Abstract:
مقدمه: اختلال نافرمانی مقابلهای یکی از مهمترین اختلالات دوران کودکی و نوجوانی است که از طیف اختلالات رفتار ایذایی و برونیساز میباشد.
هدف: هدف مطالعه حاضر بررسی مقایسهای اثربخشی درمان شناختی رفتاری و هیپنوتراپی بر بهبود مهارتهای تنظیم هیجانی و مهارتهای ارتباطی نوجوانان دارای اختلال نافرمانی مقابلهای بود.
روش: این پژوهش در قالب طرح کارآزمایی بالینی با پیشآزمون و پسآزمون و دوره پیگیری یک ماهه اجرا شد. از میان دانشآموزان مقطع دبیرستان شهرستان ارومیه، با تشخیص اختلال نافرمانی مقابلهای که در سال تحصیلی ۱۴۰۰-۱۳۹۹ مشغول به تحصیل بودند، ۵۱ نفر به شکل هدفمند و با کمک مدیران و معلمان مدارس، والدین و استفاده از مصاحبه تشخیصی و همچنین معیارهای ورود انتخاب و در سه گروه (۱۷ نفر درمان شناختی رفتاری، ۱۷ نفر هیپنوتراپی و ۱۷ نفر کنترل) به شکل تصادفی جایگذاری شدند. سپس گروه شناختی رفتاری هیپنوتراپی براساس پروتکلهای استاندارد مداخلهای تحت مداخله قرار گرفته، اعضای کنترل مداخلهای دریافت نکردند و پس از پایان یک ماه آزمون پیگیری به عمل آمد و مقیاسهای پژوهش مجدداً استفاده شد. ابزارهای این پژوهش شامل: سیاهه رفتاری نسخه والدین (CBCL)، پرسشنامهی تنظیم شناختی هیجان و پرسشنامه مهارتهای ارتباطی بود. برای تجزیه و تحلیل دادهها از نرمافزار SPSSنسخه ۲۲ و آزمون تحلیل واریانس اندازهگیری مکرر استفاده شد.
یافتهها: یافتههای این پژوهش نشان داد که هر دو درمان شناختی رفتاری و هیپنوتراپی در بهبود مهارتهای ارتباطی (۰/۲۲۴>P، ۱/۵۳=F)، مهارتهای سازگارانه تنظیم هیجان (۰/۵۷۵>P، ۲/۴۵=F) و کاهش مهارتهای ناسازگارانه تنظیم هیجان (۰/۱۲۷>P، ۲/۵۶=F) به یک میزان مؤثر واقع شدهاند و تفاوت معناداری بین آنها وجود نداشت.
نتیجهگیری: نتایج پژوهش حاضر حاکی از آن است که درمان هیپنوتراپی و درمان شناختی رفتاری به یک اندازه میتوانند در بهبود مهارتهای تنظیم هیجان و مهارتهای ارتباطی مؤثر عمل کنند و میتوان در کار با نوجوانان دارای اختلال نافرمانی مقابلهای از این مداخلات سود جست.
Introduction: Oppositional Defiant Disorder (ODD) is one of the crucial disorders in childhood and adolescence which is externalizing spectrum disorders.
Aim: The purpose of this study was to investigate of the effectiveness of cognitive-behavioral therapy and hypnotherapy on improving emotional regulation and communication skills of adolescents with oppositional defiant disorder.
Method: This research has been executed in context of a clinical trial plan with pre-test and post-test and one-month follow up period. Among the students with oppositional defiant disorder of junior school in Urmia city who were studying in the (school) year 2020- 2021 that 51 people were purposefully selected and placed randomly in three groups (17 people for cognitive-behavioral therapy, 17 people for hypnotherapy and 17 people for control) with the assistance of schools’ teachers and principals, parents and utilizing diagnostic interviews and also supplying the inclusion criteria in research. Then cognitive- behavioral therapy and hypnotherapy group has intervened based on intervention stranded protocols but members of the control group did not receive any intervention. After the end of a month, a follow-up test was performed and the research scales were used again. Tools of this research included: Child Behavioral Checklist (CBCL), Cognitive Emotion Regulation Questionnaire (CERQ) and the communication skills inventory. SPSS-22 software and repeated measures analysis of variance were used to analyze the data.
Results: The results of this research indicated that both cognitive-behavioral therapy and hypnotherapy improve communication (F=1.53, P>0.224) and adaptive emotion regulation skills (F=2.45, P>0.575) and reduction of maladaptive emotion regulation skills (F=2.56, P>0.127) were equally affected and there was no significant difference between them.
Conclusion: The results of this study signify that both hypnotherapy and cognitive- behavioral therapy can be effective equally in improving emotion regulation and communication skills and these investigations can be used to cope with adolescents with oppositional defiant disorder.
