Abstract:
سابقه و هدف: با توجه به مراجعه مبتلایان به شکستگی مهره های سینه ای و کمری و اهمیت اطلاع از وضعیت آن ها، نوع درمان ، پیش آگهی درمان، کاستی های روش های فعلی رتبه بندی این بیماران، گزارش هایی از موفقیت روش نمره بندی TLICS در این مبتلایان و عدم گزارش تجربه ای از آن در ایران، این تحقیق به منظور تعیین رتبه TLICS و موفقیت این روش روی مراجعین به بیمارستان امام حسین (ع) طی سال های 1389-1387انجام گرفت.
مواد و روشها: تحقیق به روش توصیفی روی کلیه بیماران واجد شرایط انجام شد و وضعیت نورولوژیک بر مبنای درجه بندی فرانکل، نقص عصبی و تقسیم بندی AO/Magrel طبق استاندارد آن ها مشخص شدند. کلیه بیماران مطابق سیستم نمره بندی TLICS در چهار گروه 3-1، 4، 6-5 و 9-7 مشخص و یافته های آن ها بوسیله آزمون دقیق فیشر و ضریب همبستگی مورد قضاوت آماری قرار گرفت.
یافته ها: طی مدت 2 سال، مطالعه روی 67 بیمار واجد شرایط انجام گرفت. سن بیماران 11±30 ( از حداقل 19تا 60 سال) بوده و بیماران به مدت میانگین 18 ماه ( 24- 6 ماه) بعد از عمل جراحی پی گیری شدند. 63 نفر از 67( 94٪) بیمار نمره TLICS بزرگتر یا مساوی 4 داشتند. درجه فرانکل در 60 بیمار بدون تغییر باقی ماند و در 6 بیمار بهتر شد. رنج نمره TLICS بین 2 تا 9 (متوسط 5/6) بود. ارتباط معنی داری بین وضعیت نورولوژیک و نمره TLICS (004/0 P<) دیده شد. بین نوع تقسیم بندی AO و وضعیت نورولوژیک (درجه بندی فرانکل) با نمره TLICS همبستگی بالایی به ترتیب (8/0= R) و (78/0= R) مشاهده گردید.
نتیجه گیری: به نظر می رسد نمره TLICS روش مناسبی در تعیین شدت ترومای شکستگی مهره های سینه ای و کمری و پیش آگهی انتخاب روش جراحی باشد. انجام تحقیقات تجربی بیشتر در این زمینه توصیه می گردد.
Background and Aim: Due to refer patients with thoracolumbar fractures، and the importance of treatment results in these patients، type of treatment، prediction of treatment outcome، the shortcomings of the current scoring In these patients ، reports of success of scoring Thoracolumbar Injury Classification System (TLICS) and lack of experience in Iran، Therefore، this study was conducted to determine the score of TLICS and evaluate the success of this method in patients who referred to Imam Hossein hospital during 2008-2010.
Materials and Methods: Descriptive analysis study was conducted. All patients who had undergone surgery due to the thoracolumbar fractures were studied. Clinical and radiologic data، classifying the trauma according to the Frankel classification، the Magerl/AO classification، and the TLICS score for fractures were evaluated. The results were statistically analyzed by Fisher's exact test and correlation coefficient.
Results: The study was conducted on 67 eligible patients during two years. The average age of the patients was 30 ± 11 years (range 19-60 years)، and the average follow-up time was 18 months (range 6-24 months). 63 (94%) of patients had a TLICS score≥4. The TLICS score ranged from 2 to 9 (average of 6.5). Frankel classification status remained unchanged in 61 of cases and 6 of them had some improvement. A statistic correlation was established between the neurologic status and the TLICS score (P< 0.004). An elevated correlation coefficient were found between the AO type fracture and Frankel classification status with the TLICS score (r=0.8) and (r= 0.78) respectively.
Conclusion: It seems that the TLICS score is a good approach in determining the severity of thoracolumbar fractures and the prediction of surgical management. Our results will need to be confirmed with further clinical trial studies.
