Abstract:
مهارت ارتباط پزشک با بیمار به هنگام گرفتن شرح حال و معاینه باعث جلب اعتماد بیمار و خانواده ی وی شده، در نتیجه باعث می شود معاینه دقیق تر، تشخیص و درمان صحیح تر صورت گیرد. در این مطالعه، نحوه برقراری ارتباط کلیه مقاطع پزشکی با بیماران هنگام معاینه از دیدگاه بیمار و مشاهده گر با استفاده از پرسشنامه ایوانز مورد ارزیابی قرار گرفت. این مطالعه به صورت توصیفی-مقطعی در سال 1391 انجام گرفت. در تحقیق حاضر، مهارت ارتباطی پزشکان با پرسشنامه ایوانز از پزشکان و بیماران آن ها، با دو پرسشنامه مورد ارزیابی قرار گرفت. گروه اول شامل50 نفر از پزشکان (متخصص، عمومی، دستیار و کارورز) مراکز آموزشی-درمانی دانشگاه علوم پزشکی جندی شاپور اهواز، گروه دوم 50 نفر از بیماران آن ها بودند که به صورت تصادفی ساده انتخاب شدند. توزیع هر دو پرسشنامه با حضور پژوهشگر و به صورت مشاهده و مصاحبه انجام گرفت. اطلاعات به دست آمده با آمار توصیفی و استنباطی ضریب همبستگی اسپیرمن و نرم افزار20.0 SPSS تحلیل شد. چک لیست ایوانز شامل سه مرحله معاینه (شروع، اجرا و پایان) است که در تحقیق حاضر، میانگین شروع معاینه 84/1، میانگین معاینه 20/2 و پایان معاینه 89/1 از 3 امتیاز به دست آمد. در مجموع نحوه ی برقراری ارتباط پزشک با بیمار از دیدگاه مشاهده گر و بیمار 93/1 بود که در سطح متوسط ارزیابی گردید. همچنین بر طبق هر دو دیدگاه، بیشترین نواقص در روند شروع و سپس پایان و در نهایت اجرای معاینه بود. در بعضی موارد، بین دیدگاه بیمار و مشاهده گر همخوانی وجود نداشت. بنابراین نقایصی در مهارت برقراری ارتباط پزشک با بیمار در حین معاینه وجود داشت که با توجه به اهمیت ارتباطات، ضرورت آموزش مهارت ارتباطی در مرکز مهارت های بالینی قبل از تماس با بیمار واقعی آشکار می گردد.
Good communication between physician and patients during history taking andexamination could lead to the both patient and his family' trust. Thus, examination, diagnosis,and treatment will be more precise and more accurate. The aim of this study was toinvestigate all sections of physicians’ communication with patients from the observer andpatient’s view. This study was a descriptive – cross sectional study in 2013. In order to assessthe physician’s communication skill, 3 criteria of the Evans’s questionnaire, two groups(physicians and their patients), and two questionnaires were evaluated. First group (N=50)were specialists, general, residents, and interns, second group consisted 50of their patientswere selected simple-randomly. Distribution of two questionnaires, observation, andinterviews were conducted by the researcher. Data obtained with correlation coefficientsspearman, descriptive, and inferential statistics were analyzed by SPSS 20.0 software. TheEvans’s checklist has three steps for examination (starting, implementing, and ending)through the present study; consequently, the first step (m=1.84), the second step (m=2.20),and third step (m=1.89) were obtained from 3 points and final result was (m=1.93). In total,the performance of physicians in communicating with their patients during visits was atintermediate level (m=1.93) from both the observer and patients’ views. Moreover, accordingto their views, the most areas of deficit were at the beginning (m=1.84) and then ending(m=1.89) and during conducting of the interview (m=2.20), respectively. In the most parts,there was no consistent between observer’s and patient’s views.There are some deficiencies in physicians’ communication skills during history taking andvisits. Considering the importance of communication skills, teaching them before confrontingreal patients would be necessary in Clinical Skills Learning Center.
