Abstract:
این پژوهش با توجه به شیوع خونریزی گوارشی تحتانی در کودکان و گزارش های ضد و نقیض از علل خونریزی و تاثیر عدم اطلاع از علت آن در پیش آگهی و درمان بیماران به منظور تعیین دلایل خونریزی و نیز به منظور تعیین قدرت کولونوسکوپ در تشخیص علت خونریزی، در مراجعین به مرکز طبی کودکان طی سالهای 75-1374 انجام شد. این تحقیق در تعیین علل با روش توصیفی و در تعیین قدرت کولونوسکوپی با روش کارآزمایی بالینی از نوع تشخیصی بر روی تعداد 103 کودک مبتلا به خونریزی تحتانی گوارشی صورت پذیرفت. سن، جنس، دلایل مراجعه، علت خونریزی از نظر کولونوسکوپی و آسیب شناسی مورد بررسی قرار گرفت. کولونوسکوپی به وسیله فلوی گوارش تحت نظر فوق تخصص گوارش با دستگاه کولونوسکوپ 11.5m.m و PentaxFC 34H پس از آمادگی کودک به روش استاندارد انجام گردید. علل خونریزی درنمونه ها بررسی و فاصله اطمینان آن در جامعه برآورد و قدرت تشخیص کولونوسکوپی تعیین شد. در این مطالعه 44 نفر دختر و 59 نفر پسر در سنین 6.6 سال و 81 درصد به خاطر هماتوشیزیا، 20 درصد به علت اسهال خونی و یک درصد به دلیل ملنا مورد بررسی قرار گرفتند. علل خونریزی در کولونوسکوپی به ترتیب پولیپ (32.7 درصد)، طبیعی (27 درصد)، کولیت (17.3 درصد) و از نظر آسیب شناسی کولیت غیر اختصاصی (35.6 درصد)، پولیپ (26 درصد) و کولیت اولسنر (22.1 درصد) می باشد. حساسیت کولونوسکوپ در تشخیص دلیل خونریزی 73 درصد، ویژگی 67 درصد، ارزش پیشگویی مثبت 99 درصد و منفی آن 7 درصد محاسبه شد. با توجه به زیادی خطای تشخیص منفی کولونوسکوپ توصیه می گردد، از بیمارانی که کولونوسکوپی به عمل می آید، چند نمونه بیوپسی جهت بررسی از نظر آسیب شناسی گرفته شود.
The prevalence of lower gastrointestinal (LGI) bleeding in children, conflicting reports regarding its etiology, and the effect of delayed diagnosis on prognosis and management, led us to conduct the following study to determine the causes of LGI bleeding and the diagnostic power of colonoscopy in patients referred to the Children’s Medical Center during 1995-96. Etiology was determined through a descriptive study, while the diagnostic power of colonoscopy was measured by a diagnostic clinical trial on 103 children with LGI bleeding. Patients’ age, sex, cause of referral, and the etiology determined by colonoscopy and pathology were recorded. Colonoscopy by GI flow was performed under the supervision of a pediatric gastroenterologist with a 11.5mm Pentax FC 34H colonoscope after preparing the child by standard methods. Causes of bleeding were studied in the patients, and the confidence interval for the general population and the diagnostic power of colonoscopy were determined. In this study 44 girls and 59 boys with a mean age of 6.6 years were studied. The cause for referral was hematochezia in 81%, bloody diarrhea in 20%, and melena in 1%. The etiology of bleeding as determined by colonoscopy was, in descending order, polyp (32.7%), normal (27%), and colitis (17.3%). Pathology showed the etiology to be inspecific colitis (35.6%), polyp (26%) and ulcerative colitis (22.1). Colonoscopy had 73% sensitivity, 67% specificity, 99% positive predictive value and 7% negative predictive value in determining the cause of bleeding. Considering the high negative predictive error rate of colonoscopy, obtaining a biopsy in patients undergoing colonoscopy is recommended for pathology studies.
