Abstract:
به موازات کاهش تمایلات و ایدهآلهای فرزندآوری زنان، تقاضا برای استفاده از روشهای پیشگیری از حاملگی افزایش مییابد. هدف از این مقاله بررسی شیوع کاربرد روشهای پیشگیری از حاملگی و تعیینکنندههای آن در میان زنان همسردار 49-15 سالۀ ساکن در مناطق شهری و روستایی شهرستان ایلام است. دادهها از یک پیمایش مقطعی که در تیر ماه 1393 بر روی نمونهای بالغ بر 724 خانوار انجام شد اقتباس شده است. روش نمونهگیری، ترکیبی از روشهای خوشهای چند مرحلهای، تصادفی و تصادفی سیستماتیک است. نتایج از شیوع گستردۀ استفاده از روشهای پیشگیری از حاملگی در میان زنان شهری و روستایی حکایت دارد. در هنگام بررسی، 81/8 درصد زنان در هر دو نمونۀ شهری و روستائی از یکی از روشهای پیشگیری از حاملگی استفاده میکردند. استفاده از روشهای مدرن پیشگیری از حاملگی در میان زنان شهری از عمومیّت بیشتری در مقایسه با زنان روستائی برخوردار است (75/7 در مقابل 70/7 درصد). تحلیلهای چند متغیّره نشان داد در مناطق شهری شمار فرزندان در قید حیات و هزینههای پیشگیری از حاملگی (18/8 درصد) و در مناطق روستائی شمار فرزندان ایدهآل، شمار فرزندان در قید حیات و هزینههای پیشگیری از حاملگی ( 36/7 درصد) بیشترین نقش را در پیشبینی احتمال استفادۀ زنان از روشهای پیشگیری دارند. نتایج این بررسی گویای این واقعیّت است که در شرایط عمومیّت استفاده از روشهای پیشگیری، کاهش تمایلات و ایدهآلهای فرزندآوری زنان، افزایش هزینۀ دسترسی به خدمات پیشگیری و هزینههای اجتماعی فرهنگی کاربرد روشهای پیشگیری میتواند به افزایش حاملگیهای ناخواسته، سقط جنین عمدی و تهدید سلامت مادران و کودکان منجر شود.
Parallel to reduction in women’s childbearing desires and ideals, demand for using contraceptive methods increase. This paper aims to study of the prevalence of contraceptive use and its determinants among currently married women aged 15-49 living in urban and rural areas of Ilam district. Data are taken from a survey conducted on a sample of over 724 households in July 2014. The sampling method is a combination of multi-stage cluster sampling, random sampling and systematic random sampling methods. Results show that contraceptive use is widespread among women in rural and urban area. 81.8 percent of women in both rural and urban area use a contraceptive method. The use of modern contraceptive methods are more common among urban women than their counterparts in rural area (75.7 percent vs 70.7 percent). Multivariate analysis show that the number of children surviving and costs of contraception (18.8 percent), and the ideal number of children, the number of children surviving, and the costs of contraception (36.7 percent) are the main determinants of women’s risk of contraceptive use in urban and rural area respectively. According to findings it is reasonable to conclude that regarding widespread use of contraceptives and decreasing women’s childbearing desires and ideals, increasing costs of access to contraceptives and sociocultural costs of contraceptive use may lead to unintended pregnancies, induced abortion and maternal health threats.
