مطالعه روی 117 بیمار واجد شرایط انجام گرفت که 60% آنها مذکر و 40% مونث بودند و میانگین سن آنها 11±45 (از 18 تا 82 سال) بود. متوسط تغییرات JOA قبل و بعد از عمل 2/5±18 بود و بین JOA قبل و بعد از عمل ارتباط معنی داری دیده شد (0001/0> p). میزان همبستگی بین JOA با نتایج عمل برابر 86/0 بود و معادله آنها به صورت 635/23+JOAb036/1-=DJOA بدست آمد. در مطالعه آزمایشی میزان همبستگی برآورد رتبه JOA بعد از عمل با میزان واقعی آن برابر 88/0 بود.
نتیجه گیری: به نظر می رسد رتبه بندی JOA قبل از عمل قادر به پیش بینی نتایج درمان در مبتلایان به فتق دیسک کمری می باشد. انجام یک مطالعه longitudinal در این زمینه توصیه می گردد.
Background and Aim: To study the predictive value of Japanese orthopaedic association (JOA) score in patients diagnosed with Lumbar disc herniation undergoing discectomy in many hospitals during 2008-2011.
Materials and Methods: Patients diagnosed with lumbar disc herniation who were candidate for discectomy entered this cross sectional study. The Finneson and preoperative JOA scores were determined. Patients were recalled at least three months after surgery، and the result of discectomy were recorded based on DJOA (the difference between pre and postoperative JOA score); in addition، patients’ satisfaction were measured. Then the correlation coefficient and the linear relationship between the JOA score before surgery and the DJOA were found. Finally in a pilot study on 10 patients، the linear equation was evaluated.
Results: 117 patients were eligible to enter the study during the two years course of the study. Patients’ mean age was 45±11 (18-82 y) and were followed for at least three months. The difference between pre and postoperative JOA score (18 ± 5.2) was statistically significant (P<0.0001). The correlation coefficient between the preoperative JOA score and DJOA was r=0.82. The linear relationship between preoperative JOA and DJOA was as follows: DJOA= -1.036 × preoperative JOA + 23.635. In a pilot study to estimate discectomy outcome، the correlation coefficient between the actual amount of DJOA and the estimated DJOA was r=0.88.
Conclusion: It seems that the preoperative JOA is able to predict discectomy results in patients with lumbar disc herniation. A Longitudinal study in this area is recommended.
