Abstract:
زمینه و هدف: طی حداقل نیم قرن اخیر، امید به زندگی در بدو تولد در ایران بهطور قابل توجهی افزایش یافته است، بااینوجود، هنوز اختلاف اساسی بین امید به زندگی در ایران با کشورهای توسعه یافته از جمله ژاپن باقی است. به همین منظور، این مطالعه در تلاش است تا پتانسیلهای افزایش امید به زندگی در ایران را شناسایی کند.
روش: تعداد موارد مرگومیر ثبت شده در ایران بر حسب سن- جنس و علت مرگ برای سال 1394 از نظام ثبت مرگ وزارت بهداشت، درمان و آموزش پزشکی ایران و اطلاعات فوت مربوط به کشور ژاپن برای سال 2015 نیز از وزارت بهداشت، کار و رفاه آن کشور اخذ شد. پس از ارزیابی و تصحیح دادهها، با کاربرد روش تجزیه آریاگا، سهم گروههای سنی و علل مرگ در تفاوت امید به زندگی در بدو تولد دو کشور محاسبه شد.
یافتهها: فزونی امید به زندگی در بدو تولد برای مردان و زنان ژاپن در مقایسه با ایران به ترتیب 2/7 سال و 10 سال بوده است. تفاوت دو کشور در مرگومیرهای کمتر از یکسال در هر دو جنس و همچنین مرگهای سنین سالمندی بهویژه برای زنان، سهم عمدهای در تفاوت امید به زندگی دو کشور دارند. به علاوه، بیماریهای قلبیعروقی، علت اصلی بالاتر بودن امید به زندگی مردان (7/3 سال ) و زنان (5/5 سال) ژاپن نسبت به ایرانیها بوده است.
نتیجهگیری: لازم است سیاستگذاران و برنامهریزان برای بهبود امید به زندگی و سطح سلامت جامعه به کاهش مرگومیر نوزادان و کودکان، کاهش حوادث غیرعمدی بهویژه در سنین جوانی و کنترل و بهبود وضعیت بیماریهای قلبیعروقی در سنین سالمندی توجه کنند.
Background: Since at least half a century ago, life expectancy at birth in Iran has increased significantly, however, there is still a fundamental difference between life expectancy in Iran and developed countries, such as Japan. This study seeks to identify the potential for increasing life expectancy in Iran.
Methods: The number of registered deaths in Iran by age, sex and cause of death for 2015 was obtained from the death registration system of the Ministry of Health and Medical Education of the I. R. Iran and also Japans death information for 2015 was obtained from the Ministry of Health, Labor and Welfare of Japan. After evaluating and analyzing the data, using the Arriga analysis method, the share of age groups and causes of death in the difference in life expectancy at birth of the two countries was calculated.
Results: The increase in life expectancy at birth for Japanese men and women compared to Iran was 7.2 years and 10 years, respectively. The difference between the two countries in infant mortality in both sexes, as well as deaths of old age, especially for women, play a major role in the difference in life expectancy between the two countries. In addition, cardiovascular disease has been the main driver for the lower life expectancy of men (3.7 years) and women (5.5 years) in Japan/
Conclusion: It is essential that policymakers and planners increase life expectancy and community health, by reducing infant and child mortality, reducing unintentional accidents, especially at a young age, and control and improve the condition of cardiovascular disease in old age.
