Abstract:
سابقه و هدف: فلج مغزی یک ناهنجاری غیرپیشرونده در مغز است که نقایص حرکتی و وضعیتی در رشد کودک ایجاد می کند و شیوع نوع اسپاستیک آن بیش از سایر موارد است. در توانبخشی از ارتزها برای جلوگیری از عوارض اسپاستیسیتی و بهبود راه رفتن استفاده می شود. با توجه به برخی گزارشات مبنی بر تاثیر ارتز کاهنده تون در کاهش تون کودکان فلج مغزی اسپاستیک، این مطالعه با هدف مقایسه تغییرات تون و الگوی راه رفتن (طول گام، فاصله دو پا و زاویه پا) در دو گروه از کودکان دایپلژی اسپاستیک با و بدون ارتز کاهنده تون انجام شد.
مواد و روش ها: این کارآزمایی بالینی روی 30 کودک دایپلژی اسپاستیک 8-2 ساله در دو گروه تجربی (12 نفر) و شاهد (18 نفر) انجام شد. تون عضلات بر اساس Modified Ashworth Scale و الگوی راه رفتن با ثبت ردپای کودک روی کاغذی که به زمین چسبانده شده بود، قبل و بعد از 12 هفته اندازه گیری شد. در این مدت هر دو گروه سه روز در هفته تحت درمان با NDT بودند. علاوه بر آن گروه تجربی ارتز کاهنده تون را 6 ساعت در روز می پوشیدند.
یافته ها: تون عضلات کل بدن در گروه تجربی کاهش معنی داری داشت (025/0=p). مقایسه تون عضلات پلانتار فلکسور و همسترینگ در گروه تجربی قبل و بعد از مداخله تفاوت معنی داری نشان داد (بترتیب 01/0=p و 006/0=(p، اما در گروه شاهد معنی دار نبود. میانگین طول گام پای راست (03/0=p) و چپ (04/0=p) در گروه تجربی بیشتر از شاهد بود، اما تفاوت تون اندام فوقانی در هر دو گروه شاهد (03/0=p) و تجربی (02/0=p) معنی دار بود.
نتیجه گیری: ارتز کاهنده تون در کودکان دایپلژی اسپاستیک همراه با اقدامات درمانی متداول موجب کاهش تون به خصوص تون پلانتار فلکسورها و همسترینگ ها و افزایش طول گام می شود. بنابراین این ارتز برای کاهش تون توصیه می گردد .
Background and Aim: Cerebral palsy is characterized by non-progressive abnormalities in the developing brain that create motor and postural deficits in the developing child. The most common feature of cerebral palsy is spasticity. In rehabilitation، orthotics have been used to prevent complications of spasticity and improve gait. There are some reports about effectiveness of tone-reducing AFO on decreasing tone in spastic cerebral palsy. The purpose of this study was to compare the effects of tone-reducing AFO on tone and gait between two groups of children with spastic diplegia.
Materials and Methods: This randomized clinical trial was conducted on 30 children with spastic diplegia aged 2-8 years. They were assigned to either a control group (18 subjects) or a case group (12 subjects). All children were tested before and after 12 weeks of neurodevelopmental treatment 3 days a week by Modified Ashworth Scale and recording of the foot-print on a piece of paper fastened to the floor for the measurement of stride length، width and foot angle. Children in the case group wore the tone-reducing AFO approximately 6 hours a day.
Results: A significant difference was found in muscle tone of the whole body between the two groups (p=0.025). Pretest and post-test difference of the plantar flexors and hamstrings in the case group were also significant (p=0.025، p=0.005). A significant difference was found in the right stride length (p=0.02) and the left stride length (p=0.03) between the two groups، but difference in stride width and foot angle were not significant.
Conclusion: Tone-reducing AFO would significantly reduce tone of the whole body، the plantar flexors and hamstrings and increase stride length in spastic diplegic cerebral palsy.
