برقراری ارتباط بین پرستار با بیماران دارای مشکلات ارتباطی سخت است. با وجود این مطالعات کافی در این زمینه در دسترس نیست. این مطالعه به منظور بررسی خود_ارزیابی دانش و عملکرد پرستاران در برقراری ارتباط با بیماران دارای اختلال در ارتباط انجام شد. این مطالعه ی مقطعی در بیمارستان های دانشگاه علوم پزشکی کاشان در سال 1391 انجام شد. تعداد 200 پرستار در مطالعه شرکت کردند و پرسشنامه ی محقق ساخته مربوط به خود_ارزیابی بررسی دانش و عملکرد را تکمیل نمودند. پرسشنامه مشتمل بر چهار بخش و دارای 22 سوال بود که در بردارنده ی خصوصیات دموگرافیک، سوالاتی در مورد دانش پرستاران و خود_ارزیابی دانش و عملکرد در ارتباط با بیماران دارای مشکلات ارتباطی و نیز سوالاتی در زمینه ی روش های رایج برقراری ارتباط، مشکلات رایج در برخورد با این بیماران، و علت مهارت ناکافی پرستاران در برقراری ارتباط با این بیماران بود. تجزیه و تحلیل داده ها با استفاده از نرم افزار SPSS نسخه ی 11.5 و به کمک آمار توصیفی و آزمون های تی و آنالیز واریانس انجام شد. میانگین نمره ی دانش پرستاران در زمینه ی برقراری ارتباط با بیماران دارای مشکل ارتباطی .56±1.04 (از 5) بود. بین میانگین نمره ی دانش پرستاران زن (3.55±1.05) و مرد (3.62±1.03) تفاوت معناداری مشاهده نشد (p=0.7). همچنین، بین میانگین نمره ی دانش پرستاران بخش های داخلی_جراحی (3.55±1.04)، اورژانس (3.48±1.09) و مراقبت ویژه (3.72±0.98) تفاوت معناداری مشاهده نشد (0/69=P). همچنین 79/5% از پرستاران دانش و عملکرد خود در ارتباط با این بیماران را مطلوب نمی دانستند. دانش پرستاران در زمینه ی ارتباط با بیماران دارای مشکلات ارتباطی، در حد متوسط است. آن ها عملکرد خود در این زمینه را نیز مطلوب ارزیابی نمی کنند. پیشنهاد می شود که موضوع ارتباط با بیماران دارای مشکل ارتباطی به پرستاران آموزش داده شود و نیز در برنامه ی درسی دوره ی کارشناسی پرستاری گنجانده شود.
Making a good relationship between nurses and patients with communicational problems is difficult; however, insufficient studies are available in this regard. Hence,this cross-sectional study was conducted to evaluate the nurses' opinions on their knowledge and practice about communicating with patients having communicational problems in Kashan hospitals in 2012.. In total 200 nurses participated in the study and completed a questionnaire to evaluate their knowledge and performance. In addition, the questionnaire had 4 parts and was consisted of 22 questions on demographics, knowledge, and self-rating of knowledge and performance in communicating between the nurses and patients with communicational problems, and questions on common ways of communicating, common problems and reasons for insufficient skills in this regard. Data analysis was done using 11.5th-version SPSS software and descriptive statistics, t-test and ANOVA. The mean score of nurses’ knowledge on communicating with patients was 3.56±1.04 out of 5; moreover, no significant difference was observed between the mean of knowledge in female (3.55±1.05) and male nurses (3.62±1.03) (p=0.7). Subsequently, no significant difference was indicated between the mean of knowledge of nurses in medical-surgical (3.55±1.04), emergency (3.48±1.09) and critical care (3.72±0.98) units (p=0.69); furthermore, 79.5% of nurses mentioned their knowledge and performance on communicating with the patients having communicational problems as undesirable. Nurses’ knowledge on communicating with those patients having communication problems was at the moderate level; they also evaluated their performance in this regard as unfavorable.Therefore, it is recommended that courses on special communicational skills should be established for nurses and also these topics be added to the curriculum of nursing at Bachelor of sciences degree.
