Abstract:
هدف: مطالعات نشان میدهد یکی از مسائل مهم در دوره سالمندی انزوای اجتماعی است که تاثیرات نامطلوبی بر سلامت و کیفیت زندگی سالمندان میگذارد. پژوهش حاضر با هدف تعیین شیوع و عوامل مرتبط با انزوای اجتماعی در بین سالمندان شهر تهران در سال 1398 انجام شد.
ابزار و روش: مطالعه حاضر به صورت تحلیل ثانویه با استفاده از دادههای یک مطالعه مقطعی که جامعه آماری آن سالمندان شهر تهران را تشکیل میدادند، انجام شد. برای سنجش انزوای اجتماعی از مقیاس شبکه اجتماعی لوبن LSNS-6 (Lubben Social Network Scale-6) که در بین سالمندان ایرانی روایی و پایایی آن تایید شده است استفاده شد. اطلاعات نمونهها به لحاظ ویژگیهای دموگرافیک (سن، جنس، وضعیت تاهل، ترتیبات زندگی و تعداد فرزندان)، اقتصادی-اجتماعی (سطح تحصیلات، وضعیت اشتغال و میزان درآمد) و وضعیت سلامت سالمندان (بیماریها و استفاده از وسایل کمکی) بررسی شد. دادهها با استفاده از نرم افزار SPSS نسخه 23، در سطح دو متغیره و چند متغیره مورد تجزیه و تحلیل قرار گرفتند و سطح معناداری کمتر-مساوی 0. 05 برای تحلیل دادهها در نظر گرفته شد.
یافتهها: مطالعه حاضر بر روی 1280 سالمند 60 سال و بالاتر با توزیع جنسی تقریبا مساوی انجام شد. میانگین سنی پاسخ دهنده گان 70. 97 سال (SD=8. 07) بود و حدود 40 درصد افراد بازنشسته و 12. 4 درصد به تنهایی زندگی میکرند همچنین شیوع انزوای اجتماعی 30. 8 درصد محاسبه شد. نتایج حاصل از تحلیلهای دو متغیره با استفاده از آزمون کای اسکوئر نشان داد که متغیرهای؛ ترتیبات زندگی (P2=46. 93)، وضعیت تاهل (P-Value2=26. 91)، درآمد خانوار (P2=67. 44)، سطح تحصیلات (P2=20. 26)، وضعیت اشتغال (P2=29. 21)، مشکلات اسکلتی-عضلانی (P2=15. 47)، مشکلات تنفسی (P2=10. 43)، مشکلات بینایی (P2=4. 4)، چربی خون بالا (P2=13. 53) و استفاده از عینک (P 2=3. 92) رابطه معنی داری با انزوای اجتماعی داشتند. و نتایج حاصل از تحلیل رگرسیون لجستیک نشان داد که مدل بطور معناداری (X2 (26) = 188. 35, P انزوای اجتماعی را پیش بینی میکند. همچنین جنس (مرد بودن) (P زندگی با همسر (P بیماریهای قلبی (P، دیابتOR=1. 41) (P، درآمد خانوار (P غیر شاغل بودن (P، چربی خون بالا (P و زندگی در مناطق برخوردار (مناطق توسعه یافته) (P رابطه معناداری با انزوای اجتماعی دارند.
نتیجهگیری: با توجه به وجود ارتباط قوی که انزوای اجتماعی با بعضی ویژگیهای اقتصادی-اجتماعی، برخی بیماریهای مزمن و تعدادی از ویژگیهای جمعیت شناختی داشت، سیاستگذاران و متخصصین حوزه بهداشتی و سلامت میتوانند برنامههای غربالگری و پیشگیری کننده مربوط به سلامت روانی و اجتماعی سالمندان تهرانی را در جهت تعدیل و کنترل این عوامل تدوین کنند.
Objective: Studies show social isolation is one of the most important issues in aging that negatively affects quality of life and health of older adults. The present study was conducted to determine the prevalence and associated factors of social isolation among older adults in Tehran in 2020.
Tools and Methods: Data for this secondary analysis study were obtained from a large cross-sectional study conducted on older adults in Tehran in 2020. Data collection was conducted from January to March 2020. The Persian validated version of Lubben Social Network-6 Scale (LSNS-6) was used to measure social isolation. The demographic characteristics (age, sex, marital status, living arrangements and number of children), socio-economic status (level of education, employment status and household income) and health status of the elderly (chronic medical conditions and use of assistive devices) were collected using checklist.
Data were analyzed using SPSS software version 23 at the bivariate level and multivariate logistic regression. The significance level was considered at p≤0.05.
Results:The study sample consisted of 1280 older adults aged 60 years and older with an almost equal sex distribution. The mean age of the respondents was found to be 70.97 (SD=8.07) years. In terms of employment status 40.2% of the sample were retired. In terms of living arrangement about 12.4% of the respondent were living alone. The prevalence of social isolation was found to be 30.8%. The results of bivariate analysis using a series of chi-square tests revealed that living arrangement (P2=46.93) , marital status (P2=26.91) , household income (P2=67.44) , level of education (P2=20.26) , employment status (P 2=29.21) , musculoskeletal problems (P2=15.47) , respiratory problems (P2=10.43) , visual problems (P2=4.4) , hyperlipidemia (P2=13.53) , and glasses use (P 2=3.92) were significantly associated with social isolation. Conclusion: The results of the present study revealed that chronic medical conditions, socioeconomic and demographics characteristics significantly contribute to social isolation imply that policymakers and health professionals make appropriate plans such as screening and preventive programmes to reduce and control contributing risk factors of social isolation among older adults in Tehran.
