Abstract:
با توجه به اهمیت و نقش مهم غده تیرویید در بدن و نظر به فراوانی بیماریهای تیرویید و به منظور تعیین اندیکاسیونهای تیروییدکتومی، این پژوهش روی 300 مورد بیمار مراجعه کننده به بیمارستان شهدای تجریش طی سالهای 75-1371 انجام گرفت. تحقیق به روش مطالعه داده های موجود صورت پذیرفت. اطلاعات لازم که شامل سن، جنس، علایم بالینی، نتیجه ایزوتوپ اسکن، تشخیص قبل از عمل جراحی و نتیجه پاتولوژی بعد از عمل بود، از پرونده بیماران استخراج و با آمار توصیفی ارایه و فاصله اطمینان هر یک در جامعه برآورد گردید. تعداد زنان 3.2 برابر مردان بود و بیشترین گروه سنی (33.3%) تحت عمل جراحی در محدوده 40-30 سال قرار داشتند، 88 درصد بیماران با شکایت توده گردنی یا بزرگی غده تیرویید مراجعه کرده بودند. هفتاد درصد بیماران در معاینه جسمی، ندول منفرد داشتند و در 79 درصد موارد در اسکن ایزوتوپ، ندول سرد گزارش شده، بیشترین تشخیص قبل از عمل، ندول سرد بود، در 16.7% بیماران، نتیجه آسیب شناسی بعد از عمل، بدخیم و در 83.3% درصد، خوش خیم بوده است. 23.6 درصد ندولهای سرد، بدخیم بودند و هیچ یک از ندولهای داغ یا گرم بدخیم نبوده اند. با توجه به اهمیت حفظ این غده به منظور پرهیز از عمل جراحی غیر ضروری، توصیه می شود از فنونی همچون اسکن تیرویید و نمونه برداری FNA و سونوگرافی قبل از اقدام به عمل جراحی، استفاده گردد.
The following study was conducted in 300 patients referred to Tajrish Shohada Hospital for a thyroidectomy during 1992-96, to determine the indications for this procedure. In this study of existing data, characteristics such as age, sex, clinical manifestations, results of isotope scanning, preoperative diagnosis and postoperative pathology results were extracted from the patients’ files, and the confidence interval for each variable was estimated in the general population. The number of females was 3.2 times greater than the males. The peak age group undergoing thyroidectomy was 30-40 years (33.3%). In 88%, the chief complaint was a neck mass or enlargement of the thyroid. Physical examination revealed a single nodule in 70%, and in isotope scanning, cold nodules were reported in 79%. The most frequent preoperative diagnosis was cold nodules (45.3%). Postoperaive pathology results showed malignancy in 16.7%, and a benign lesion was reported in the remaining 83.3%. Of the cold nodules reported, 23.6% were malignant, while no malignancy was reported in the hot or warm nodules. Due to the importance of saving this gland and in order to prevent unnecessary operations, we recommend techniques such as thyroid scanning and fine needle aspiration biopsy be performed before attempting surgery.
