Abstract:
مقدمه: اختلال استرس پس از سانحه، یک اختلال شدید روانشناختی است که پس از تجربه یک رخداد استرسزا و آسیبزا بهوجود میآید. هدف پژوهش حاضر تعیین اثربخشی مواجهه درمانی مداوم بر تابآوری و کاهش نشانه های بالینی دختران آزار دیده ی جنسی مبتلا به اختلال استرس پس از آسیب بود. روش: طرح پژوهش تک آزمودنی از نوع A-B با دوره پیگیری بود. جمعیت آماری پژوهش را کلیه دختران 12 تا 18 سال مبتلا به اختلال استرس پس از آسیب ناشی از آزار جنسی مراجعهکننده به مرکز اورژانس اجتماعی شهرستان ایذه در سال 1399 تشکیل دادند. دو نفر به شیوه نمونهگیری هدفمند انتخاب شدند. برای هر کدام از شرکت کنندگان 3 جلسه خط پایه در نظر گرفته شد. جلسات مداخله به صورت انفرادی بر اساس برنامة مواجهه درمانی مداوم در 10 جلسه 90 دقیقه ای انجام گرفت. ارزیابیها با استفاده از مقیاس نشانگان اختلال استرس پس از سانحه کودکان (فوآ و همکاران، 2018) و پرسشنامه تاب آوری (کانر و دیویدسون، 2003) در مرحله خط پایه، جلسات مداخله و یک ماه بعد از پایان درمان (پیگیری) انجام گرفت. تجزیه و تحلیل دادهها با استفاده از تحلیل بصری نمودار، محاسبه درصد بهبودی و شاخص درصد پایا انجام شد. نتایج: یافته ها نشان داد هر دو مراجع با شروع مداخله روند کاهشی و رو به بهبودی را در طراز نمرات مقیاس نشانگان اختلال استرس پس از سانحه و روند افزایشی و رو به بهبودی را در طراز نمرات مقیاس تابآوری نشان دادند. آزمودنیها این نتایج را تا یک ماه پس از درمان حفظ نمودند. بحث و نتیجهگیری: براساس یافتههای پژوهش پیشنهاد میشود که درمانگران حوزة کودک و نوجوان در افزایش تاب آوری و بهبود علایم ناشی از استرس از مواجهه درمانی مداوم برای اثربخشی بیشتر استفاده کنند. این یافتهها تلویحات مهمی در زمینه آموزش و ارتقای بهداشت روانی کودکان ارائه میکنند.
Introductoin: Post-traumatic stress disorder is a severe psychological disorder, which arises due to undergoing a stressful and traumatic event. The present study aimed at identifying the effectiveness of Prolonged Exposure therapy in resilience and reducing the clinical symptoms in sexually abused girls with post-traumatic stress disorder.Method: The research design was a single subject of A-B type with a follow-up period. The statistical population of the study included all 12 to 18-year-old girls with post-traumatic stress disorder due to sexual harassment in Izeh city who were referred to the Social Emergency Center in 2020. Two of them were selected through purposive sampling. Three baseline sessions were considered for each subject. Intervention sessions were performed individually based on a prolonged exposure therapy program in 10 sessions of 90 minutes (2 days a week).Evaluations were performed by the PTSD Symptom Scale (Foa et al., 2018) and resilience questionnaire (Connor and Davidson, 2003) in three stages baseline, intervention sessions, and one month following the treatment. Data analysis was done using visual analysis of graphs, calculation of recovery percentage, and reliable percentage index.Results: Findings proved that with the beginning of the intervention, both subjects showed a decreasing and improving process in the scores of post-traumatic stress disorder syndrome, and an increasing and improving process in the scores of Connor and Davidson resilience scales. Subjects preserved these results for up to one month following the treatment.Discussion and Conclusion: Based on the findings, it is recommended that therapists in the field of children and adolescents use prolonged exposure therapy to increase resiliency and improve stress-made symptoms to achieve more effectiveness. These findings provide important implications for educating and promoting children's mental health.
