Abstract:
با توجه به روند رو به افزایش پیوند اعضا بویژه کلیه و سرکوب سیستم ایمنی جهت بقای پیوند و به منظور تعیین ملاحظات قبل، حین و بعد از عمل جراحی بی ارتباط با پیوند کلیه، این تحقیق در مراجعین به بیمارستان طالقانی طی سالهای 1376-1368 انجام گرفت. تحقیق حاضر به روش مطالعه داده های موجود صورت پذیرفت و خصوصیات سن، جنس، گروه خونی، تظاهرات بالینی، تشخیص بیماری و پیش آگهی بیماران از پرونده آنها استخراج و گزارش شده است. موارد انطباق تظاهرات بالینی و پاراکلینیکی با یافته های پاتولوژی در اولین عمل و اعمال بعدی مشخص و شیوع این جراحی ها با فاصله اطمینان (CI) با احتمال 95 درصد در بیماران پیوندی برآورد گردید. از 410 نفر بیمار پیوندی، روی 16 نفر (3.9 درصد) (CI=2.1-5.7) بیست و پنج عمل جراحی انجام گرفت که شایعترین جراحی ها آپاندیسیت و آبسه شکمی هر کدام 20 درصد، کله سیستیت حاد 16 درصد، انسداد کاذب 12 درصد و بقیه مربوط به سایر موارد بود. میزان انطباق تظاهرات بالینی و پاراکلینیکی با یافته های پاتولوژی در اولین اعمال حدود 35 درصد و در اعمال بعدی 64 درصد می باشد. آماده سازی قبل از عمل در بیمارانی که مورد پیوند کلیه قرار گرفته اند، ملاحظات خاصی را مطرح می کند که عدم توجه به آن در تشخیص، اداره و پیش آگهی بیماران تاثیر عمده ای دارد.
Considering the increasing number of organ transplants, especially kidney transplants, and the immunosuppression necessary for graft survival, the following study was performed in kidney transplant patients referred to Ayatollah Taleghani Hospital during 1989-1997, to determine the per-, intra- and post-operative features of operations unrelated to their transplant. In this study of existing data, patients’ age, sex, blood group, clinical manifestations, diagnosis and prognosis were extracted from their records. Cases in which the clinical and paraclinical manifestations correlated with pathology findings in the first and subsequent operations were identified, and the frequency of these operations was estimated in transplant patients with a confidence interval (CI) of 95%. In this 410 transplant patients studied, 25 operations were performed in 16 cases (3.9%) (CI=2.1-5.7). The most frequent operations were for appendicitis and abdominal abscess (20% each), acute cholecystitis (16%) and pseudo-obstruction (12%). Clinical and paraclinical manifestations correlated with pathology findings in 35% of the first operations and 64% of the subsequent operations. Preoperative preparation of kidney transplant patients requires special consideration, which may have a profound effect on the diagnosis, management and prognosis of these cases.
